The LWRS program serves to help the U.S. nuclear industry adopt new technologies and engineering solutions that facilitate the continued safe operation of the plants and extension of the current operating licenses. The long term viability of existing nuclear power plants in the U.S. will depend upon maintaining high capacity factors, avoiding nuclear safety issues and reducing operating costs. The slow progress in the construction on new nuclear power plants has placed in increased importance on maintaining the output of the current fleet of nuclear power plants. Recently expanded natural gas production has placed increased economic pressure on nuclear power plants due to lower cost competition. Until recently, power uprate projects had steadily increased the total output of the U.S. nuclear fleet. Errors made during power plant upgrade projects have now removed three nuclear power plants from the U.S. fleet and economic considerations have caused the permanent shutdown of a fourth plant. Additionally, several utilities have cancelled power uprate projects citing economic concerns. For the past several years net electrical generation from U.S. nuclear power plants has been declining. One of few remaining areas where significant improvements in plant capacity factors can be made is in minimizing the duration of refueling outages. Managing nuclear power plant outages is a complex and difficult task. Due to the large number of complex tasks and the uncertainty that accompanies them, outage durations routinely exceed the planned duration. The ability to complete an outage on or near schedule depends upon the performance of the outage management organization. During an outage, the outage control center (OCC) is the temporary command center for outage managers and provides several critical functions for the successful execution of the outage schedule. Essentially, the OCC functions to facilitate information inflow, assist outage management in processing information and to facilitate the dissemination of information to stakeholders. Currently, outage management activities primarily rely on telephone communication, face to face reports of status and periodic briefings in the OCC. Much of the information displayed in OCCs is static and out of date requiring an evaluation to determine if it is still valid. Several advanced communication and collaboration technologies have shown promise for facilitating the information flow into, across and out of the OCC. Additionally, advances in the areas of mobile worker technologies, computer based procedures and electronic work packages can be leveraged to improve the availability of real time status to outage managers.

Several researchers involved in this pilot project have commercial NPP experience and believed that very little technology has been applied towards outage communication and collaboration. To verify that the technology options researched and demonstrated through this pilot project would in fact have broad application for the US commercial nuclear fleet, and to look for additional outage management best practices, LWRS program researchers visited several additional nuclear facilities.

The Outage Control Center (OCC) Pilot Project was directed at carrying out the applied research for development and pilot of technology designed to enhance safe outage and maintenance operations, improve human performance and reliability, increase overall operational efficiency, and improve plant status control. Plant outage management is a high priority concern for the nuclear industry from cost and safety perspectives. Unfortunately, many of the underlying technologies supporting outage control are the same as those used in the 1980's. They depend heavily upon large teams of staff, multiple work and coordination locations, and manual administrative actions that require large amounts of paper. Previous work in human reliability analysis suggests that many repetitive tasks, including paper work tasks, may have a failure rate of 1.0E-3 or higher (Gertman, 1996). With between 10,000 and 45,000 subtasks being performed during an outage (Gomes, 1996), the opportunity for human error of some consequence is a realistic concern. Although a number of factors exist that can make these errors recoverable, reducing and effectively coordinating the sheer number of tasks to be performed, particularly those that are error prone, has the potential to enhance outage efficiency and safety. Additionally, outage management requires precise coordination of work groups that do not always share similar objectives. Outage managers are concerned with schedule and cost, union workers are concerned with performing work that is commensurate with their trade, and support functions (safety, quality assurance, and radiological controls, etc.) are concerned with performing the work within the plants controls and procedures. Approaches to outage management should be designed to increase the active participation of work groups and managers in making decisions that closed the gap between competing objectives and the potential for error and process inefficiency.

