Book excerpt: The objective of this study was to evaluate the in vivo performance of the hydroxyapatite (HA) coating and hydrofluoric acid (HF) etching zirconia (ZrO) implants and to compare the result with titanium (Ti) implants treated in a similar manner. A total of four different implant types were tested in this study. Threaded zirconia implants with HA coating (Test 1) and zirconia implants with HF-treated surfaces (Test 2) were used to compare to the same size of titanium implants treated in identical fashion (control 1 and control 2). All implants measured about 3.5 mm at the thread diameter and 7.0 mm in total length. Each rabbit received two zirconia and two titanium implants treated in the same manner (either HA-coated or HF-etched). The samples were implanted into the rabbit tibias and retrieved at 6 weeks. Upon retrieval, 24 specimens (6 samples for each group) were fixed and dehydrated. The samples were then embedded undecalcified in PMMA for histomorphometry to quantify the bone-to-implant contact (BIC). Another 24 samples were kept in 0.9% saline and were evaluated using removal torque (RT) analysis to assess the strength of the implant-to-bone interface. The histomorphometric examination demonstrated direct bone-to-implant contact for all four groups. HA particle separation from the implants surface was seen in a majority of the HA-coated samples. No signs of inflammation or foreign body reaction were found during examination. Due to the HA particle smear contamination in the ZrO-HA group, no data was collected in this group. The mean BIC at the first three threads of the Ti-HA, Ti-HF and ZrO-HF were 57.78"18.22%, 46.41"14.55% and 47.41"14.05%, respectively. No statistically significant difference was found pair-wise among these three groups. When comparing the BIC data with the machined-surface implants, a statistically significant difference was found between the Ti-HA versus Ti implant group and the Ti-HF versus Ti implant group. The esthetic outcome of restorations supported by titanium implants may be compromised if the dark gray color of the implant shows through a thin peri-implant mucosa or if the implant fixture becomes visible following soft tissue recession. Also titanium might cause allergic reactions. For these reasons, zirconia implants have been considered as alternative materials because of their white color, high material properties and biocompatibilities. Still, further investigations are necessary to confirm the in-vivo performance of these implants. Purpose: The purpose of this study is to determine the histomorphometric and biomechanical properties of zirconia implants manufactured by Shofu Inc. with a sand-blasted and acid-etched surface treatment, compared to that of the titanium implants from the same manufacturer with a sand-blasted and acid-etched surface treatment in a canine model. Material and Methods: Six beagle dogs (1-2 years old) will be used in this split mouth trial. After 8 weeks following extraction of the second to fourth mandibular premolars, zirconia implants (experimental group) and titanium implants (control group) were placed on the each side of mandible. At 8 weeks and 12 weeks after implant placement, the animals were sacrificed, and implants were removed in block sections, and histological and histomorphometric analyses were measured. Specifically, the bone-implant contact (BIC), bone area (BA), removal torque (RTQ), mineral apposition rate (MAR), bone forming area (BFA), and Periotest value (PTV) of the two groups were studied and compared. Results: At 12 weeks post operatively, one Shofu thread type Ti implant were not integrated. Over all failure implant was 0/12 in zirconia group, and 1/12 in titanium group. In 8 weeks samples, only statistical differences were higher BA (p=0.02) in macro threads area and BFA (p=0.02) in zirconia implants group than titanium implants group. In 12 weeks group, zirconia implant group showed higher MAR at 9-10 and 10-11weeks time frame (p=0.02, and 0.04 restectively), and PVT value (p=0.01) than titanium implants group. Removal torque value increased in both titanium and zirconia group with time. Average of removal torque value showed higher in titanium implants than zirconia implants, but the differences were not statistically significant in both 8 weeks (Ti; 64.16±16.93 N-cm, ZrO2: 42.5±6.01 N-cm : p=0.247) and 12 weeks (Ti; 82.5±9.41 N-cm, ZrO2: 51.3±19.38 N-cm: p=0.16). In the removal torque analysis, it is observed that the bone-implant interface seems to be of more rigid and brittle in nature as indicated by the lower averaged onset angle, peak angle, area under curve and RTQ. Conclusion: Zirconia implants group showed higher value of BA with macro thread and BFA in 8weeks and MAR at 9-10, 10-11 weeks period, and PVT in 12 weeks post operatively. Within the limited number of samples tested in this study, there is no difference between the BIC and RTQ of zirconia and titanium implants after 8 and 12 weeks of implantation.

