Book excerpt: " Central line-associated bloodstream infection (CLABSI) is among the groups of hospital-acquired infections (HAIs). CLABSI carries an increased risk of morbidity, mortality, increased hospital length of stay, and is very expensive to treat. The literature reviewed recommended the Centers for Disease Control and Prevention (CDC) guidelines and checklist bundle system as an effective method in CLABSI prevention; however, this required strict adherence and compliance to be effective. The project manager (PM) implemented the quality improvement project in a 14-bed surgical intensive care unit (SICU) for ten weeks. A pre-test questionnaire based on CDC guidelines tested the nurses' knowledge. A CDC checklist was provided to the nurses to monitor compliance with the bundle and followed by an educational intervention presentation. A post-test questionnaire score was used to assess knowledge gained. Compliance with the insertion and maintenance bundle was collected and tabulated. CLABSI rate for SICU for the same period was 1 case for a total of 197-line days, equal to an infection rate of 5.08 higher than the national benchmark of 1.53, compared to the other units was 5 cases for a total of 1,497 line days resulting in an infection rate of 3.34 for level 1a hospital. Results indicated a post-test knowledge of 91.6%, exceeding the facility benchmark of 80% for competency education. Compliance with all the bundle components was 100%. However, SICU had one CLABSI case in January and none in February. These findings indicate that CLABSI prevention using the bundle system is effective in reducing and preventing hospital-acquired infections. Gaining knowledge on all the bundle components fostered compliance and increased quality patient care outcomes. Nurses played an integral part in CLABSI prevention.
