Book excerpt: Briefly, OTA found that screening women for osteoporosis at menopause and placing those with low bone density on long-term estrogen replacement therapy would deliver an additional year of life for between $22,000 and $27,000, a reasonable cost per added year of life compared with many preventive interventions currently accepted for Medicare payment. OTA also found that placing all women on long-term estrogen replacement therapy at menopause, without screening for bone density, would deliver an additional year of life for about the same amount, roughly $23,000. The background paper emphasizes, however, the substantial uncertainty about the benefits and risk of hormone replacement therapy and about the quality-of-life effects of hormone replacement therapy. Current medical practice is to prescribe estrogen with progestins. Under this therapeutic regimen, endometrial cancer risks are eliminated, but coronary heart disease benefits also are likely to decline. Consequently, hormone replacement therapy as currently prescribed to most women is less cost-effective than estrogenonly therapy - costing roughly $70,000 per added year of life.

Covering every aspect of a complete cost-effectiveness analysis, this book shows you how to find which data you need, where to find it, how to analyze it, and how to prepare a high-quality report for publication. Designed for the classroom or the individual learner, the material is presented in simple and accessible language for those who lack a biostatistics or epidemiology background, and each chapter includes real-world examples and "tips and tricks" that highlight key information. Exercises throughout allow you to test your understanding with practical application, and the companion website features downloadable data sets for students, as well as lecture slides and a test bank for instructors. This new third edition contains new discussion on meta-analysis and advanced modeling techniques, a long worked example using visual modeling software TreeAge Pro, and updated recommendations from the U.S. Public Health Service's Panel on Cost-Effectiveness in Health and Medicine. This is the second printing of the 3rd Edition, which has been corrected and revised for 2018 to reflect the latest standards and methods. Cost-effectiveness analysis is used to evaluate medical interventions worldwide, in both developed and developing countries. This book provides process-specific instruction in a concise, structured format to give you a robust working knowledge of common methods and techniques. Develop a thoroughly fleshed-out research project Work accurately with costs, probabilities, and models Calculate life expectancy and quality-adjusted life years Prepare your study and your data for publication Comprehensive analysis skills are essential for students seeking careers in public health, medicine, biomedical research, health economics, health policy, and more. Cost-Effectiveness Analysis in Health walks you through the process from a real-world perspective to help you build a skillset that's immediately applicable in the field.

Book excerpt: Bone is hard tissue that is in a constant state of flux being built up by bone-forming cells called osteoblasts while also being broken down or resorbed by cells known as osteoclasts. During childhood and adolescence bone formation is dominant; bone length and girth increase with age ending at early adulthood when peak bone mass is attained. Males generally exhibit a longer growth period resulting in bones of greater size and overall strength. In males after the age of 20 bone resorbtion becomes predominant and bone mineral content declines about 4% per decade. Females tend to maintain peak mineral content until menopause at which time it declines about 15% per decade. Osteoporosis is a disease characterized by low bone mass and structural deterioration of bone tissue leading to bone fragility and an increased susceptibility to fractures especially of the hip spine and wrist. Osteoporosis occurs primarily as a result of normal ageing but can arise as a result of impaired development of peak bone mass (e.g. due to delayed puberty or undernutrition) or excessive bone loss during adulthood (e.g. due to estrogen deficiency in women undernutrition or corticosteriod use). Osteoporosis-induced fractures cause a great burden to society. Hip fractures are the most serious as they nearly always result in hospitalization are fatal about 20% of the time and produce permanent disability about half the time. Fracture rates increase rapidly with age and the lifetime risk of fracture in 50 year-old women is about 40% similar to that for coronary heart disease. In 1990 there were 1.7 million hip fractures alone worldwide; with changes in population demographics this figure is expected to rise to 6 million by 2050. To help describe the nature and consequences of osteoporosis as well as strategies for its prevention and management a WHO Scientific Group meeting of international experts was held in Geneva which resulted in this technical report. This monograph describes in detail normal bone development and the causes and risk factors for developing osteoporosis. The burden of osteoporosis is characterized in terms of mortality morbidity and economic costs. Methods for its prevention and treatment are discussed in detail for both pharmacological and non-pharmacological approaches. For each approach the strength of the scientific evidence is listed. The report also provides cost-analysis information for potential interventions and discusses important aspects of developing national policies to deal with osteoporosis. Recommendations are made to the general population care providers health administrators and researchers. Lastly national organizations and support groups are listed by country.

Book excerpt: Osteoporosis is a common chronic condition which poses a substantial clinical, economic, and health-related quality-of-life (HRQOL) burden to the individual, the U.S. health care system, and society in general. The overall objective of this study was to evaluate the economic, clinical and humanistic outcomes of current osteoporosis interventions employed in the prevention of osteoporotic fractures in the Department of Defense (DoD) population. The overall objective encompassed four primary objectives: to assess the epidemiology of osteoporotic fracture in women> age 50; to determine the effectiveness of current osteoporosis interventions; to identify significant risk factors and other covariates in the prediction of osteoporotic fracture; and to determine the cost- effectiveness of current osteoporosis interventions. A three-year sample-based retrospective cohort study was conducted using DoD health care and prescription claims from fiscal years 2000 to 2003. Using an intent-to-treat study design, a total of 49,851 women> age 50 were followed for osteoporotic fracture. The effectiveness of the interventions was determined by performing a series of both logistic and direct Cox proportional hazard regressions. The net-benefit regression method of cost-effectiveness was employed to determine the cost-effectiveness of the treatment interventions and to determine the importance of covariates on the marginal cost-effectiveness of an intervention, while statistically controlling for the presence of risk factors and other covariates. The epidemiologic study results showed that the three-year cumulative incidence of an osteoporotic fracture was 2.5 % for the cohort (0.4% in patients without a diagnosis of osteoporosis; 6.1% in patients with a diagnosis of osteoporosis).

Available in PDF, EPUB and Kindle. Book excerpt: When I was a young intern in internal medicine, osteoporosis was defined mainly as a fracture occurring in elderly people. However, although plain X-ray examination was recognized as an insensitive way to detect osteoporosis, hypodense bone was already considered as the reflection of the disease. Over the past 20 years, con siderable progress has been accomplished. In terms of prevalence, incidence, risk factors, and the burden of osteoporosis and low-energy fracture, we can appreciate the magnitude of the problem and its impact on quality oflife. Weare wellaware that vertebral fracture, which can be acutely associated with a low clinical expression, leads to significant long-term impairments. The costs for the individual and for the health budget, Le. society, are estimated better. Wenow have a clear definition of the disease, such as low bone mass and architectural defects, resulting in increased fragility. For the former part of the definition, the clinician can use accurate and pre cise tools allowing them to distinguish how their patient's bone mineral content differs from that of a young healthy population in which fracture occurs very rarely. For the second series of fragility determinants, i. e. structure, methods are in rapid progress and will provide information to clinicians on these variables in the near future. Basicand clinical research have allowed major improvements in the understand ing of the pathophysiology of the disease. Newgenes implicated in lowand/or high bone mass have been characterized.
