Book excerpt: Ziel dieser Studie ist Veränderungen des bi-exponentiellen Signals bei Patienten mit mesialer Temporallappen-Epilepsie und Hippocampus-Sklerose zu charakterisieren. Dies erfolgt durch Vergleich zu einer im Alter und Geschlecht angepaßten Referenzgruppe. Diffusionsparameter werden nach diffusionsgewichteter Bildgebung an 3-Tesla MRT, mit mono- und bi-exponentiellem Verfahren berechnet. Dabei werden die sog. MRT-Rohdaten mit Hilfe von Matlab Program, FMRIB-Software und SPSS Statistik-Analyse analisiert und bewertet. Bi-exponentielle Anpassungsfunktion ergibt Parameter von rapiden und langsamen Diffusionskomponenten und deren Fraktion, wobei eine Analyse mit b-Werten bis 3000 s/mm2 wird realisiert. Resultat mono-exponentieller Approximation zeigt Amygdala, Hippocampus und normal erscheinende Weißgewebe-Gebiete mit erhöhter Apparent-Diffusion. Hingegen liegen Fraktionswerte der langsamen Diffusionskomponenten niedriger im Vergleich zu identischen Parameter der Referenzgruppe. Diese Daten demonstrieren: Veränderungen der molekularen Umgebung bei Patienten mit unilateraler mesialer Temporallappen-Epilepsie beeinflussen sowohl Grau- wie Weißgewebe-Strukturen und verursachen eine Verschiebung einzelner Wasserfraktionen im Gehirn.*****The aim of this work is to characterize bi-exponential diffusion signal changes in mesial temporal lobe epilepsy patients with hippocampal sclerosis compared to an age- and sex-matched control group. Diffusion parameters are measured by mono- and bi-exponential approaches after acquiring diffusion-weighted images on a 3T MRI system. The raw data is analyzed using Matlab program, FMRIB software and SPSS statistical application. Bi-exponential fits yield parameters for the fast and the slow diffusion components and the fraction of them using high b-value analysis up to 3000 s/mm2. As a result of mono-exponential fitting, amygdala, hippocampus and normal appearing white matter show increased apparent diffusion, while the fraction of the slow diffusion component is decreased compared to the control group. These data demonstrate that the alteration in molecular environment of unilateral mesial temporal lobe epilepsy affects diffusion in both gray and white matter structures causing the shift between different water fractions.
