Book excerpt: Are you constantly online? Or are you offline sometimes? Are you offline if you are not interacting with your connected devices? Or if no data about you is being collected? Do you check Instagram and Twitter during dinner? Do you turn off your smartphone at night? Do you check work emails on vacation? Do you feel you have to disconnect regularly – to relax, to concentrate, or to protect your privacy? Or do you feel more relaxed when constantly connected because your loved ones, a work emergency, or the news are always at your fingertips? Why are some people – even within networked societies – still completely offline given the tremendous opportunities of the Internet? And what does it even mean to be online or offline in the age of hyper-connectivity? In ON/OFF, Sarah Genner assesses the risks and rewards of the anytime-anywhere Internet, focusing on digital divides, social relationships, physical and mental health, and data privacy. She discusses implications for a variety of decision-makers in the world of work, in education, in families, and in politics. The author deconstructs the online/offline dichotomy and suggests the ON/OFF scale as a new theoretical framework for researchers and practitioners.

Available in PDF, EPUB and Kindle. Book excerpt: Can private health insurance fill gaps in publicly financed coverage? Does it enhance access to health care or improve efficiency in health service delivery? Will it provide fiscal relief for governments struggling to raise public revenue for health? This book examines the successes, failures and challenges of private health insurance globally through country case studies written by leading national experts. Each case study considers the role of history and politics in shaping private health insurance and determining its impact on health system performance. Despite great diversity in the size and functioning of markets for private health insurance, the book identifies clear patterns across countries, drawing out valuable lessons for policymakers while showing how history and politics have proved a persistent barrier to effective public policy. Available in PDF, EPUB and Kindle. Book excerpt: The German health care system is on a collision course with budget realities. Costs are high and rising, and quality problems are becoming ever more apparent. Decades of reforms have produced little change to these troubling trends. Why has Germany failed to solve these cost and quality problems? The reason is that Germany has not set value for patients as the overarching goal, defined as the patient health outcomes achieved per euro expended. This book lays out an action agenda to move Germany to a high value system: care must be reorganized around patients and their medical conditions, providers must compete around the outcomes they achieve, health plans must take an active role in improving subscriber health, and payment must shift to models that reward excellent providers. Also, private insurance must be integrated in the risk-pooling system. These steps are practical and achievable, as numerous examples in the book demonstrate. Moving to a value-based health care system is the only way for Germany to continue to ensure access to excellent health care for everyone.

Book excerpt: This book provides a detailed introduction to how the Lander (the 16 states of Germany) function not only within the country itself but also within the wider context of European political affairs. Some knowledge of the role of the Lander is essential to an understanding of the political system as well as of German federalism. This book traces the origin of the Lander. It looks at their place in the constitutional order of the country and the political and administrative system. Their organization and administration are fully covered, as is their financing. Parties and elections in the Lander and the controversial roles of parliaments and deputies are also examined.

Book excerpt: Cognitive enhancement is the use of drugs, biotechnological strategies or other means by healthy individuals aiming at the improvement of cognitive functions such as vigilance, concentration or memory without any medical need. In particular, the use of pharmacological substances (caffeine, prescription drugs or illicit drugs) has received considerable attention during the last few years. Currently, however, little is known concerning the use of cognitive enhancers, their effects in healthy individuals and the place and function of cognitive enhancement in everyday life. The purpose of the book is to give an overview of the current research on cognitive enhancement and to provide in-depth insights into the interdisciplinary debate on cognitive enhancement.

The author, formerly director at the Max-Planck-Institute for European Legal History, Frankfurt, provides a perceptive overview of the history of social security and social welfare in Germany from early modern times to the end of World War II, including Bismarck’s pioneering introduction of social insurance in the 1880s. The author unravels “layers” of social security that have piled up in the course of history and, so he argues, still linger in the present-day welfare state. The account begins with the first efforts by public authorities to regulate poverty and then proceeds to the “social question” that arose during the 19th-century Industrial Revolution. World War I had a major impact on the development of social security, both during the war and after, through the exigencies of the war economy, inflation and unemployment. The ruptures as well as the continuities of social policy under National Socialism and World War II are also investigated.

Available in PDF, EPUB and Kindle. Book excerpt: Health insurance is a key component of the current social security system in European Union countries. In most countries, modern health insurance funds and health care insurers are an essential role in implementing the public health insurance system. Many of these modern health insurance funds have a fascinating and long ancestry, clear traces of which can be seen today in the organisation of national health insurance, as well as the structure of health insurance funds and insurers. In their study Two Centuries of Solidarity, the authors compare health insurance, health insurance funds and health care insurers in Germany, Belgium and the Netherlands. Given the similar political, economic and social development that these countries have undergone in the past 60 years and the qualitatively high level of health care they provide, one might expect a degree of likeness in these countries' health care insurance systems. The dissimilarities are surprising, however. In fact, differences are becoming ever more apparent between the different national systems in general, and between the structure and operation of the health insurance funds and health care insurers in particular. Differences include the compulsory nature of insurance, the extent of cover, premiums, health insurance business, mutual competition, and the degree of private insurance. Many of these national singularities can be understood and explained only by considering the historical background of the current national health insurance systems, the insurers, and their evolution over de past centuries. This study adopts an institutional and political perspective towards further understanding of the development of health insurance, and of how this ultimately determined the specific nature of the health care insurers and funds, and how they currently operate in the three countries studied.
