Book excerpt: Increased incidence of and survival from breast cancer have resulted in growth of the number of women who have survived this disease and are faced with the subsequent consequences of their diagnosis and treatment. Physical activity (PA) is a modifiable health behavior that has the potential to address both the emotional and physical needs of women with early stage breast cancer. However, for PA to be seen as a viable treatment option, and for a change in routine care to occur, its effectiveness must be determined. Thus, the objectives of this pilot study were to: 1) quantify the effect of a 12-week homebased walking intervention on quality of life, body composition, and estrogen metabolism in survivors of breast cancer, and 2) develop and test the feasibility of PA intervention materials for future studies in this population. A total of 33 participants were enrolled and 21 of these were randomized to the walking program. No participants met inclusion criteria for the estrogen metabolism sub-study. Completion of counseling calls and adherence to the walking prescription were high among the walking group. Medium to large increases in PA as well as medium improvements in many domains of health-related quality of life were found among walking but not control participants. Home-based walking programs appear to be a feasible and promising form of intervention for breast cancer survivors.

Book excerpt: Inhaltsangabe:Abstract: Research in the field of exercise therapy for cancer patients primarily focused on individuals with breast cancer who receive exercise interventions under outpatient conditions. This research study investigated individuals with colorectal cancer under inpatient conditions and aimed to broaden the application of exercise intervention treatments beyond that of breast cancer. The research included two study groups; standard exercise group (SEG, n=44) receiving indoor gymnastic exercise (IGE) and modified exercise group (MEG, n=39) receiving IGE plus additional outdoor walking activity. Participation in the specific exercise therapy program was independent of other therapies received in the inpatient rehabilitation program (IRP). The patients completed QOL questionnaires (QLQ-C30 & QLQ-CR38), psychological distress scale (HADS) and underwent treadmill stress-testing (TST) at the beginning (T1) and upon completion (T2) of the IRP. The QOL and the HADS questionnaires were administrated after a six month follow up (T3) proceeding the IRP completion. Results indicate significant QOL and HADS improvements in pre to post-testing for both study groups. The TST results indicate superior improvements in the MEG in comparison to the SEG. Some of the T1 to T2 QOL and HADS improvement maintained at T3. We conclude that 3 weeks IRP was adequate to reveal improvements in QOL, psychological measures and functional capacity. Future research should emphasize patient motivation for participation in physical activity after completing IRP. This background chapter encompasses the different fields of knowledge which are relevant to the present study, starting with colorectal cancer (epidemiology, etiology, pathology and the International Classification of Functioning, Disability and Health), continuing with the subjects Quality of life, Movement and Sports in the Rehabilitation. Descriptive epidemiological knowledge of colorectal cancer is essential for understanding the etiology of the disease and is used in the process of developing screening methods. All subjects received instruction to follow an eight-week, home-based exercise program. Two preliminary analyses have been conducted. The first examined the relationship of exercise to fatigue and quality of life in 31 subjects. This analysis demonstrated that the effect of exercise was strongly mediated by fatigue and that fatigue accounted for 71% of the variance in quality of life. The second analysis examined the pattern of fatigue over the first three cycles of chemotherapy in 68 subjects. Results indicated a curvilinear relationship between fatigue and duration of exercise; as the number of minutes of exercise increased fatigue declined. Findings from both analyses support exercise as an intervention for fatigue in women with breast cancer receiving chemotherapy. The results indicate that while there appears to be a dose response relationship of exercise to fatigue, fatigue appears to be the mechanism through which exercise affects quality of life. This research indicates that exercise is important to help maintain functional ability, reduce fatigue and improve quality of life during breast cancer treatment.

Thirty-one subjects have been enrolled in the 8-week home based exercise study. Subjects receive weekly telephone follow-up calls to remind them to return exercise logs and collect data on cancer treatment-related fatigue, symptoms, and affect. No adverse events have been observed, and subject recruitment is progressing as planned.

Book excerpt: [Truncated abstract] Previously, research has demonstrated that cancer patients and survivors, through participation in moderate intensity exercise, can achieve significant reductions in disease and adjuvant treatment morbidities. Of concern, is that current research findings have shown that cancer survivors are not exercising at levels sufficient enough to attain health benefits, yet the reasons for this behaviour pattern are not well understood. Therefore the research in this thesis has been conducted from the perspective of Self-Determination Theory (SDT) in order to examine the exercise motivations of Western Australian breast cancer survivors, and the impact that these motivations had on exercise behaviour and quality of life (QOL) outcomes. ... Strong positive correlations were observed between the exercise regulations and the psychological needs across all 58 participants at baseline and weeks 12 and 24. Significant time by group interactions were demonstrated for all psychological needs variables and exercise regulations except for amotivation. Furthermore, from baseline to week 12, the IEG demonstrated improvements on psychological needs with corresponding significantly more self-determined exercise regulations. Conversely, no changes were observed for the DEG. From week 12 until week 24, the DEG demonstrated significant increases in perceived psychological needs and reported more self-determined exercise regulations. Psychological needs and exercise regulations measured at baseline did not predict adherence to the exercise programme. Overall, SDT assisted in explaining the motivation / exercise relationship for cancer survivors. Perceived autonomy and competence positively influence the development of identified and intrinsic motivations, which are significant factors for the adoption and maintenance of an exercise programme. Identification of these factors, as well as determining patients readiness to exercise, will assist in the development of future exercise programmes. Of importance, is that these programmes should be tailored to an individual s needs and well-being. Increased exercise levels may reduce the impact of treatment morbidities for cancer patients and survivors, as well as improve their QOL. Recommendations from this research are provided for health professionals, whilst recommendations for future research are also offered.
