Book excerpt: " Central line-associated bloodstream infection (CLABSI) is among the groups of hospital-acquired infections (HAIs). CLABSI carries an increased risk of morbidity, mortality, increased hospital length of stay, and is very expensive to treat. The literature reviewed recommended the Centers for Disease Control and Prevention (CDC) guidelines and checklist bundle system as an effective method in CLABSI prevention; however, this required strict adherence and compliance to be effective. The project manager (PM) implemented the quality improvement project in a 14-bed surgical intensive care unit (SICU) for ten weeks. A pre-test questionnaire based on CDC guidelines tested the nurses' knowledge. A CDC checklist was provided to the nurses to monitor compliance with the bundle and followed by an educational intervention presentation. A post-test questionnaire score was used to assess knowledge gained. Compliance with the insertion and maintenance bundle was collected and tabulated. CLABSI rate for SICU for the same period was 1 case for a total of 197-line days, equal to an infection rate of 5.08 higher than the national benchmark of 1.53, compared to the other units was 5 cases for a total of 1,497 line days resulting in an infection rate of 3.34 for level 1a hospital. Results indicated a post-test knowledge of 91.6%, exceeding the facility benchmark of 80% for competency education. Compliance with all the bundle components was 100%. However, SICU had one CLABSI case in January and none in February. These findings indicate that CLABSI prevention using the bundle system is effective in reducing and preventing hospital-acquired infections. Gaining knowledge on all the bundle components fostered compliance and increased quality patient care outcomes. Nurses played an integral part in CLABSI prevention. Book excerpt: Central line associated blood stream infections (CLABSI) are still causing patients in the intensive care unit (ICU) to spend longer time in the ICU, as well, it adds an increased cost of care to the patient, and for the medical center involved. While these central lines (CL) are important, they do not come without possible complications. The use of guidelines from the CDC, can help with the implementation of a CL bundle, that when combined with continuing education for nurses and physicians, will help to decrease the number of CLABSI in the ICU. This evidenced-bases data supports continued education and a CL bundle to improve patient outcomes. The data is used to persuade administration to make a change in policy and patient care. By using theory for change as guidance, the theory from Kurt Lewin will best fit, as it has a focus of forgetting what you have learned, and be open and accepting of new information and change for the good. Continued education of staff with the use of posters in the ICU, PowerPoint Self-learning on the computer, and the use of pretests and posttests to assess the knowledge of the staff will be used. The use of a CL bundle checklist will be used to make sure all areas of the bundle are followed. This includes chlorhexidine gluconate (CHG) scrub, hand washing, line placement choice, aseptic technique, and daily evaluation of line necessity. Testing will be done prior to education and then again after. Nursing educators and the infection control nurses will follow this implementation, using the data received from the staff after seven months after implementation of the plan. Make any changes to the plan that these results indicate necessary for improved patient care. The identified key stakeholders, administrators, physicians, nurses, and all CL insertion team staff will be able to review this data, for possible use by other facilities in the nursing community.

Book excerpt: Based on documented studies on the topic of central line-associated bloodstream infections (CLABSI), it is evident that more can be done to prevent them. Traditionally, central lines were only prevalent in intensive care settings, but have quickly become common other inpatient (Tedja, Gordon, Fatica, and Fraser, 2014) and outpatient settings as well (Saffer, 2012). CLABSI occurrences are costly; multiple studies show that meticulous, streamlined and intentional care can prevent CLABSI 100% of the time (Jackson and Cooper, 2012, and Matocha, 2013). To increase knowledge for staff and patients, and decrease CLABSI occurrences, this author proposes a new approach to education about CL care. Staff will be reeducated on CL care and patients will be educated by staff on care of CL while inside and outside the inpatient setting.

Book excerpt: This Open access book offers updated and revised information on vessel health and preservation (VHP), a model concept first published in poster form in 2008 and in JVA in 2012, which has received a great deal of attention, especially in the US, UK and Australia. The book presents a model and a new way of thinking applied to vascular access and administration of intravenous treatment, and shows how establishing and maintaining a route of access to the bloodstream is essential for patients in acute care today. Until now, little thought has been given to an intentional process to guide selection, insertion and management of vascular access devices (VADs) and by default actions are based on crisis management when a quickly selected VAD fails. The book details how VHP establishes a framework or pathway model for each step of the patient experience, intentionally guiding, improving and eliminating risk when possible. The evidence points to the fact that reducing fragmentation, establishing a pathway, and teaching the process to all stakeholders reduces complications with intravenous therapy, improves efficiency and diminishes cost. As such this book appeals to bedside nurses, physicians and other health professionals.

