Book excerpt: The Health Belief Model (HBM), as proposed by Janz & Becker (1984), is a theory of adherence to medical recommendation. Findings on the association between the HBM and adherence and metabolic control in adolescents with Type 1 diabetes have been inconsistent (e.g. Brownlee-Duffeck et al., 1987; Patino et al., 2005). The current study aimed to investigate the relationship between parent and adolescent health beliefs and adherence/metabolic control. Further, the study aimed to identify variables that may moderate the parent/adolescent health belief and adherence/metabolic control relationship; namely, amount of parent responsibility for diabetes care, parent emotional support, and executive functioning of parent/adolescent. Findings were generally unsupportive of HBM and moderation hypotheses but patterns emerged that may be important in future research and clinical work with this population. These themes include the tendency for Caucasians and younger adolescents to have better metabolic control, and the importance of parent perception of severity and susceptibility of diabetes complications and the significant correlation both parent and adolescent executive functioning abilities have with adherence/metabolic control. Clinically, results speak to the need of further assessment and intervention targeting health beliefs and executive functioning in parents of adolescents with type 1 diabetes. Results further indicate the need for additional and more nuanced research on health beliefs in this population.

Book excerpt: Patients with insulin-dependent diabetes mellitus (IDDM) must adhere to a complex treatment regimen. Adherence is often poor during adolescence, when the regimen behaviours may interact negatively with many of the developmental tasks. In a randomized controlled outcome study, 15 adolescents and their families participated in a family-based intervention for improving adherence to the diabetic regimen. Fourteen comparison group adolescents with IDDM spent a comparable amount of time learning stress management techniques. Adolescents in both groups attended a diabetes management review session with the clinic nurse. Inclusion criteria included mean glycosylated hemoglobin (HbAlc) levels $>$9.0% over the previous nine months. At a 6-month follow-up, adolescents in the family intervention group were testing blood glucose levels more regularly (p $$1% (0% vs. 29%, $\chi\sp2$ = 6.32, p =.05). Methodological issues were discussed.

Book excerpt: Type 2 diabetes mellitus represents a daunting self-management challenge due to its complicated daily treatment regimen which includes adhering to diet, exercise, and medication recommendations. Little research has evaluated perceived barriers to diabetes self-care in a group that is at particular risk for diabetic complications: U.S. Latinos. This study had two aims. The first was to provide evidence that experimental and validated self-report instruments selected for this study functioned in a comparable manner in both Spanish and English in a sample of low income, predominantly Spanish-speaking diabetic patients (n = 77). The second aim was to examine hypothesized relationships of selected predictors to two criterion variables measuring health status (i.e., glycosylated hemoglobin A1c and Diabetes Impact) using a method described by Baron & Kenny (1986). Predictors included age, education, and income as covariates, and perceived environmental barriers to self-care, self-efficacy for treatment adherence, fatalistic beliefs (external health locus of control), and Latino cultural diabetes beliefs. Potential moderator and mediator variables of the relationship between perceived barriers to self care and criterion variables included self-efficacy for treatment adherence, fatalistic beliefs and Latino cultural diabetes beliefs. Scales used in this study had coefficient alpha values ranging from .82-.96 in Spanish and .74-.97 in English and also demonstrated strong factor structure integrity. Book excerpt: The purpose of this study was to examine the relationship between family functioning, health-related quality of life, and metabolic control in order to identify areas for intervention that can improve medical and psychosocial outcomes for children and adolescents with type 1 diabetes. Children (N = 45) ages 8-17, both male and female, with type 1 diabetes, along with one caregiver (parent or legal guardian) (N = 45) were asked to complete the Pediatric Quality of Life Inventory (PedsQL) generic and diabetes-specific form to assess health-related quality of life along with the Diabetes Family Behavior Checklist and the Family Relationship Index of the Family Environment Scale to assess family functioning. Recent Hemoglobin A1c (A1c) was obtained from the physician at their visit or by parent report to assess metabolic control. Results indicated a significant relationship with poorer metabolic control relating to poorer physical health-related quality of life, as reported by children. All reports indicated a significant correlation between metabolic control and both general and diabetes specific health-related quality of life. There was a significant relationship with mother ̓s educational level on the outcome variables; educational level was therefore used as a control variable in all regression analyses. Child-reported general family functioning accounted for a significant amount of variance in child reported general health-related quality of life. Parent score on the non-supportive scale for diabetes specific family functioning accounted for a significant amount of variance in parent reported general health-related quality of life and diabetes specific health-related quality of life for their child. Results demonstrated the importance of obtaining both child and parent perspectives on issues regarding general family functioning and health-related quality of life and diabetes specific family functioning and health-related quality of life. In addition, they illustrated the importance of assessing health-related quality of life for children with diabetes as opposed to merely looking at the physical effects.
