Malgré la prise en charge adjuvante, une partie des patientes développent des métastases non curables. L'objectif du traitement de la maladie métastasique vise à prolonger la survie en contrôlant les symptômes. L'arrivée des thérapies ciblées a contribué à d'immenses progrès cliniques et un allongement de la survie des malades. L'évolution des stratégies thérapeutiques s'accompagne d'un accroissement considérable des dépenses liées aux anticancéreux. L'objectif de notre étude rétrospective monocentrique est d'évaluer l'impact clinique et économique de la prise en charge du cancer du sein métastasique au CHU de Besançon sur une période de 10 ans. Deux cent soixante patientes traitées de 1998 à 2007 sont incluses. L'analyse économique tient compte des dépenses liées aux anticancéreux, aux hospitalisations de jour et aux transports. Les tumeurs sur-exprimant HER2 sont distinguées des autres. L'ensemble de notre cohorte a une médiane de survie de 33 mois et un coût global de prise en charge de 44 000 euros. Dans les tumeurs HER2 négatif, les facteurs significatifs de survie globale en analyse multivariée sont l'âge, la présence de métastases viscérales, la poursuite du traitement au-delà de 3 lignes, et surtout le recours au bevacizumab. Avec ce recours, la survie passe de 28 à 43 mois ; en parallèle, le coût global de la prise en charge passe de 14 800 à 70 900 euros par patiente. Chez les patientes dont la tumeur sur-exprime HER2, le pronostic global (58 mois) et le coût de prise en charge sont stables depuis l'utilisation systématique du trastuzumab.
