Ces maladies sont dues à des mutations codant pour des protéines impliquées dans ces voies métaboliques. Alors que leur prévalence individuelle est faible, on estime leur prévalence collective entre 1/5000 et 1/10000 naissances. L'outil diagnostic principal de ces anomalies est l'analyse de la carnitine et des acylcarnitines dans les liquides biologiques. Actuellement, ce sont des analyses semi-quantitatives qui prédominent, en FIA-MS/MS, sur matrice de sang total. Elles font partie des programmes de dépistage néonatal dans de nombreux pays. Nous avons voulu proposer, au-delà du cadre du dépistage néonatal, une analyse quantitative de la carnitine et des acylcarnitines plasmatiques. Nous avons donc développé une méthode chromatographique couplée à la spectrométrie de masse en tandem. Nous avons procédé à la validation de cette méthode et nous avons effectué une estimation intra-laboratoire des valeurs de référence biologiques pour différentes tranches d'âge. Des améliorations restent à apporter à cette méthode mais nous espérons qu'elle aura un impact clinique positif sur le diagnostic de ces pathologies.
