Book excerpt: Glaucoma is a condition of the eye in which the optic nerve is damaged due to increased fluid pressure in the eye. Left untreated, the condition may lead to permanent blindness. Optical coherence tomography (OCT) is a non-invasive imaging test that uses light waves to take cross-section pictures of the retina, the light-sensitive tissue lining the back of the eye (geteyesmart.org). OCT is commonly used in the evaluation of patients with glaucoma. This manual is a concise guide to the use of OCT for the diagnosis of glaucoma. Beginning with an introduction to OCT, the book then provides in depth discussion on its use in glaucoma. Each of the following chapters describes the use of OCT for analysing associated parts of the eye, including the optic nerve, retinal nerve and ganglion cell, as well as macular and anterior segment OCT. The advantages and common pitfalls in OCT imaging and its interpretation are discussed at length. Key points Concise guide to OCT for diagnosis and evaluation of glaucoma Explains use of OCT for analysis of associated parts of the eye In depth discussion of advantages and common pitfalls in OCT imaging Includes more than 115 images and illustrations

Book excerpt: Glaucoma is a general term that includes an array of ocular conditions that cause a specific neuropathy of the optic nerve (Greenfield, Bagga, et al. 2003) of which abnormalities associated with this disorder are localized at the level of the retinal ganglion cell layer (Epstein 1997; Quigley & Broman 2006). This structure-function relationship is not clear as it relies on several factors such as variability from the structural and functional tests, differences in measurement scales between the two modalities (Greaney et al. 2002; Katz 1999; Drance 1985; Hood et al. 2007) and physiological variation amongst individuals (Pan & Swanson 2006). The global aim of this thesis was to relate visual function of the retinal ganglion cells to structure of the optic nerve head and retinal nerve fiber layer with respect to the following perimetry techniques: i) standard automated perimetry (SAP), ii) frequency doubling technology (FDT), iii) flicker defined form (FDF), and iv) the motion detection test (MDT), and the following imaging instruments: i) confocal scanning laser ophthalmoscopy (HRT), ii) optical coherence tomography (OCT), and iii) scanning laser polarimetry (GDx VCC). The specific purpose of this study was to i) compare the test-retest characteristics of the perimetry techniques, ii) determine which may be more sensitive for early detection, iii) evaluate the structure-function relationship between measures of retinal nerve fiber layer and visual function, and iv) perform a preliminary study to determine which techniques may be most suitable to monitor progression, in patients with early stage glaucoma. MDT showed little change in the 1-year follow-up study thus being unsuitable for monitoring change. FDT and FDF gave a similar performance and are likely optimal for the detection of early functional damage. Poor diagnostic agreement was seen between the HRT and each perimetry technique. Because no one perimetry test showed both high sensitivity and high specificity, it is recommended that a combination of FDF with either SAP, FDT or MDT be used as the functional component in the diagnosis and follow-up of patients with glaucoma. The strongest global structure-function correlations for OCT were seen with SAP, FDT and MDT; for GDx, the strongest association was seen with FDF. These results suggest that FDF and GDx used in combination are best to detect early glaucomatous changes.

Open-angle glaucoma, closed-angle glaucoma and normal-tension glaucoma are some types of glaucoma. Testing for glaucoma comprises of an eye examination with measurements of the intraocular pressure, an anterior chamber angle examination, and optic nerve evaluation. Scanning laser polarimetry, optical coherence tomography and scanning laser ophthalmoscopy help in assessing the retinal nerve fiber. Vision loss due to glaucoma is permanent. If the disease is detected early, it may be possible to slow or even stop the progression of the disease with the aid of laser treatment, medication or surgery. These treatments seek to lower eye pressure. This book is a valuable compilation of topics, ranging from the basic to the most complex advancements in the diagnosis and management of glaucoma. From theories to research to practical applications, case studies related to all contemporary topics of relevance to this condition have been included herein. It will help new researchers by foregrounding their knowledge in glaucoma.

As with prior meetings, it was a daunting task to seek and obtain consensus on broad subject matter that ranges from diagnosis, risk profiling and screening of the disease. As it is unclear how each of us decides how we practice and the evidence to guide us often is sparse, this consensus, as well as the others, is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus is to provide a foundation for diagnosing and managing primary open-angle glaucoma and how it can be best done in clinical practice. Identification of those areas for which we have little evidence and, therefore, the need for additional research always is a high priority. We hope that this consensus report will serve as a benchmark of our understanding. However, this consensus report is intended to be fluid. It is expected that it will be revised and improved with the emergence of new evidence.

Abstract: Abstract of thesis entitled "The characterization of retinal nerve fiber layer thickness in normal, high- tension and normal- tension glaucoma using Optical Coherence Tomography." Submitted by Kwok Hei MOK for the degree of Doctor of Philosophy at The University of Hong Kong in May 2005 Glaucoma is a complex multifactor disorder known to cause progressive loss of retinal ganglion cells and their axons, leading to a reduction in the thickness of the Retinal Nerve Fiber Layer (RNFL). There remains considerable disagreement within the glaucoma community as to the possible morphologic damage pattern differences in the primary chronic open angle glaucoma patients with elevated intra-ocular pressure (HTG) and those with normal intra-ocular pressure (NTG). It would be intriguing to determine if there is any morphological difference in RNFL between HTG and NTG patients since this may provide clues as to possible different factors responsible for the damage to their eyes. iiOptical coherence tomography (OCT, Humphrey systems Inc., Dublin, CA, USA) is a noninvasive technology that allows high resolution (approximately 10μm) in vivo cross-sectional imaging of the retinal structures based on the principle of Michelson interferometry. It allows a direct and objective estimation of RNFL thickness. One major benefit of RNFL assessment using OCT is that twelve clock hour thickness values along a circular path around the optic disc are displayed. This regional data analysis makes it more sensitive to detect localized changes. However, reproducibility of regional data was reported to be unsatisfactory. We found that increasing the number of scans to five scans per examination significantly improves reproducibility of regional values in both healthy and glaucomatous eyes. The peri-papillary RNFL under investigation exhibited double-hump patterns. RNFL was thickest in the supero- and infero-temporal regions (double humps) and thinnest in the temporal and nasal regions (double troughs). We observed that the RNFL in 129 eyes of 129 Hong Kong-Chinese subjects appeared to significantly decrease both over time cross-sectionally (3.3μm per decade, pTo best of my knowledge, there has been no study specifically investigating the profile of RNFL injury at different stages of glaucoma. Specific glaucomatous RNFL profiles injury may not only serve as a useful basis for diagnosing and monitoring disease progression, but also provide insights into the mechanism and pattern of ganglion cell atrophy in glaucomatous optic neuropathy. Glaucomatous RNFL loss occurs with regional preferences at different stages of the disease. We first reported that only infero-and supero-temporal regional values were found to decrease continuously, and that a localized defect was observed first in the infero-temporal region in pre-perimetric and early stages. This finding of preferential damage in the inferior as opposed to superior fibers in the very early stage of glaucoma damage is intriguing, and may shed light into the mechanism of the damage.. In NTG and HTG patients, our findings showed that RNFL topographical changes are similar i
