Implementing a plan for hourly rounding is necessary to improve patient safety, involving nurses checking on patients and observing patient's rooms and surroundings. After the plan is implemented, an evaluation of the process is required to determine if the process or plan has been effective. Evaluation is often skipped over in EBP research, however it is necessary to determine the effectiveness of the plan and to let others know the results of the trial. Shortly after the plan is evaluated, it should be disseminated to other nurses, stakeholders and the public in order for the information to be useful. Dissemination is also important because it allows nurses to see results of studies in a quick, efficient way without having to spend hours looking up the research themselves.

The patient population consists of cardiovascular and thoracic post-operative surgical patients of all ages primarily adults. The implementation of the evidence based practice of hourly rounding has been proven to increase overall patient satisfaction by improving staff responsiveness and increasing patient safety within many hospitals. The measurement of effectiveness and success will be evidenced by seeing an increase in patient care scores and a reduction of falls within the unit; when these metrics are met, there is data that proves that this evidence based practice will drive the results we are looking for. In this microsystem, there are many indicators that a more efficient workflow is necessary to be established. There is the constant sound of call lights ringing about an estimated 12-15 in an eight hour shift for non-urgent needs. Upon interviewing many staff, I found there was an inconsistent way and lack of education on how hourly rounds should be performed. Basic patient needs were not addressed within each visit from a staff member. By using a process map, timeline, PDSA, Lewin's Change Theory, SWOT analysis and graphs, I can show you my process of improving staff responsiveness within the nursing unit. In conclusion, as a Clinical Nurse Leader and patient advocate, I have utilized the information gained from the microsystem data and implementation of hourly rounding, thereby proving how to improve quality, safety and patient care experiences within the busy, medical surgical nursing unit with the evidence based practice of hourly rounding.

The staff can also benefit by reinforcing the importance of hourly rounding through the yearly house side mandatory review session accessed through the hospital's intranet website. This proposal will include an implementation plan that shows the positive benefits implementing hourly rounding re-education given to nurses and nursing assistants has improved patient outcomes, specifically improved patient safety as seen by a decrease in falls, and improved patient satisfaction. In addition, evaluation of the findings as well as disseminating them to all unit department directors and the nursing community will be discussed.

It was observed that the role that nurses and other direct care givers play in giving the best care to the patient plays a huge role in patient satisfaction as well as willingness to recommend the facility to others. Safety is also an important subject observed to be a concern for every patient admitted into the hospital for care, and nurses are direct care givers who spend the most time with the patient which forms the basis of patient trust and confidence. The need to improve patient satisfaction, and prevent harm was identified in the medical/surgical unit of the hospital with the largest volume of hospital admissions. A pilot study to last for two months was proposed for one arm of the unit on the south side. The purpose of the study was to test the efficacy of hourly rounding, and its effect on patient satisfaction due to the presence of staff in the room to proactively meet with patient needs of Pain, Potty, Position, Possession, and Parting (5P's) every hour. Hourly rounding is designed to reduce the use of call lights, prevent falls, and reduce long hospital stays. This is in contrast to the current system of waiting for the sound of the bed alarm, or a patient to call before entering the room, which has resulted in many falls, delayed responses to call lights, and long hospital stays. Research evidence shows that in many hospitals across the nation where hourly rounding was done intentionally to meet with patient needs there was great improvement in several patient related factors. These factors include higher patient satisfaction scores, reduced harm such as falls, increased prevention of pressure ulcers, and reduced use of call lights.

Health care expense is increased due to the injury caused from fall and the suffering of patient increases. According to Centers of Disease Control and Prevention (CDC), fall injuries for those 65 years of age and above costs $19 billion and one in every three adults age 65 and above falls each year (CDC, 2012). A person's ability to function as a productive member can be affected significantly by the fall. Health care organizations are impacted financially as the health insurance does not reimburse the expense caused by the fall. The direct medical cost of fall was $ 30 billion in 2010 (CDC, 2012). The second most frequent cause of harm in the hospitals is patient falls. Hospitals are making every effort to prevent the fall- associated mortality and morbidity in older adults. Patients are assessed for fall risk and the fall prevention programs are initiated on admission and every shift. Even with these programs in place, fall rate continued to remain high. So hourly rounding on patients was implemented as a measure and studies were conducted to determine the effectiveness of this intervention. This intervention of hourly rounding addressing the needs of pain, potty, personal belongings and position has found to be effective for not only decreasing the falls, but also increasing the patient satisfaction. Hourly rounding significantly decreased the use of call light, increased patient satisfaction, and improved patient safety. So hourly rounding is suggested to be an operational change in hospitals for fall prevention. In order to implement this intervention, a multi-modal intervention of education is conducted among the health care workers to make them aware of the benefits to the patients as well as the hospitals. The theory of behavioral change is incorporated to change the health care workers' attitude and behavior towards the new intervention. Evaluate the outcome after the implantation to see the effectiveness in reducing falls. Motivation, reminders and rewards are supported by evidence in being compliant with the new intervention. Audits can reveal the outcome in which the patients and the hospitals are benefited.
