ICC abutments lacking structures for vertical stop might sink axially into the inner surface of the implant. Axial displacement is caused by factors u2014 machining tolerance and the settling and wedge effects. Machining tolerance and the settling effect are inevitable phenomena in both external and ICC implants. However, the wedge effect, which induces concentrated axial compressive force in the direction of abutment sinking, is observed only in ICC implants. Although axial displacement in screw-type prostheses has been reported previously, no information is available regarding axial displacement in cement-retained prostheses. Differences in implantu2013abutment connection type have been known to result in variations in axial displacement in various clinical situations. Three typical implantu2013abutment connectionsu2014the external-butt (EXT), internal-conical (INT), and internal-butt (INT-Butt) connectionsu2014have been known to provide satisfactory results in clinical practice.Aim/Hypothesis:The purpose of this study was to quantitatively evaluate axial displacement in three different implantu2013abutment connections in cement-retained monolithic zirconia and polymethylmethacrylate (PMMA) provisional prostheses with the computer aided designu2013computer aided manufacturing (CAD-CAM) custom abutments.Materials and Methods: Three types of implants with different connectionsu2014external and internal butt connection implants and an internal conical connection implantu2014were evaluated. In each implant group, implant replicas were embedded vertically and angulated at 15u00b0. Sixty titanium custom abutments were fabricated using a CAD-CAM system. The geometries and surface morphologies of custom and ready-madeabutments were comparatively evaluated by scanning electron microscopy. Micro-gaps in abutment connections were scanned byoptical microscopy during screw tightening, re-tightening, and cyclic loading. Zirconia and provisional acrylic prostheses were fabricated and evaluated for internal gap by an indirect silicone technique. Cemented prostheses on abutments were mounted on a universal testing machine and subjected to 250 N sine wave cyclic loads. Cumulative axial displacement was measured at loadingperiods of 3, 10, 100, and 1,000,000 and analyzed by repeated measures analysis of variance (ANOVA). Internal gap width and netchange in axial displacement were analyzed by multi-way ANOVA.Results: There were no significant differences in internal gap widths of prostheses according to implant or prosthesis type. However,measurement site and implant angulation were influencing factors on internal gap. Surface geometries and morphologies of custom abutments varied according to the implantu2013abutment connection. Machining tolerance control and precision fit were difficult to achieve with the custom CAD-CAM abutment. The internal and external conical connection implants exhibited the greatest and lowest axial displacements, respectively. Acrylic prostheses exhibited greater axial displacement than zirconia prostheses. The vertically positioned implant group exhibited greater axial displacement than the angulated implant group in the early cyclic loading periods. Axial displacement in the late cyclic loading period was relatively low.Conclusions and Clinical implications:Custom abutments for cement-retained prostheses exhibited greater axial displacement than screw-retained prostheses. Axial displacement mostly occurred in the early loading period and was self-limited. Axial displacement should be managed by provisional restoration, with consideration of implant connection type and angulation, abutment fabrication method, and occlusal force of the patient.

The study included the following groups, Group 1: CAD/CAM produced all-zirconia abutments, Group 2: titanium abutments, Group 3 zirconia-abutments adhesively luted to a titanium base, Group 4: prefabricated all-zirconia abutments and Group 5: zirconia-abutments glass soldered to a titanium base. Half the number of samples in each group was exposed to 1.2 million loading cycles (5-years simulation) in the chewing simulator. The samples that survived the artificial aging were later tested for fracture strength in a universal testing machine. The remaining 8 samples of the group were directly tested for fracture strength.ResultsAll samples exposed to the 5-years artificial aging survived except of six samples in one group (Group 1). The surviving samples were later fracture tested in the universal testing machine. The bending moments (Ncm) values were as follow: Exposed groups: Group 1: 94.5 Ncm; Group 2: 599.2 Ncm; Group 3: 477.5 Ncm; Group 4: 314.4 Ncm; Group 5: 509.4 Ncm. Non-exposed groups: Group 1: 269.3 Ncm; Group 2: 474.2 Ncm; Group 3: 377.6 Ncm; Group 4: 265.4 Ncm; Group 5: 372.4 Ncm. Except in Group 1, the values were higher in the exposed groups, although, statistically there was no difference (p>0.05). The one-piece ZrO2-abutment group (Group 1 and Group 4) exhibited lower values, while the two-piece ZrO2-abutment groups (Group 3 and Group 5) showed similar values and fracture modes like the titanium abutment group. The titanium abutment group showed the highest values of bending moments among all groups.ConclusionThe implant-abutment connection area appeared to influence the bending moment value and the fracture mode of the tested abutment groups, and it was found to be the weakest part of an internal connection one-piece zirconia abutment. The titanium base in the two-piece zirconia abutment worked as a substitute for the weakest part of the abutment. Therefore, the titanium base can reinforce the fracture strength of a zirconia abutment

