Book excerpt: Despite years of research, debate and changes in mental health policy, there is still a lack of consensus as to what recovery from psychosis actually means, how it should be measured and how it may ultimately be achieved. In Recovering from a First Episode of Psychosis: An Integrated Approach to Early Intervention, it is argued that recovery from a first episode of psychosis (FEP) is comprised of three core elements: symptomatic, social and personal. Moreover, all three types of recovery need to be the target of early intervention for psychosis programmes (EIP) which provide evidence-based, integrated, bio-psychosocial interventions delivered in the context of a value base offering hope, empowerment and a youth-focused approach. Over the 12 chapters in the book, the authors, all experienced clinicians and researchers from multi-professional backgrounds, demonstrate that long-term recovery needs to replace short term remission as the key target of early psychosis services and that, to achieve this, we need a change in the way we deliver EIP: one that takes account of the different stages of psychosis and the ‘bespoke’ targeting of integrated medical, psychological and social treatments during the ‘critical period’. Illustrated with a wealth of clinical examples, this book will be of great interest to clinical psychologists, psychiatrists, psychiatric nurses and other associated mental health professionals.

Available in PDF, EPUB and Kindle. Book excerpt: In 2003 the Bush Administration's New Freedom Commission asked mental health service providers to begin promoting "recovery" rather than churning out long-term, "chronic" mental health service users. Recovery's Edge sends us to urban America to view the inner workings of a mental health clinic run, in part, by people who are themselves "in recovery" from mental illness. In this provocative narrative, Neely Myers sweeps us up in her own journey through three years of ethnographic research at this unusual site, providing a nuanced account of different approaches to mental health care. Recovery's Edge critically examines the high bar we set for people in recovery through intimate stories of people struggling to find meaningful work, satisfying relationships, and independent living. This book is a recipient of the Norman L. and Roselea J. Goldberg Prize from Vanderbilt University Press for the best book in the area of medicine.

Available in PDF, EPUB and Kindle. Book excerpt: Can severe mental illness be prevented by early intervention? Mental illness is highly prevalent in the general population and has its onset mostly in adolescence and young adulthood. Early intervention usually leads to improved prognosis. This book describes a newly developed, evidence based cognitive behavioural intervention that can be used by clinicians to treat the precursor symptoms of psychosis and other severe mental illness. CBT for those at risk of a First Episode Psychosis offers a detailed new psychotherapy that has been shown to reduce the chance of transition to a first psychotic episode and to improve the chance for recovery. This encompasses: Psycho-education about prepsychotic symptoms A review of literature about psychological processes that are known to play a role in the development of psychosis A comprehensive manual – illustrated by numerous clinical vignettes - that can be used to treat help-seeking subjects with an increased risk of developing psychosis. Links to online resources and exercises to be used in therapy and education. A description of the multicentre randomized clinical trial investigating this new psychotherapy. The vast collective experience and expertise of the authors of this handbook results in an invaluable text for clinicians working in mental health care, as well as students, lecturers and researchers who have an interest in the prevention of schizophrenia and other severe mental illness.

Clinical practice is striving to incorporate the unfolding research findings and clinical innovations that are occurring on an international scale. This book presents the most comprehensive review of best practices written to date. Unfortunately, clinical practice guidelines often fail to make a significant impact because of adoption and implementation difficulties. This book assists widespread implementation by presenting views on best practises from around the world. A wide variety of contributors discuss their unique efforts at implementing the principles of care espoused in the Intervention Guide. These discussions are followed by a series of chapters on state-of-the-art applications of specific intervention issues by world leaders in the early psychosis field.

This book was released on 1999-07-28 with total page 527 pages. Available in PDF, EPUB and Kindle. Book excerpt: The treatment of early psychosis has been bedevilled by an entrenched pessimism, stemming from the asylum era and the Kraepelinian model of schizophrenia. More recently, however, there has been a surge of interest in preventively oriented treatment of patients showing the first signs of psychotic illness, with the realization that these illnesses are frequently highly responsive to early treatment. This is the first text to focus on the potential of early detection of psychosis, and the practicalities of treatment. Based on the pioneering experience and research of a now well-established prevention and intervention centre, and with contributions from international authorities, the book outlines a framework for intervention, reviews the evidence available to guide clinical practice, and describes models of treatment. Incorporating many personal narratives and case histories, it is strong on theory, sensitive on practical issues, and will challenge, inform and guide clinicians.

Book excerpt: The First Episode Psychosis Services Fidelity Scale (FEPS-FS 1.0) is a highly reliable scale that assesses the degree to which mental health teams deliver specialized evidence-based care to people experiencing a first episode psychosis. The scale comprises 35 components each rated on a 1 to 5 scale. It has been used in the United States, Canada and Europe. It can be used for on site fidelity reviews, remote fidelity assessment or self-report. Published papers document its psychometric features and allow comparisons with a representative sample of US programs. It is suitable for research, quality improvement and accreditation. The Manual provides a practical guide for scoring a FEPS program against the criteria set out in the fidelity scale. It is designed to increase the reliability and consistency of ratings across different sites and assessors. It includes a definition and rationale for each component, data sources, decision rules and a structured interview guide. There are also modules to support training the key informant and data abstractor. Templates support structured feedback to programs for quality improvement. The scale can be adjusted to rate care for different diagnostic groups including the schizophrenia spectrum disorders, bipolar disorder and those with an attenuated psychosis syndrome.

Book excerpt: The First Episode of Psychosis is the ideal book for patients experiencing the frightening and confusing initial episode of psychosis, which often occurs during late adolescence or early adulthood, and which affects nearly 3% of all people over the course of their lifetime. The book covers a range of disorders, focusing on primary psychotic disorders such as schizophrenia and schizophreniform disorder, clearly describing symptoms, early warning signs, and treatment--information that is essential for patients and families faced with the challenges posed by psychosis. The book also discusses psychiatric evaluation, healthy lifestyle choices, and the stigma often associated with mental illnesses. Worksheets allow readers to keep records of symptoms to facilitate communication with care providers, and an extensive glossary clarifies the dizzying array of terms used by medical professionals. Optimistic, practical, and recovery-oriented, The First Episode of Psychosis will help patients and their families to take an active, informed role in their care to ensure the best possible prognosis.
