Book excerpt: Introduction: le vieillissement de la population a entrainé une augmentation du nombre d'admission du sujet âgé en réanimation. Nous avons cherché à connaitre le devenir et la qualité de vie d'une population de patients âgés après un passage en réanimation. Méthode: nous avons inclus tous les sujets de 80 ans et plus ayant séjourné dans le service de réanimation médicale de l'hôpital Henri Mondor entre mai et octobre 2008. Nous avons étudié les caractéristiques de leur séjour en réanimation, leur autonomie et statut cognitif avant l'admission et leur devenir à 3 mois de l'hospitalisation en réanimation. Pour les survivants, nous avons évalué leur degré d'autonomie et leur qualité de vie liée à la santé grâce aux échelles ADL et IADL et au questionnaire générique d'état de santé SF-12. Résultats: nous avons inclus 40 patients dont l'âge moyen était 84.3ans. A 3 mois de la sortie de réanimation, le taux de survie était de 67.5%. Les 40 patients inclus étaient des patients poly pathologiques, avaient de bons scores ADL et IADL moyen, ne présentaient pas de troubles majeurs des fonctions supérieures. 50% des patients étaient hospitalisés pour détresse respiratoire aigue. 27 patients ont été évalués à 3 mois, leur autonomie était conservée et leur qualité de vie était semblable à celle de la population générale de même âge avec un score résumé physique(PCS) plus altéré que le score résumé mental(MCS).85.2% des patients étaient satisfaits de la prise en charge en réanimation et 77.8% accepteraient un nouveau séjour si leur état de santé le nécessitait. Conclusion: trois mois après une hospitalisation en réanimation, les patients ayant été hospitalisés sont autonomes et ne présentent pas de troubles majeurs des fonctions supérieures. Leur qualité de vie est semblable à des sujets de même âge. L'absence de consensus sur le seuil de vieillesse et sur les instruments de mesures à utiliser pour la qualité de vie et les comorbidités rend difficile la comparaison de nos résultats avec les données de la littérature. Cependant, celles-ci s'accordent pour dire que l'âge seul n'est pas un critère suffisant pour refuser une hospitalisation en réanimation, nos résultats vont dans ce sens.

Le bien fondé d'une prise en charge intensive pour les patients âgés repose sur la balance bénéfice-risque liée aux techniques de suppléances invasives. Le devenir de ces patients reste mal connu. MATERIEL ET METHODE : Tous les patients de 80 ans et plus hospitalisés en réanimation médicale entre le 1er juillet 2011 et le 25 avril 2012 à l'hôpital Saint-Anfré du CHU de Bordeaux ont été inclus. Leurs caractéristiques ont été relevées, à savoir leur lieu de vie, leur comorbidité évaluée à l'aide du score de Charlson et de Mac Cabe, leur état nutritionnel, leur autonomie à l'aide du score de Katz, leur provenance, leur diagnostic d'admission, le score IGS2, leur durée de séjour, les moyens entrepris, une décision de limitation ou arrêt thérapeutique. De même leur devenir en termes de mortalité et d'autonomie a été suivi à trois mois à l'aide du score de Katz. RESULTATS : 101 patients de 80ans et plus ont été inclus durant cette période. Ils avaient une autonomie conservée et vivaient à domicile pour 91,5 % d'entre eux. Le diagnostic d'admission le plus fréquent était l'insuffisance respiratoire aigue, 26 % des patients ont été intubés. Une décision de limitation thérapeutique a été prise pour 55 %de ces patients. 24 % sont décédés en réanimation. A trois mois, 13 % des survivants sont décédés. Leur autonomie a diminué avec un score de Katz moyen inférieur à celui d'avant l'hospitalisation. CONCLUSION : Ces résultats confirment que les patients âgés admis en réanimation sont autonomes et ont peu de comorbidités. Ils sont moins intubés et bénéficient plus souvent de séances de VNI que les patients plus jeunes. Leur taux de mortalité est plus élevé mais ils restent peu dépendantds à trois mois et vivent pour la très grande majorité toujours à domicile. Ces résultats encouragent l'admission des patients de 80 ans et plus en réanimation.

Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth.
