Genomic data can identify new drug targets for both common and rare diseases, can predict which patients are likely to respond to a specific treatment, and has the potential to significantly reduce the cost of clinical trials by reducing the number of patients that must be enrolled in order to demonstrate safety and efficacy. A key component of the approval of targeted therapeutics is the ability to identify the population of patients who will benefit from treatment, and this has largely hinged on the co-development and co-submission to the FDA of a companion diagnostic test.The co-development process, or the development of the test and drug for the simultaneous submission to FDA, has led to a major alteration in the way that drugs are being developed, with traditionally separate entities-pharmaceutical and diagnostic companies-now working in close collaboration. Refining Processes for the Co-Development of Genome-Based Therapeutics and Companion Diagnostic Tests is the summary of a workshop held by the Roundtable on Translating Genomic-Based Research for Health on February 27, 2013 to examine and discuss challenges and potential solutions for the codevelopment of targeted therapeutics and companion molecular tests for the prediction of drug response. Prior to the workshop, key stakeholders, including laboratory and medical professional societies, were individually asked to provide possible solutions to resolve the concerns raised about co-development of companion diagnostic tests and therapies. Workshop speakers were charged with addressing these solutions in their presentations by providing insight on (1) whether the proposed solutions address the problems described, (2) whether there are other solutions to propose, and (3) what steps could be taken to effectively implement the proposed solutions.

In A Chancellor's Tale Snyderman reflects on his role in developing and instituting these changes. Beginning his faculty career at Duke in 1972, Snyderman made major contributions to inflammation research while leading the Division of Rheumatology and Immunology. Upon discovering that temperatures in the laundry facilities were over 110 degrees he had air conditioning installed. He also implemented programs to help employees gain needed skills to advance. Snyderman discusses the necessity for strategic planning, fund-raising, and media relations and the relationship between the medical center and Duke University. He concludes with advice for current and future academic medical center administrators. The fascinating story of Snyderman's career shines a bright light on the importance of leadership, organization, planning, and innovation in a medical and academic environment while highlighting the systemic changes in academic medicine and American health care over the last half century. A Chancellor's Tale will be required reading for those interested in academic medicine, health care, administrative and leadership positions, and the history of Duke University.

The primary purpose of this book is not only to document these achievements, but also to examine the nature of the Training Program and to whatever extent possible, define the factors which have led to the outstanding results. This programmatic description encompasses the period from 1946 through 2009. The material in this manuscript has been extensively revised and enlarged since the original publication: Duke Cardiology Fellows Training Program: Origin to the Present, published in 2004. Throughout the manuscript, the designation "fellow" denotes a physician either during or after finishing the fellowship. In order to be included, a fellow must have been enrolled in the Training Program for at least one year. In presenting the large body of material, Chapters II-VII deal with specific time periods. Each of these chapters presents both a narrative of the programmatic development as well as a Table listing the names of the fellows, the types of training and future careers. (A detailed explanation of the content of these Tables is given at the end of Chapter II.) The remaining chapters deal with specific aspects of the Training Program. The Appendix documents the sources of the data. A "Rogues' Gallery" contains pictures obtained usually in a non-academic setting of a number of the fellows and faculty.
