Book excerpt: Background: Childhood obesity accompanied by low health-related physical fitness (HRPF) is a major threat to public health, both internationally and locally. Children with intellectual disability (ID), especially adolescents, have a high risk of being overweight or obese and of having poor HRPF. More interventions, therefore, are needed to help this population attain their optimal health levels. However, few studies have examined this population, in contrast to the numerous studies designed for their typically developing (TD) peers. The purpose of this study was to design a physical activity (PA) intervention for this under-researched population and to evaluate its effectiveness on obesity-related outcomes (primary), HRPF outcomes (primary), and blood pressure (secondary) as well as its sustained effects. Methods: First, a systematic review and a meta-analysis of lifestyle intervention studies on obesity and HRPF were conducted to identify effective intervention strategies for the target population and to provide evidence for designing the PA programme. Three databases were systematically searched for studies published until May 2020. The random effects model was used to pool the weighted results with inverse variance methods, and the I2 statistic was applied to assess heterogeneity among the included studies. Second, a school-based randomised controlled trial (RCT) was implemented for 12 weeks. Participants (N = 30) aged 12–18 years were recruited and randomly allocated to either the intervention group (IG) or the waitlist control group (CG). The participants in the IG received a fun game–based moderate-to-vigorous PA (MVPA) training program (2 sessions/week, 60 minutes/session, for a total of 24 sessions). The intensity of the activities increased in a progressive manner. Participants in the CG received no treatment during the 12-week intervention period. All participants were followed up for 12 weeks afterwards to evaluate the sustaining effects of the intervention. Study outcomes included primary outcomes (obesity- and fitness-related outcomes) and a secondary outcome (blood pressure). All measurements were repeatedly taken at baseline, after intervention, and after follow-up. Analysis of variance for repeated measures was applied to detect within-group differences in the measured outcomes across three time points, whereas analysis of covariance was performed to evaluate the between-group differences. Results: The systematic review and meta-analysis revealed that PA was the predominant intervention component, as it was adopted in 27 of the 29 examined studies. A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

However, such studies are still relatively rare. This research topic is intended to encourage more relevant projects to be published. This research topic aims to encourage researchers to use biomechanical performance to design advanced physical medicine and rehabilitation approaches, evaluate the effectiveness of the rehabilitation approaches, and explore the mechanisms by which rehabilitation approaches work for neuromusculoskeletal disorders. Some studies have developed stretching approaches for the rehabilitation of knee osteoarthritis in older adults by measuring biomechanical performance during functional activities. Some studies indicated that the mechanism of physical activity to reduce falls in older adults lies in its effectiveness in increasing proprioceptive sensitivity, and further indicated that rehabilitation of proprioception may be a key to reducing falls in the fall-prone older adult population. Some other studies analyzed biomechanical performance in ankle ligament injuries to understand when, how, and why ligaments fail. As a result, this research topic will expand the application of biomechanical performance to better understand and treat neuromusculoskeletal disorders.

Book excerpt: Increasing numbers of children and adolescents internationally are being diagnosed with secondary health problems (e.g., overweight-obesity, diabetes, asthma, anxiety, etc.) due in part, or at least related to, a lack of physical activity. Children and adolescents with various forms of special needs (for example, children and adolescents with physical or intellectual disabilities, children and adolescents from disadvantaged social backgrounds and children and adolescents with chronic illnesses) seem to be particularly at risk for secondary health problems, which in the end limit their social participation and inclusion, as well as their ability to achieve their full potential and to lead happy and fulfilling lives. For these children and adolescents, involvement in regular physical activities (including fitness activities and sports) may have far reaching benefits. For instance, organized physical activities are known to represent an effective vehicle for interventions for children and adolescents with special needs who do not seem to benefit as much as others from more traditional, verbal-oriented approaches. Organized physical activities (in or out of school) further provide these children and adolescents with opportunities to interact in a positive manner with prosocial peers and adults who may serve as positive role models for them. There is currently a paucity of research about physical activities that effectively include children and adolescents with a range of special needs or research that identifies evidence-based strategies that seed success in maximizing the involvement in, and the positive biopsychosocial outcomes associated with, the practice of physical activity. This dearth of research is impeding progress in addressing the biopsychosocial disadvantage that these children and adolescents encounter, the development of new solutions for enabling full potential, and ensuring that children and adolescents with special needs not only succeed, but also flourish in life. This volume includes examples of theory, research, policy, and practice that will advance our understanding of how best to encourage these children and adolescents to participate regularly in physical activity, how to maximize the biopsychosocial benefits of involvement in physical activities, and how to ensure that these physical activities are inclusive for children and adolescents with special needs. The focus will be placed on research-derived physical activity practices that seed success for children and adolescents with special needs, and new directions in theory, research, and practice that have implications for enhancing physical activity practices with at-risk children and adolescents. Six adolescents with visual impairments attending a Midwestern residential school for the blind served as participants. The study implemented a multiple baseline across participants single subject design and physical activity behavior was measured by steps taken through the use of Fitbit Zips. In addition to physical activity, questionnaires were used to collect data (i.e., change scores) on selected social cognitive theory constructs. Data were analyzed through visual analysis and descriptive statistics. Results of the multiple baseline design did not demonstrate a functional relation between the number of steps taken and the intervention. These results may have been directly influenced from several limitations of the study, such as seasonality (i.e., the program took place in the winter time where limited leisure-time physical activity opportunities were available) and the truncation of the program for Participants 4, 5, and 6. Because of these limitations, the results may not represent the full effectiveness of the intervention for individuals with visual impairments. With the lack of physical activity intervention research related to adolescent-aged individuals with visual impairments (Haegele & Porretta, 2015), and the influence physical activity has on health-related outcomes (e.g., obesity), further research is needed.

Book excerpt: Overweight and obesity are associated with an array of negative physical and psychosocial symptoms in adolescents. Numerous pediatric obesity interventions have emerged in an attempt to address this significant public health concern. However, few have focused on African American females, a particularly high risk group for negative health behaviors and associated complications. Moreover, research examining the psychosocial changes associated with increases in physical activity and fitness, although promising in elder and adult populations, is nearly nonexistent in adolescents. Thus, the current study addressed limitations in the research and examined the psychosocial outcomes of adolescent participants in T.E.E.N.S., a culturally sensitive, multidisciplinary weight loss intervention. From baseline to 6 month post-testing, program participants demonstrated significant improvements in physical, emotional, and social functioning, as well as body dissatisfaction. Moreover, increases in physical activity and cardiorespiratory fitness were associated with significant enhancements in several domains of psychological well-being.