Book excerpt: Utilization of central venous catheters (CVC) is common in the intensive care units (ICUs). Central intravenous lines on patients admitted to this critical care setting provide intravenous access for medications, total parenteral nutrition, and for hemodynamic monitoring. Although CVCs aid hospital staff in providing efficient care for these critically ill patients, these tools also place the patients at risk for central line-associated bloodstream infections (CLABSI). CLABSI, a nosocomial infection, is a major source of morbidity and mortality. It is also associated with increased cost of care for healthcare facilities across the country, averaging $45,000 per single infected patient. (Hakko, Guvenc, Karaman, Erdem, and Cakmakci, 2015) This project proposal calls for an implementation of a six-month pilot study to be conducted in the facility's medical intensive care unit to adopt a new updated central line management bundle. The new bundle will incorporate the utilization of disinfectant port protectors or alcohol-impregnated port protectors as a measure to prevent bloodstream infections. The utilization of disinfectant port protectors, which are saturated with alcohol, has been proven by several published research studies in preventing central line-associated bloodstream infections. In addition to the pilot program, facility-wide education will be conducted to educate nurses and other allied healthcare personnel about central line-associated bloodstream, central line management bundle, and prevention strategies. Educating healthcare workers about CLABSI and proper maintenance of central intravenous lines is primary prevention strategy, which can aid decrease the facility's CLABSI rate, and also decrease the facility's unnecessary expenditures of treating these infections. It is also important to disseminate these possible results, so that nurses know the value of compliance and strict adherence to protocols of central line management care in preventing CLABSI.

Book excerpt: Central line catheter-associated bloodstream infections (CLABSI) are a common occurrence in Intensive Care Units. These hospital acquired infections can be costly and lethal but most importantly, preventable. This research project's purpose is to analyze several studies demonstrating how central line-associated infections are preventable. The methods used for this project was a collaborative study examining the interventions that are incorporated in the On The CUSP: Stop BSI program through various research articles and how these interventions decreased central line-associated bloodstream infections in the Intensive Care Unit. This comprehensive unit-based safety program was developed with the goal of preventing bloodstream infections and provides a framework to educate health care professionals that are in direct care of patients with central lines. Physicians, physician assistants, registered nurses, radiology technicians, and other bedside care providers follow a collaborative five step process. The five step process as designed by On the CUSP: Stop BSI are to educate regarding the safety techniques, implement teamwork and communication tools, identify defects, address the defects, and collaborate with a unit leader for guidance. The infection rates are monitored before, during, and up to twelve months after the interventions were initiated. When these improvement efforts were evaluated, the data showed a significant decrease in central line-associated bloodstream infections. The conclusion is that implementing the On the CUSP: Stop BSI program produced significant and sustaining data that central line-associated bloodstream infections decreased in diverse ICU settings through multiple cohort studies that utilized these evidence-based prevention bundles over a year to two year time frame.

Book excerpt: Infections are among the most frequent complications in patients with hematological malignancies and in those undergoing high-dose chemotherapy and autologous hematopoietic stem cell transplantation. A profound knowledge on the epidemiology, diagnostic approaches, treatment modalities and prophylactic strategies is essential for the clinical management of these complications in patients who are often severely immunocompromised owing to their underlying diseases and in particular, the intensive myelosuppressive chemo and immunotherapy. This textbook provides a clinically oriented, compact and up-to-date overview on infections in hematology patients and their management. The typical pathogens to be considered in different subgroups of patients are identified and further aspects of the microbiological background are explored. Clinical, imaging, and laboratory-based diagnostic techniques are discussed and therapeutic strategies appropriate to different situations are then presented, with due attention to the pitfalls, toxicities and interactions that can arise during antimicrobial treatment. Strategies to prevent infection are also outlined, encompassing antimicrobial prophylaxis, isolation procedures, hospital hygiene, protective immunization and the use of hematopoietic growth factors.

