The authors use local-level data to test for fungibility of World Bank financing of rural road rehabilitation targeted to specific geographic areas of Vietnam. A simple double difference estimate suggests that the project's net contribution to rehabilitated road increments is close to zero, suggesting complete displacement of funding. However, with better controls for the endogeneity of project placement the authors find much less evidence of fungibility, with displacement accounting for around one-third of the aid. In an ideal democracy, all citizens should have equal influence on government policy—but as this book demonstrates, America's policymakers respond almost exclusively to the preferences of the economically advantaged. Affluence and Influence definitively explores how political inequality in the United States has evolved over the last several decades and how this growing disparity has been shaped by interest groups, parties, and elections. With sharp analysis and an impressive range of data, Martin Gilens looks at thousands of proposed policy changes, and the degree of support for each among poor, middle-class, and affluent Americans. His findings are staggering: when preferences of low- or middle-income Americans diverge from those of the affluent, there is virtually no relationship between policy outcomes and the desires of less advantaged groups. In contrast, affluent Americans' preferences exhibit a substantial relationship with policy outcomes whether their preferences are shared by lower-income groups or not. Gilens shows that representational inequality is spread widely across different policy domains and time periods. Yet Gilens also shows that under specific circumstances the preferences of the middle class and, to a lesser extent, the poor, do seem to matter. In particular, impending elections—especially presidential elections—and an even partisan division in Congress mitigate representational inequality and boost responsiveness to the preferences of the broader public. Various and distinguished scholars - including economists, theologians, philosophers, psychologists, physicians, and anthropologists - have weighed in on this debate in recent years. So too have lawyers, politicians, and other governmental officials. In Iran and India, living donors themselves have been given voice by Zargooshi and Goyal et al., whose compelling work has shown that quot;paid donationquot; in conditions of abject poverty can have disastrous consequences for the quot;donors.quot; However, given the conditions in which these donations took place, there may be significant limitations to the generalizability of the findings, particularly with regard to their applicability to the case of paying for organs in the US. In this paper, we hope to shed light on the issue of paying for organs in the context of the US by giving voice to would-be donors in the US who were ruled out because of incompatibility. Toward that end, we provide an overview of the current state of the ethical debate on the issue of paying for organs and then present these ruled-out donors' opinions on various aspects of this issue. While recognizing that these findings do not definitively answer all the questions at hand, we think they represent important contributions that bring us closer to answering some of the critical questions that demand further empirical study, including whether paying for organs will achieve the presumed goal of increasing the number of suitable organs available for transplantation.

Resources, bibliography, and index occupy a full 36 pages, yet for the most part this book escapes the drudgery of a research-laden study and instead reads as a fascinating story about a very human issue. By statutory authority this edition may be cited "U.S.C. 2012 ed." As adopted in 1926, the Code established prima facie the general and permanent laws of the United States. The underlying statutes reprinted in the Code remained in effect and controlled over the Code in case of any discrepancy. In 1947, Congress began enacting individual titles of the Code into positive law. When a title is enacted into positive law, the underlying statutes are repealed and the title then becomes legal evidence of the law. Currently, 26 of the 51 titles in the Code have been so enacted. These are identified in the table of titles near the beginning of each volume. Powell shows that contributions have considerable influence in some state legislatures but very little in others. Using a national survey of legislators, she develops an innovative measure of influence and delineates the factors that explain this great variation across the 99 U.S. state legislative chambers. Powell identifies the personal, institutional, and political factors that determine how much time a legislator devotes to personal fundraising and fundraising for the caucus. She shows that the extent of donors' legislative influence varies in ways corresponding to the same variations in the factors that determine fundraising time. She also confirms a link between fundraising and lobbying with evidence supporting the theory that contributors gain access to legislators based on donations, Powell's findings have important implications for the debate over the role of money in the legislative process.

Book excerpt: In 1997, the Institute of Medicine published a report entitled Non-Heart- Beating Organ Transplantation: Medical and Ethical Issues in Procurement. The findings and recommendations of that study defined the ethical and scientific basis for non-heart-beating organ donation and transplantation, and provided specific recommendations for practices that affirm patient welfare, promote patient and family choice, and avoid conflicts of interest. Following the 1997 study, the Department of Health and Human Services requested a follow up study to promote such efforts. The central activity for this study was a workshop held in Washington, D.C., on May 24-25, 1999. The workshop provided the opportunity for extensive dialogue on non-heart-beating organ donation among hospitals and organ procurement organizations (OPOs) that are actively involved in non-heartbeating organ and tissue donation and those with concerns about whether and how to proceed. The findings and recommendations of this report are based in large measure on the discussions and insights from that workshop. Non-Heart-Beating Organ Transplantation includes seven recommendations for developing and implementing non-heart-beating-donor protocols. These recommendations were based on the findings and recommendations from the 1997 IOM report and consensus achieved among participants at the national workshop. The committee developed these recommendations as steps towards an approach to non-heart-beating-donor organ donation and procurement consistent with underlying scientific and ethical guidelines, patient and family options and choices, and public trust in organ donation.