Untextured (S-YTZP) discs were used as controls. Abstract: Zirconia offers a favorable choice for esthetic indirect dental restorations with excellent mechanical properties and biocompatibility. In the hostile oral environment, the longevity of dental restorations relies heavily on whether a strong and stable bond between the restoration and underlying supporting tooth structures can be achieved. However, unlike conventional ceramics, zirconia is a chemically inert material which makes it notoriously difficult to bond with any dental composite resin cement. This thesis aimed at developing and evaluating different zirconia surface pretreatments for improving the resin-zirconia bonding. The first laboratory experiment evaluated the effect of Er, Cr: YSGG laser treatment on topographic changes of zirconia surface. Laser parameters including power, irradiation time and water spray at different levels were tested and the treated zirconia surfaces were assessed by scanning electron microscope (SEM) and atomic force microscope (AFM). Results showed that all three tested laser parameters could affect the topographic changes of zirconia surface. Significant topographic changes were found when the power was set at 3.0 W with 50% water spray for 120 s or the power was set at 4.0 W with 75% water spray for 60 s. The second experiment evaluated the effects of grit-blasting (sandblasting), selective infiltration etching (SIE), 3.0 W and 4.0 W laser zirconia surface treatments on the resin-zirconia bonding. Resin composite was bonded to the treated zirconia which was then sectioned into resin-zirconia micro-beams of a standard size. Microtensile bond strength (MTBS) tests were performed for adhesion strength testing. In dry storage conditions, SIE, grit-blasting and 3.0 W laser groups showed significantly higher MTBSs than the control group while no difference in MTBSs was found between the 4.0 W laser group and the control group. After water storage, SIE treatment showed significant higher MTBSs than the remaining groups. MTBSs of water storage specimens were significantly less than the dry specimens except the ones in 4.0 W groups. Zirconia surface treatments could enhance the resin-zirconia bonding in dry storage conditions. The SIE treatment appeared to provide the strongest resin-zirconia adhesion despite hydrolytic degradation. The third experiment evaluated the fatigue performance of resin-zirconia bonding with different surface treatments. Resin-zirconia beams were prepared and loaded at 36 N with 1.5 Hz frequency for 104 cycles at room temperature, before sectioned for MTBS tests. In dry storage conditions, SIE and grit-blasting groups showed significantly higher MTBSs than the control group while no differences in MTBSs were found among the 3.0 W, 4.0 W and control groups. After fatigue loading, only the SIE and 3.0 W groups showed significantly lower MTBSs while the other groups did not. After water storage, MTBSs of water storage specimens were significantly less than the dry specimens except the ones in SIE and 4.0 W groups. No significant differences in MTBSs were found in all groups after fatigue loading. The SIE group showed significantly higher MTBSs than the other study groups after water storage and fatigue loading. Titanium implanthas been shown to be a predictable method for replacing missing teeth. Clinical studies revealed that implant failure rate in irradiated regionwas high. Many studies showed that modifications of implant surface could enhance implant osseointegration by improving cell attachment, cell growth and bone formation. Nevertheless, there were few studies investigating the effect of implant surface modification on osseointegration in irradiated bone. In the first experiment, the effect of fluoride-modified (FM) titanium surface on irradiated osteoblast attachment was assessed. The morphology and chemical composition of FM surface was assessed by SEM, AFM and XPS. Osteoblasts received 0Gy, 2Gy, 4Gy, 6Gy, 8Gy, 10Gy radiation. Cell number, fluorescence intensity and cell area of irradiated osteoblasts were assessed. The number of osteoblasts onFM surface was fewer than those on NF surface after 0Gy, 2Gy, 8Gy and 10Gy radiation. Cell area of osteoblasts on FM surface was less at 2Gy radiation but larger at 6Gy radiation than on NF surface. The fluorescence intensity of osteoblasts was also higher on NF surface than on FM surface after receiving 0Gy, 2Gy, 4Gy, 10Gy radiation. In the second experiment, an animal model was established to study the effect of radiation on osseointegration. Rabbits were divided into 15Gy and 30Gy radiation groups. Only the left leg was exposed to radiation, and the right leg was protected from radiation. Totally, 24 implants were inserted. Implant stability quotient (ISQ), bone volume to total volume (BV/TV), bone-to-implant contact (BIC), and bone growth rate were measured. After 15Gy and 30Gy of radiation, ISQ and BV/TV were significantly reduced. At week 3, 15Gy radiation group displayed slower bone growth rate comparing with the control side. Fluorochrome results showed that the 30Gy radiation side had a significantly slower apposition of new bone.In addition, BIC on30Gy radiation side was notablypoorer