Book excerpt: Intensive care is a rapidly changing area of medicine, and after four years from the 2nd edition the volume editors and authors have deemed necessary to update it. In the recent years, in fact, five new randomised controlled trials and five new meta-analyses demonstrate that selective decontamination of the digestive tract [SDD] is an antimicrobial prophylaxis to prevent severe infections of not only lower airways but also of blood. Additionally, SDD has been shown to reduce inflammation including multiple organ failure and mortality. An intriguing observation is the evidence that SDD using parenteral and enteral antimicrobials reduces rather than increases antimicrobial resistance. Moreover, a new chapter on microcirculation had been added. The volume will be an invaluable tool for all those requiring in depth knowledge in the ever expanding field of infection control.

Book excerpt: Despite of many changes adapted by healthcare providers in the past decades, the incidences of central line associated blood stream infections (CLABSIs) are still occurring in the intensive care units (ICUs) and non-ICU settings. The failure to remove unnecessary central lines in a timely manner has resulted in many CLABSIs. This issue has been noted due to lack of communication between the staff and the providers. The bundle approach has been widely used, but there is a need to implement new measures that would enhance communication between all those who are involved in the care of the patient. Educating staff and creating a tool in the electronic medical recording (EMR) system, as well as a handoff tool to nurses would ensure that the central lines that are not in use anymore or not needed will be discontinued. These tools will help prevent CLABSI, therefore will decrease healthcare associated infections, decrease patient mortality and morbidity, decrease length of stay and cost to the facilities.

Book excerpt: "Patients in pediatric intensive care units (PICUs) with central venous lines (CVLs) are at risk for developing catheter-related bloodstream infections (CRBSIs). Adverse consequences to patient health resulting from these infections can negatively impact patient outcomes, prolong patient length of stay, and increase the overall cost of care. The author implemented an evidence-based practice protocol using a team approach in a 12 bed PICU using a care bundle to prevent central line infections. The purpose of this change project was to educate PICU staff caring for patients with central lines about utilization of bundle elements to reduce risk of CRBSIs. The goal was to implement each step of the bundle when caring for every patient with a CVL during the eight week change project. The author measured staff compliance to bundle elements including hand hygiene, maximal barrier precautions, chlorhexidine skin antisepsis, and daily review of line necessity. Weekly audits demonstrated 100% provider adherence to care bundle interventions over the implementation phase. In accordance with evidence from similar studies, these findings suggest compliance with bundle interventions can lead to significant reduction in the rate of catheter-related bloodstream infections. Keywords: central line, catheter-related bloodstream infections, care bundle, pediatric " -- Abstract.

Available in PDF, EPUB and Kindle. Book excerpt: Central venous catheters also known as central lines are long, thin, flexible tubes that provide medicine, food and nutrients to patients over time (WebMd, 2010). They are also used to monitor and maintain adequate tissue perfusion, infusions and transfusions(CDC, 2011). These catheters provide immense benefit and as so, they are being utilized in the settings of acute care more and more. Yet, with these benefits come complications and one that is the most common is infections. These are infections are commonly known as (CLABSI) or Central Line Associated Bloodstream Infections. The primary reason for the complications is due to lack of proper insertion techniques, cleanliness and lack of management after insertion (CDC, 2011). The issues that are related to this complication of infections are morbidity, mortality, longer hospital stays, and financial costs to the patient and the hospital (Jessica and Robin, 2010). These infections are estimated to range in the levels of about of 41,000 on annual basis (CDC, 2013). These infections can cause longer stays in the hospital and at least an added $30,000 per happening. CLABSI costs healthcare organization in the USA over $2 billion on a yearly basis (Guerin and others,). The situation is so extreme that several respected institutions have labeled and added CLABSI to their never event list (Miller-Hoover, 2011). It is up to providers of health care to be able to assess the situation and finding ways to prevent these infections. This increases the potential of higher quality in the outcomes of patients and decreased costs overall. The review of literature leads the author to believe that one of the best techniques in preventing CLABSI is through the use of a suggestion by the IHI. This suggestion is about the use of the (CLB) or the Central Line Bundle. (CLB) is comprised of specific suggestions and tips that target any issues that might cause the infections. These tips cover aspects of where exactly the catheter must go, the selection of the site, protocols for hygiene, utilization of (chlorhexidine) to cleanse the dermis, high barrier safety measures, and constant management of the line (Royer, 2010). The aim of this project is to prevent CLABSI through the utilization, implementation and maintenance of EBP of the CLB in the (ICU) at Baylor University Medical Center in Dallas (BUMC). Favorable outcomes are to be ensured through the proper implementation of the CBL through rigorous planning, execution, attainment of resources and the education of staff involved.