To make matters worse, modern information and communications technology is making it possible to collect and store data faster than our ability to use it for making decisions. However, in most applications, especially outages, raw data has no value in itself; instead, managers, engineers and other specialists want to extract the information contained in it. The complexity and sheer volume of data could lead to information overload, resulting in getting lost in data that may be irrelevant to the task at hand, processed in an inappropriate way, or presented in an ineffective way. To prevent information overload, many data sources are ignored so production opportunities are lost because utilities lack the ability to deal with the enormous data volumes properly. Decision-makers are often confronted with large amounts of disparate, conflicting and dynamic information, which are available from multiple heterogeneous sources. Information and communication technologies alone will not solve this problem. Utilities need effective methods to exploit and use the hidden opportunities and knowledge residing in unexplored data resources. Superior performance before, during and after outages depends upon the right information being available at the right time to the right people. Acquisition of raw data is the easy part; instead, it is the ability to use advanced analytical, data processing and data visualization methods to turn the data into reliable information and comprehensible, actionable information. Techniques like data mining, filtering and analysis only work reliably for well-defined and well-understood problems. The path from data to decision is more complex. The ability to communicate knowledge during outages and emergent issues is crucial. This paper presents an approach to turn the unused data into an opportunity: applying principles from semiotics, human factors and visual analytics to transform the traditional way of processing outage data into media that will improve the collective situation awareness, knowledge, decisions, actions and overall performance of the entire outage team, and also support the reliability, quality and overall effectiveness of maintenance work. The application of the proposed visualization methods will become the medium of a semi-automated analytical process where humans and machines cooperate using their respective, distinct capabilities for the most effective results.

Three effective solutions: a steady state control scheme, a transient stability control scheme, and an interactive system-wide and local scheme have been proposed using those new methods and tools. A steady state control scheme can help detect and prevent the possible cascading outage at its initial slow steady state progress stage. It uses new methods and new tools to solve the line overload, congestion or bus high/low voltage problems. New methods, such as vulnerability index (VI), margin index (MI), network contribution factor (NCF), topology processing and selected minimum load shedding (SMLS), and new tools, such as transmission network control based on a network contribution factor (NCF) method, generator control based on a generator distribution factor (GDF) method, and load control based on a load distribution factor (LDF) method have been proposed and developed. A transient stability control scheme can help prevent and mitigate the possible cascading outage at its transient progress stage if there is enough time to take action. It uses one Lyapunov direct method, potential energy boundary surface (PEBS) method, and sensitivity analysis of transient energy margin for fast stabilizing control. The results are verified by the accurate time-domain transient stability analysis method. The interactive scheme takes advantage of accurate system-wide and local information and analysis results, uses some techniques from both steady state control and transient stability control, works at both the system-wide level and local substation level, monitors the system all the time, and takes actions when needed to help detect, prevent and mitigate the possible cascading outage. Comprehensive simulation studies have been implemented using the IEEE 14- bus, 24-bus, 39-bus and 118-bus systems and promising results show the ability of the proposed solutions to help detect, prevent and mitigate cascading outages.

Book excerpt: Occurrence of outages in overhead distribution systems is a significant factor in determining distribution system reliability. Analysis of animal-related outages has practical value since animals cause a large number of outages in overhead distribution systems. This dissertation presents several different methods to investigate the impact of weather and time of the year on the animal-related outage rate. The animal-related outages from year 1998 to year 2007 for different cities in Kansas are provided by Westar Energy. From examinations of the historical data, two factors which influence the animal-related outages, the month type and the number of fair weather days are taken as inputs along with historical outage data for prediction models. Poisson regression model, neural network model, wavelet based neural network model and Bayesian model combined with Monte Carlo simulations are applied to the weekly data of different cites. Even though Poisson regression models, Bayesian models and neural network models are able to recognize the changing pattern of outage rates under different weather conditions, they are limited in their ability to follow the high peaks in the time series of weekly animal-related outages. The introduction of wavelet transform techniques overcomes this problem. Simulation results indicate that the wavelet based neural network models are able to capture the pattern of fast fluctuations in the weekly outages of different cities in Kansas of various sizes. A hyperpermutation method inspired by artificial immune system algorithm is used to solve the overtraining problem in the application of neural networks. Finally, Monte Carlo simulations based on conditional probability tables from Bayesian models are used to find out the confidence intervals of the predictions. We aggregate the weekly data and carry out the analysis on a monthly and yearly basis too. Simulation results indicate that the models are able to capture the pattern as at least 90% of the observed values are within the upper limits of 95% confidence in the predictions for weekly, monthly and yearly animal-related outages of different cities in Kansas. The results obtained from Monte Carlo simulations are compared with the wavelet based neural network model to indentify years with more than expected level of outages.
