Book excerpt: Excerpt from Diseases of the Heart and Arteries: Their Causes, Nature and Treatment A colleague, a patient of mine, now on the retired list, was many years ago refused by a life insurance office because he had a valvular defect in his heart. Now, if he had been a layman, or if he had not had the advantage of knowing something more than is taught in ordinary text books, he might have done as many a one has done under like circumstances - gone home, given himself up for lost, and verified the insurance doctor's prognosis by dying in a short time. He did nothing of the kind. He went to work as usual, made up his mind to insure himself, and has lived long enough to save the sum he would have paid in premiums three or four times over. Not very long ago an old patient of mine came to me in a terrible state of agitation. A note he brought me, announcing the fact that an insurance office to which he had applied for a policy could not see its way to accept him, explained his trouble. He had a valvular defect, it is true, but it was fully compensated. I assured him that there was not the least necessity for him to regard the refusal as a sentence of death, and advised him to follow the example of my friend and insure himself. Life took a new aspect to him he recovered his spirits and has not required any assistance from me since that time. As my object is to deal more especially with the therapeutic: of heart disease, I shall only devote to the consideration of the pathology of diseases of the heart and its vessels just so much Space as is needed for my purpose. My work on Iodide of Arsenic in Organic Disease of the Heart being now out of print, I have made use of the material it contained in the present volume, and also of the cases narrated in a paper which I contributed to the World's Homoeopathic Congress of r893, entitled The Curative Action of Homoeopathic Remedies in Cases of Organic Disease of the Heart. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Cardiovascular disease now accounts for nearly 30 percent of deaths in low and middle income countries each year, and is accompanied by significant economic repercussions. Yet most governments, global health institutions, and development agencies have largely overlooked CVD as they have invested in health in developing countries. Recognizing the gap between the compelling evidence of the global CVD burden and the investment needed to prevent and control CVD, the National Heart, Lung, and Blood Institute (NHLBI) turned to the IOM for advice on how to catalyze change. In this report, the IOM recommends that the NHLBI, development agencies, nongovernmental organizations, and governments work toward two essential goals: creating environments that promote heart healthy lifestyle choices and help reduce the risk of chronic diseases, and building public health infrastructure and health systems with the capacity to implement programs that will effectively detect and reduce risk and manage CVD. To meet these goals, the IOM recommends several steps, including improving cooperation and collaboration; implementing effective and feasible strategies; and informing efforts through research and health surveillance. Without better efforts to promote cardiovascular health, global health as a whole will be undermined.