Yet, it is still not clear howstable these reconstructions are.Aim: To assess the bending moments and fracture patterns of zirconia mesio-abutments on titanium bonding-bases restored with differentall-ceramic monolithic crowns and compare them with titanium abutments restored with all-ceramic crowns.Materials and Methods: 48 implants with an internal connection (Conelog, Camlog) were restored with 4 different CAD-CAM abutment-crown combinations(n=12). 36 customized zirconia mesio-abutments were bonded to titanium bonding-bases and divided into 3 groups according to thedifferent crown material: (T1) monolithic lithium disilicate (e-max CAD, Ivoclar); (T2) monolithic zirconia (Lava Plus, 3M ESPE) and(T3) monolithic hybrid ceramic (Enamic, Vita) Titanium customized abutments restored with monolithic lithium disilicate (e-max cad,Ivoclar) crowns served as control group (C). All crowns were cemented using a resin-cement (Panavia 21, Kuraray). The sampleswere embedded in acrylic holders. After aging (1'200'000 cycles, 49 N, 1.67 Hz, 5u00b0C-50C, 120s), static load was applied until failure(ISO-Norm 14801). Bending moments were calculated for comparison of the groups. Values were statistically compared with one-wayANOVA followed by Tukey post hoc test (p.05). Failure mode was analyzed descriptively.Results: The means of the bending moments were 356.4 u00b1 20.8 Ncm (T1), 385.5 u00b1 21.2 Ncm (T2), 357.7 u00b1 26.3 Ncm (T3) and 358.8 u00b1 25.3Ncm (C). Group T2 exhibited significantly higher mean bending moments than the other groups (p 0.05). No differences were foundbetween zirconia mesio-abutments on titanium bonding-base abutments and customized titanium abutments (p > 0.05). No failureswere registered during the aging. In all the groups failures occurred due to fracture of the abutment in the internal connection.Conclusions and clinical implications: The use of zirconia mesio-abutments bonded to titanium bonding-bases appeared to be a stable combination when compared to thecustomized titanium abutments which still represent the gold-standard. Regarding the crown material, monolithic zirconia revealed tobe mechanically superior than the other crown materials.

Book excerpt: Abstract: Introduction: An all-ceramic abutment system offers superior esthetics, translucency, biocompatibility and strength in comparison to metal abutments. Naturally, with all the variables presented by human beings, there is a risk that the final location and angulation of the implant would be not as exact as planned. The restorative dentist has an option to use prefabricated ceramic abutments and customize them as necessary to achieve highly esthetic results. However, there is not much information on how different abutment angulations affect strength. The new Procera system (Nobel Biocare) offers customized abutments in wide variety of clinical situations, which would be especially convenient in the restorative esthetic cases of angled implants. Purpose: This study examined the effects of implant angulation on the strength of ceramic abutment designs. Materials and methods: Twenty custom made Procera (Nobel Biocare) zirconia abutments fabricated with CAD/CAM technique were used in this study. The typical design of an abutment for a cemented central incisor crown was used. The strength of zirconium abutments was measured using four different axial alignments of implants: Group A, axis of implant goes through the cingulum area, Group B, through the incisal edge, Group C, 150 labial inclination and Group D, the anterior angled limit allowed by the abutment screw height (300). Using an MTS hydraulic machine, load was applied to a consistent palatal/incisal reference point on each abutment. Measurements of fracture load were collected and analyzed using ANOVA. Where ANOVA detected a difference between tested variable levels, REGWQ test was performed and determined which variable level was more significant. Results: Group A withstand significantly (p=0.005) higher fracture load than Group B, Group C and Group D. Mean peak load values were 518.7 N, 269.4 N, 232.0 N and 227.4 N, respectively. Conclusion: Comparison of fracture load measurements of different abutment angulations revealed the most fracture resistant abutment design. Results from this study will help to create guidelines for prosthodontists in making decisions in various clinical situations and improve pre-operative treatment planning, particularly as they relate to the Procera (Nobel Biocare) all-ceramic system.

For several years, stock abutments have been the only option available for clinicians.Custom made CAD/CAM abutment acts as adequate support for soft tissues, and the connection between abutment and a cement-retained crown is moved more towards coronal, which makes residual cement removal easier during cementation of a prosthetic restoration.Aim/HypothesisThe aim of this study was to compare stock and individual CAD/CAM abutments after one year in function. A total of 64 subjects with one missing tooth, were included in this study. The missing tooth was replaced with a dental implant, an abutment and a porcelain-fused-to-metal crown.Materials and MethodsA total of 64 subjects had a single tooth loss restored by dental implant and porcelain-fused-to-metal crowns were fabricated. In one group there were a total of 34 stock Ti abutments and in the second group of subjects 30 CAD/CAM made Ti abutments. Examination of soft tissues around dental implants included determination of modified plaque index (mPI) and modified bleeding index (mBI), as well as measurement of sulcus probing depth (PPD). The average modified plaque index (mAPI) was calculated from the measured values, as the average measurement value on all four crown surfaces. The average modified bleeding index (mABI) was calculated in the same way. PPD measurement was performed on the mesial, distal, vestibular and lingual crown surface, and the average values of measured probing depth (APPD) were calculated. The values mAPI, mABI and APPD were measured during the first check-up (a week after cementing the crown) and at check-ups after four, eight and twelve months.ResultsAverage plaque index values (API), initial and after four, eight and twelve months, have not indicated significant differences between two groups of subjects. Average bleeding index values - initial (i-ABI) and after four months (4m-ABI) revealed no significant differences between two tested groups.
