Ils nécessitent une prise en charge spécifique afin d’éviter les pneumopathies d’inhalation entraînées par des fausses routes consécutives. La dysphagie génère des détériorations significatives dans la qualité de vie du patient et est un enjeu vital pour un tiers d’entre eux. L’évaluation précoce constitue donc un état des lieux des potentialités, essentiel à la prise en charge des patients touchés. Deux questions directrices découlent de ces constats - Le bilan de déglutition pratiqué au sein de l’UNV de l’Hôpital Bretonneau permet-il de dresser des profils de patients qui risquent une chronicisation de leur dysphagie trois mois après leur AIC? - Existe-t-il des items du bilan initial prédicteurs de troubles dysphagiques à trois mois ? Cette étude a été menée auprès de treize patients admis pour un AVC ischémique. Notre recherche s’est basée sur l’examen de la déglutition effectué en phase aigu. Nous avons procédé à l’examen des praxies, de différents réflexes relatifs à la sphère ORL et à un test de déglutition écologique, afin d’établir quels étaient les risques de fausse route de chaque patient. Puis, trois mois plus tard, nous avons déterminé quels patients conservaient des troubles de déglutition en administrant le Déglutition Handicap Index ainsi qu’un questionnaire sur la déglutition. Aucune corrélation statistique n’est ressortie de nos analyses. Cependant, à partir de certains éléments du bilan initial, un profil à risques de troubles de déglutition à trois mois peut être ébauché sur un plan exclusivement clinique. Sa validation et sa sensibilité restent à déterminer sur une population plus large, au cours de futures recherches.

The design of this device is significantly different from the previously used devices, notably laryngeal mask airways. Its bowl is made of a soft, non-inflatable thermoplastic elastomer, anatomically preformed to mirror the perilaryngeal structure. The I-Gel contains an additional epiglottis rest, which helps to prevent the epiglottis from down-folding or obstructing the laryngeal inlet. The device also includes a gastric channel, allowing drainage of air and gastric contents or passage of a gastric tube. This book provides a brief history of airway management and the classification of supraglottic airway devices; the mechanisms and forces contributing to their seal, and highlights the main differences between the devices.
