Book excerpt: The second edition of this superbly illustrated book equips practitioners with all the information that they will require in order to perform echocardiography appropriately and to interpret findings correctly. After clear exposition of fundamentals of the examination technique via both the transthoracic and the transesophageal approach, the functional anatomy of all normal and pathologic thoracic structures as observed on echocardiography is presented in detail. A wide range of basic and advanced applications of echocardiography in the emergency, perioperative, and intensive care settings are then described. A final supplementary section considers further applications of ultrasound in the ICU, such as in patients with respiratory failure, trauma, or venous thrombosis. This edition has been extensively revised and updated and includes entirely new chapters on various techniques and applications. It will be a tremendous aid to current best practice for all intensivists, emergency physicians, and anesthesiologists who intend to use echocardiography and ultrasound for diagnosis and guidance of optimal management in the critically ill.

Book excerpt: The hemodynamic evaluation of patients with acute circulatory failure and respiratory failure has in the past usually been performed using invasive procedures but in recent years less invasive monitoring devices have been introduced. Echocardiography can be used for both the diagnosis and the management of circulatory and respiratory failure. This book provides all the essential information required by readers in order to perform optimal hemodynamic management of the critically ill based on echocardiographic guidance. After an introductory section on basic principles, hemodynamic assessment using echocardiography is discussed in detail. The diagnosis and management of all types of circulatory and acute respiratory failure by means of echocardiography are then rigorously considered, and specific situations such as thoracic trauma and acute aortic syndrome are examined. The final section is devoted to future issues and applications.

Available in PDF, EPUB and Kindle. Book excerpt: Ultrasound in the Intensive Care Unit explores the current state of evidence supporting use of bedside ultrasound for procedural guidance and for the critical care-focused assessment of a variety of organ systems. This text covers standard practice areas, such as ultrasound guidance for vascular access in the ICU, as well as novel, less well-known applications such as the use of ultrasound for assessment of diaphragm function in patients with respiratory failure. As current ultrasound education is often dependent on widely varying local expertise, this book serves as a standard reference in what is at present a fragmented and challenging field of study for busy clinicians. By providing a comprehensive overview of the current state of critical care ultrasonography, with content contributed by experts in the field, this text will serve as a valuable resource that will allow education in critical care ultrasound to keep pace with the rapid technological growth that has driven this vital area of critical care medicine. Ultrasound in the Intensive Care Unit is essential reading for pulmonologists, practitioners in internal, hospitalist, and critical care medicine; educators and instructors in critical care ultrasound; and medical students, residents, and critical care trainees.

This book was released on 2015-09-30T00:00:00+02:00 with total page 513 pages. Available in PDF, EPUB and Kindle. Book excerpt: Primary Raynaud phenomenon, as clinical symptom of altered peripheral microcirculation, affects 5ˆ15% of individuals in the general population and almost 15% of cases will shift from the primary to the secondary phenomenon (associated with the systemic sclerosis and other connective tissue diseases) in a mean time of 29.4±10 months. Nailfold videocapillaroscopy (NVC) represents the safest, noninvasive, less expensive and reliable method to analyze microvascular abnormalities in systemic sclerosis and the findings can be scored and quantified. Since microvascular damage and dysfunction represent early markers of systemic sclerosis and are clinically mirrored by secondary Raynaud phenomenon, the diagnostic, prognostic and therapeutic implications of microvessel morphological analysis by NVC enables the best clinical management. Reduced capillary density on NVC predict and correlates with a high risk of developing digital skin ulcers and the presence of pulmonary arterial hypertension, and can therefore be used as a marker of systemic sclerosis severity and progression, as well as to monitorize the therapeutical effects. Growing interest and intense recent research on capillaroscopy have resulted up to 2010 in almost 350 papers reported in PubMed in the last 10 years, accounting for almost 50% of the total number of publications on the subject from 1951. However, an atlas explaining the capillaroscopy in rheumatic diseases in a modern, updated and physiologically-based way with large discussion of clinical cases was lacking and is now available. The authors of the atlas have been selected among the best international experts of the field and are all members of the board of the international courses on capillaroscopy supported by the European Ligue Against Rheumatic Diseases (EULAR). The reader after a carefull and progressive analysis of the text will learn to understand the capillaroscopic findings that characterize the most important rheumatic diseases and will use the atlas for his daily clinical practice.