than that on 15Gy radiation side and on 30Gy control side. Based on the animal model, the third experiment investigated effects of calcium phosphate nanocrystals on implant osseointegration in irradiated bone. Titanium implants treated with nano-scale calcium phosphate (CaP) crystals served as the test group while ones with dual acid-etching only served as the control group. The left leg of rabbits received 15Gy radiation and implants were placed in the irradiated leg. Significant higher ISQ was detected in the nano-CaP group at week 12. The bone growth rate in nano-CaP group was more than doubled than the control group at both week 6 and week 9. The fourth experiment evaluated artifacts on micro-CT images caused by titanium dental implant. Implants were assigned into four groups: (1) implant only; (2) implant with covering screw; (3) implant with resin embedding; and (4) implant with covering screw and resin embedding. Each implant was scanned by micro-CT at 3 angulations. Implant angulation was the most determining factor followed by resin embedding. A novel coating of self-assembled monolayer of phosphonate molecules (SAMP) that chemically enhances metallic surfaces by covalently bonding to their naturally occurring oxidized surfaces has been developed.1 The SAMP coating is chemically stable, and its in-vitro results showed that it promotes cell proliferation, osteoblast adhesion and enhances biological attachment to metal surfaces.1 The purpose of this study is to establish the efficacy of this novel chemical surface treatment in improving the fixation of metallic implants in both cortical and cancellous bone over a conventional untreated implant. We hypothesize that a SAMP monolayer coating would lead to greater in-vivo bone on-growth and fixation of SAMP-coated stainless steel (SS) implants when compared against uncoated ones. An established in-vivo model was used. The outcomes used to establish the coating's efficacy were the histological appearance of the interface between implant and bone and the static mechanical strength of the interface. Cylindrical 316L SS implants were machined and highly polished and half of the implants were coated with the SAMP coating. A total of 44 male, skeletally mature New Zealand White rabbits, weighing between 3.5 and 5kg, were implanted bilaterally in the distal femoral intramedullary canals. Each animal received one coated implant in one leg and one uncoated implant in the other leg. Cylindrical implants were implanted in 9 rabbits per time point. Additionally at each time-point, 2 rabbits were implanted with cylindrical pins that have 6 longitudinal crevices and their cross-section were circumferentially serrated. The rabbits were randomized to one of four time points: 4, 8, 12 and 16 weeks. In the cylindrical implants, bone formation was evident in the epiphyseal cancellous bone surrounding the coated implants after 4 weeks. Histological evaluation of early time points has showed cell types associated with new bone formation, in both the cortical and cancellous 5 bone, in higher numbers in the treated implants. A thicker layer of unmineralized osteoid and higher number of osteoblasts were observed along the interfaces with the coated surfaces than the uncoated ones in the 4-week time-point. That indicated that there is greater bone formation and a stronger initial bone response to the coating. At the later time points, however, there was no difference between the amount of bone formed around the coated implants and the uncoated ones. At the 4-week time point, the coated samples of the creviced design showed significant osteoconductive potential of the treatment, which was observed histologically as recruitment and proliferation of osteoblasts into areas of initially non-bony contact are present. Bone had formed completely into the interior crevices of the coated implants. In the uncoated implants, bone was only present in the outer area surrounding the implants. Micro-Computed Topography scanning was performed on all the cancellous bone samples in order to visualize the amount of newly formed bone around the implants and calculate bone volume to total volume within the relative effective diameter. In the early time points, it was found that bone has grown in a larger volume around the coated implants. No statistical difference in bone volume was found at the later time points. Mechanical pullout testing was performed on cylindrical implants at each time-point (n=6) of the coated and uncoated implants. A mechanical pullout apparatus that ensures pullout in a uniaxial direction at under displacement control was used, and the resistive shear force at failure was measured. Mechanical testing showed no difference in the average of failure force of the coated group when compared to that of the uncoated group at each time point. There was also no statistical difference between time points among the coated samples. The SAMP coating did not increase the shear strength of the interface. In conclusion, in a rigorous, established model, we have shown that at early time points, the SAMP treatment elicits a significant bony apposition to the implant surface. However, this effect does not appear to be maintained.
