Book excerpt: This research investigated the effects of static lower extremity posture on hip strength, and then examined their collective influence on knee and hip joint kinematics during a single leg squat in males and females. Thirty one healthy males and 31 healthy females, predominantly college students, between the ages of 18 and 35 participated in a single data collection session during which six lower extremity posture characteristics were measured, followed by measurement of hip abduction and extension strength and concluded with neuromuscular and kinematic analysis of the hip and knee during a single leg squat. Hip torque was normalized to body mass and electromyographic data were normalized to maximum voluntary isometric contractions. Five single leg squats were performed on the dominant stance limb to a depth of 60° of knee flexion. Path analysis, implemented by structural equation modeling, was used to examine whether greater lower extremity posture characteristics predicted decreased hip torque and whether greater lower extremity posture characteristics and decreased hip torque collectively predicted greater dynamic valgus knee motion (increased hip adduction and internal rotation, and knee external rotation and valgus). Separate multivariate analyses of variance determined whether females and males differed on measures of lower extremity posture, hip strength, and total hip and knee motion during the single leg squat. The findings were that greater hip anteversion predicted decreased hip abduction torque, and greater tibiofemoral angle predicted decreased hip extension torques. Direct relationships were noted between greater hip anteversion and genu recurvatum with greater knee external rotation, and between greater navicular drop and hip anteversion with greater hip internal rotation during the single leg squat. Furthermore, decreased hip abduction torque predicted greater knee external rotation while decreased hip extension torque predicted greater knee valgus during a single leg squat. Hence, it was concluded that greater lower extremity posture characteristics predicted decreased posterio-lateral hip strength, and collectively, greater lower extremity posture characteristics and decrease posterio-lateral hip strength predicted greater functional valgus collapse during the single leg squat.

Book excerpt: Previous research has shown that muscle activation patterns of the anterior thigh influence knee joint mechanics and in particular, patellar tracking. Vastus lateralis and vastus medialis muscles are located on the lateral and medial sides of the quadriceps group respectively. They are the primary muscles responsible for maintaining patellar tracking through the patellofemoral groove in order to prevent knee conditions such as patellofemoral pain syndrome. If either the vastus lateralis or vastus medialis muscle has a predominant level of activation, then the patellar will tend to be drawn to one side of the patellofemoral groove. Rehabilitation protocols for treating patellar maltracking and patellofemoral pain often focus on improving muscle activation of vastus medialis. Vastus medialis is the smallest anterior thigh muscle responsible for leg extension, with lower activation levels often resulting in the patellar being drawn laterally. Although rehabilitation protocols may focus on medial muscle activation, both muscles are innervated by the same nerve, suggesting that improved vastus medialis muscle activation will also result in improved vastus lateralis muscle activation. To combat this problem it has been suggested that turning the foot out to 45 degrees may improve vastus medialis activation due to the interaction of the adductor magnus muscle. Another theory suggests that vastus medialis will be in a relatively higher position than vastus lateralis when seated, and therefore, the muscle needs to work harder to oppose gravity. Three exercises (leg press, squat and terminal leg extension) were assessed in order to determine if commonly employed rehabilitation protocols can improve vastus medialis activation. The exercises were performed with two foot positions (parallel/vertical and turned out to 45 degrees) at 5 0 % of 3 R M and the resultant E M G activity was normalised to a series of isometric M V C trials. Single differential electrodes were placed on the muscle bellies of vastus medialis and vastus lateralis, running parallel to the muscle fibres, with an earth electrode over the patella. Synchronised E M G and video data were collected to allow for the assessment of muscle activation during seated and standing posture exercises, open and closed kinetic chain exercises and for both foot positions to determine if any of these exercise conditions would result in improved vastus medialis muscle activation. The results showed that any improved vastus medialis activation also coincided with improved vastus lateralis activation, with both muscles contracting to perform leg extension motion. Higher levels of muscle activation generally occurred when the foot was in a parallel/vertical foot position, and the squat exercise resulted in higher levels of muscle activation than both of the seated exercises. The two closed kinetic chain exercises also demonstrated higher levels of muscle activation when the leg was in a flexed position, while the open kinetic chain exercise produced higher levels of muscle activation as the leg moved toward an extended position.

