Heart rate variability (HRV) defined as the beat to beat alteration in heart rate, provides a non-invasive tool for exploring the autonomic nervous system. Diabetes mellitus tends to affect the vagal nerve first with consequence being a reduced heart rate variability associated with poor cardiovascular prognosis. Intensive glycemic control prevents the development of CAN in patients with type 1 diabetes. There is little information on whether the intensification of glycemic control using insulin will improve cardiac autonomic functions in patients with type 2 diabetes mellitus. Aim: Our main objective was to determine the effect of optimizing glycemic control using insulin on the heart rate variability in type 2 diabetes mellitus patients.Method: We conducted a single arm clinical trial at the National Obesity Centre of the Yaoundu00e9 Central Hospital. We have previously demonstrated that exercise prevents insulin resistance and the associated derangements in insulin-stimulated vasodilation in microvessels of rats prone to obesity and diabetes. Here, we describe new evidence indicating that exercise also reverses these derangements. The beneficial effect of exercise on vascular insulin action was consistent across studies of conduit artery blood flow in vivo and microvascular reactivity to insulin in vitro, in both humans and rodents, in response to both short-term and chronic exercise training, and were consistently associated with improvements in indices of glycemic control. Collectively, these data provide strong evidence that exercise is a powerful mediator of the metabolic and vascular effects of insulin.

Medical nutrition therapy and exercise are effective as oral antidiabetics and/or insulin treatment and must be in combination with pharmacological intervention. We aimed to study the effect of lifestyle intervention in obese type 2 diabetics using insulin. Patients above BMI>30 kg/m2, and using more than >60 iu insulin per day were enrolled in the study. Their daily calorie requirments were calculated and medical therapy were arrenged. In collabaration with Physical Rehabilatation Clinic, patients were ordered to do physical exercises in the hospital. Before the therapy; patients were monitorised in terms of ECG and blood pressure. Maximum heart rate was calculated and 60-80 % of it was calibrated during the exercise. Bicyle warm up for 5 minutes, 20 minutes of active exercise, 5 minutes of cooling was done during the exercise for 20 consecutive days. After discharge from hospital, patients were told to do active exercise for at least 45 minutes every other day. After 6 months, weight, BMI, A1c changes and daily insulin doses were compared. There were 21 females (64%) 12 males (36%), mean ages were 60.52u00b17.45 years. After six months, mean weight loss was 8.44u00b11.22 kgs (p=0.002), mean BMI decrease was 1.45u00b10.71 kg/m2 (p=0.49), mean A1c decrease was 2.36u00b11.68 % (p=0.001) and mean daily insulin dose reduction were 24.18u00b113.96 iu (p=0.001). It should be kept in mind that, unnecessary high daily insulin doses could be the reason for poor glycemic control and obesity especially in patients who are obese and have high A1c levels. Lifestyle intervention is the first and maybe the most effective treatment for obesity and insulin dose reduction, so patients must be motivated and should be followed up regularly.

Patients were phone contacted on their progress at Week 1 and 2, and their insulin titration ability assessed using a standardized assessment form at Week 3. If they failed the assessment, diabetes nurses would lead the insulin adjustment accordingly. Phone interviews continued until patients achieved their target FBGs. Result & Outcome33 patients (male=22; female=11; mean age 60.5 u00b1 10.8 years) completed the program. At Week 0, mean baseline HbA1c was 9.5 u00b1 1.2%. At 5 months, HbA1c was lowered to 8.0 u00b1 0.7%. 19 patients (57.5%) had adjusted insulin dose according to the prescribed algorithm. There were a total of 119 insulin titration episodes with few episodes of mild hypoglycemic event during the study period: titration by nurse=56.3% (n=67); by patients = 33.6% (n=40); by doctor = 10.1%(n=12). 6 patients did not require insulin titration as home FBGs were optimized by the prescribed starting dose of insulin therapy. Conclusion Empowering patients to take greater control of their diabetes, through education and patient-oriented insulin titration algorithms, can achieve targeted FBGs at a faster and more effective manner.
