Book excerpt: Selon l’Association Internationale pour l'Etude de la Douleur (IASP), la douleur est « une expérience sensorielle et émotionnelle désagréable, associée à une lésion tissulaire existante ou potentielle, ou décrite en termes d'une telle lésion ». La douleur est une expérience personnelle, subjective, et un phénomène complexe représenté selon un modèle multidimensionnel comprenant des composantes sensori-discriminatives, affectives et émotionnelles, cognitives et comportementales. Depuis plusieurs décennies il existe une prise de conscience des corps médicaux, paramédicaux et des pouvoirs publics concernant l’importance d’une prise en charge optimale de la douleur, notamment chez les personnes âgées. En effet, la population gériatrique comporte des spécificités auxquelles tout praticien doit savoir s’adapter, tant sur le plan somatique que psychologique, social, culturel, dans un contexte de polypathologie et souvent de polymédication. L’évaluation de la douleur peut représenter un véritable défi au sein de ces populations, d’autant plus lorsqu’il existe des troubles cognitifs. Des outils d’évaluation uni- ou multi-dimensionnels ont été validés et diffusés. Malheureusement de nombreuses études montrent que la douleur est encore sous diagnostiquée, mal évaluée et sous traitée chez les sujets âgés. Pour le praticien l’enjeu sera de mener la stratégie diagnostique, évaluative et thérapeutique la plus adaptée possible. Il devra tenir compte du risque iatrogène de cette population, en favorisant le développement des interventions non médicamenteuses .