Book excerpt: Occupational exposure to high knee flexion postures such as squatting or kneeling is associated with an increased risk of developing knee osteoarthritis. The exact mechanisms behind the increased risk are currently unknown. The literature is inconclusive on whether the duration spent in knee bending postures or the act of cycling through these postures is more detrimental to the knee. The goal of the study was to investigate whether the type of squat (cyclic or sustained) leads to different changes in power, neuromuscular control and lower-limb muscular blood flow parameters. It was hypothesized that sustained squatting would cause more detrimental effects to biomechanical and hemodynamic measures that are related to injury than cyclic squatting. Knee joint power, knee extensor muscle activation patterns, and lower limb kinematics were recorded before and after exposures to cyclic and sustained squats, whereas vastus medialis muscle oxygenation was measured during squat exposures. Thirty healthy young participants (15 Male/15 Female, age: 22.3 ± 2.2 years, height: 1.70 ± 0.09 m, weight: 66.88 ± 11.16 kg) were recruited for the study. Participants completed a cyclic and sustained squatting exposure. The total time spent in a squat posture was equivalent between exposures. The cyclic squat consisted of a 40-second squat followed by 20 seconds of rest, repeated for six cycles. The sustained squat was a static squat hold of four minutes. Two countermovement jumps were performed pre and post each squatting exposure, for a total of eight countermovement jumps. Between squat exposures, a four minute standing recovery occurred. Vastus medialis muscle oxygenation was measured using near-infrared spectroscopy. Lower limb kinematics were collected using 3D-motion capture, kinetics were measured using two embedded force plates, and vastus medialis and vastus lateralis muscle activity was collected with surface electromyography. The sustained squatting exposure was found to cause a significantly greater decrease to peak knee power from baseline compared to the cyclic squat (−0.71 W/kg and −0.33 W/kg respectively, p = .016). A sex difference was also found, with males experiencing greater decreases in knee power than females (−0.73 W/kg vs. −0.32 W/kg, p = .013). A significant interaction occurred for integrated EMG (iEMG) between muscle and phase (p = .037). During the deceleration phase on average iEMG values decreased from baseline values, and increased during the preparatory phase (− 2.48 mV∙s vs. 0.62 mV∙s), with vastus medialis activity being reduced by a larger degree than vastus lateralis. Changes to tissue saturation index % during the squat exposure also displayed a sex main effect (p = .012), with on average males experiencing decreases and females increases (−1.97% vs. 2.60%). Regardless of squat type, the squatting exposure did not cause significant changes to frontal plane motion, delays in muscle onset or changes in percent normalized muscle oxygenation. Historically, occupational squatting and its relationship to knee osteoarthritis has been studied from an epidemiological framework, therefore, the current study was novel in that it provided a more systematic and quantitative assessment of squatting exposures. The study demonstrated that acute exposures to high knee flexion postures cause detrimental changes to power generation, neuromuscular control and hemodynamics. The results suggested that sustained exposures were more detrimental than cyclic exposures. Sex effects indicated that males were more affected by the squatting exposures than females. Our work highlights the importance for workplace guidelines in occupations with frequent squatting to limit the duration of static high knee flexion activities, and provide ample rest between exposures.

Book excerpt: "The purpose of this study was to evaluate compensatory biomechanical patterns in the lower extremity created by restricted knee flexion and ankle dorsiflexion when performing squats. Forty two healthy subjects (21 men, 21 women; 22.5 (4.5) years, 73.8 (17.8) kg, 167.5 (12.5) cm) participated in the study. Data were collected using a force plate and a 3-d electromagnetic tracking device for bilateral lower extremity analyses. Three parallel squats were performed in non braced, right knee restricted and right ankle restricted conditions. Dependent measures were hip, knee and ankle total joint displacement and work done on the hip, knee and ankle during the eccentric portion of the squat. Three repeated measures ANOVAs compared lower extremity kinematics between conditions, while one repeated measure ANOVAs evaluated lower extremity kinetics. Mean hip, knee and ankle ROM was reported, as was sagittal plane work done on the hip, knee and ankle for each condition and limb. The primary findings of this study indicate hip and ankle flexion displacement significantly decreased in the contralateral (non-braced) limb during the ankle joint restricted condition. Ipsilateral (braced) limb hip, knee and ankle flexion significantly decreased during the knee restricted condition, while ipsilateral knee and ankle flexion decreased during the ankle restricted condition. Lower extremity sagittal plane energetic changes occurred in the ipsilateral knee and ankle when the knee joint was restricted and at the ipsilateral ankle in the ankle restricted condition. Relative and absolute shifts in work done on the hip, knee and ankle when compared to the non braced squat were observed. This study may best serve as a general sagittal plane model for clinicians and coaches to reference when using the parallel squat in patients/athletes with knee and ankle dysfunction. This has practical significance to clinicians as these substitutions in work could result in overuse (secondary) injury to the compensatory site or insufficient loading to the dysfunctional site, rendering it weak and susceptible to additional primary injury or limiting the athletes maximal performance."--Abstract from author supplied metadata.
