Book excerpt: A troubling study of the role that medical racism plays in the lives of black women who have given birth to premature and low birth weight infants Black women have higher rates of premature birth than other women in America. This cannot be simply explained by economic factors, with poorer women lacking resources or access to care. Even professional, middle-class black women are at a much higher risk of premature birth than low-income white women in the United States. Dána-Ain Davis looks into this phenomenon, placing racial differences in birth outcomes into a historical context, revealing that ideas about reproduction and race today have been influenced by the legacy of ideas which developed during the era of slavery. While poor and low-income black women are often the “mascots” of premature birth outcomes, this book focuses on professional black women, who are just as likely to give birth prematurely. Drawing on an impressive array of interviews with nearly fifty mothers, fathers, neonatologists, nurses, midwives, and reproductive justice advocates, Dána-Ain Davis argues that events leading up to an infant’s arrival in a neonatal intensive care unit (NICU), and the parents’ experiences while they are in the NICU, reveal subtle but pernicious forms of racism that confound the perceived class dynamics that are frequently understood to be a central factor of premature birth. The book argues not only that medical racism persists and must be considered when examining adverse outcomes—as well as upsetting experiences for parents—but also that NICUs and life-saving technologies should not be the only strategies for improving the outcomes for black pregnant women and their babies. Davis makes the case for other avenues, such as community-based birthing projects, doulas, and midwives, that support women during pregnancy and labor are just as important and effective in avoiding premature births and mortality.

Book excerpt: Black women have had a long history of poorer access to quality medical services as well as a history of being mistreated and not listened to within the medical system (Petersen et al., 2019). With regard to maternal healthcare services related to pregnancy and the birthing process, the inequities and mistreatment of black women place both mothers and their infants at higher risks of life-threatening medical complications, and traumatizing experiences during the process. "During 2007-2016, a total of 6,765 pregnancy-related deaths occurred in the United States (Pregnancy related mortality ratios (PRMR) = 16.7 per 100,000 births). PRMRs were highest among black (40.8) and AI/AN (29.7) women; these rates were 3.2 and 2.3 times the PRMR for white women" (Petersen et al., 2019). Doulas have been defined as a trained professional who provides continuous physical, emotional and informational supports to mother before, during and shortly after childbirth to help her achieve the healthiest, most satisfying experience possible (DONA, 2015). A growing body of literature demonstrates that when women are attended by a doula, there are positive medical outcomes for both mother and baby. Further, research suggests a favorable relationship between labor support and improved birth outcomes, psychologic benefits for the woman, and enhanced fetal well-being with doula support (Sauls, 2002). The purpose of this scoping review was to understand, summarize, determine and define what is currently known about the experiences, supports and services doulas provide in regards to Black women. Literature from studies and articles over the last ten years, between 2010-2021 from a variety of databases were used. Narratives of doulas and mothers were explored assessing the overall experiences to understand how the assistance of doulas during and after birth can help to improve the emotional and mental health wellbeing of Black women in the birthing process. Findings from this review raises awareness and offers information about the need for advocacy and support for black birthing communities.
