Book excerpt: Iterative learning control (ILC) has its origins in the control of processes that perform a task repetitively with a view to improving accuracy from trial to trial by using information from previous executions of the task. This brief shows how a classic application of this technique – trajectory following in robots – can be extended to neurological rehabilitation after stroke. Regaining upper limb movement is an important step in a return to independence after stroke, but the prognosis for such recovery has remained poor. Rehabilitation robotics provides the opportunity for repetitive task-oriented movement practice reflecting the importance of such intense practice demonstrated by conventional therapeutic research and motor learning theory. Until now this technique has not allowed feedback from one practice repetition to influence the next, also implicated as an important factor in therapy. The authors demonstrate how ILC can be used to adjust external functional electrical stimulation of patients’ muscles while they are repeatedly performing a task in response to the known effects of stimulation in previous repetitions. As the motor nerves and muscles of the arm reaquire the ability to convert an intention to move into a motion of accurate trajectory, force and rapidity, initially intense external stimulation can now be scaled back progressively until the fullest possible independence of movement is achieved.

At the same time, as a guest of the Southern California Chapter of the National Spinal Cord Injury Association, I was part of a small booth (among the more than two hundred exhibitors) in which we had spinal cord injured people up and walking with a lower extremity bracing system (the reciprocating gait orthosis) used at the PEERS Spinal Injury Program in Los Angeles. I had a young man, a C6/7 level quadriplegic, walking with electrical muscle stimulation and lower extremity bracing. The system is reviewed in Chapter 8 of this book. As these "disabled" persons walked erect and upright among their wheel chair bound colleagues and took long, confident strides past exhibits extol ling the latest technological virtues of yet another "new" wheelchair (Fig. 1), I reflected on the paradox of it all. What a majority of these paralyzed people W0re really looking for was an alteration oftheir disability so that they could more normally function (in an unaltered environment). What the great majority of the exhibitors were offering was an alteration of the environment so that they could more normally function (with an unaltered disability).

Functional Electrical Stimulation (FES) is a technology that restores voluntary movement via application of electrical current to nerves and muscles. Our work aims to restore movement in the paralyzed upper limb. When implementing FES systems, effective controllers are needed to translate the current and desired arm positions into a pattern of muscle stimulations that achieve the target position accurately and efficiently. Although a range of upper-extremity neuroprosthesis controllers exist, none is capable of restoring accurate, natural arm movement in a clinical setting. For the purpose of advancing upper-extremity FES control technology, we explore reinforcement learning (RL), a control strategy that uses delayed reward and a trial-and-error search to develop its action policy. A potential advantage of RL control for upper-extremity FES systems is that human user preferences can be incorporated into controller training through the use of user-generated rewards of the controller actions. To date, RL control has been minimally explored for FES systems, and human rewards have never been incorporated for this application. An RL controller was implemented for a planar 2 degree of freedom biomechanical arm model, and this project explored the feasibility of using human rewards to train the RL controller. Simulation experiments were performed using pseudo-human, computer generated rewards that simulate the rewards that a human would be likely to assign. A range of experiments was performed to examine the learning properties of RL control using human-like rewards, and it was determined that RL controller learning occurs over a measurable time frame. Subsequently, human rewards were introduced to train the RL controller. Ten human subjects viewed animations of arm reaching movements, and assigned rewards to train the RL controller based on the quality of each movement. The RL controllers trained by humans learned well, although pseudo-human reward training was found to be equally effective. We discuss the potential benefits of using pseudo-human rewards for initial RL controller training, with subsequent fine-tuning training using human rewards. Reinforcement learning is a promising control strategy to restore natural arm movement to individuals with high-level paralysis.

Visual feedback training (VFT) and functional electrical stimulation (FES) have shown promise in iSCI standing balance rehabilitation. Combining VFT and lower-limb FES was hypothesized to yield more effective standing balance therapy. This thesis developed a VFT-complementary FES system for regulating ankle muscles. In my previous work, I have developed a VFT system with another student. For my thesis project, I (1) designed an FES controller, (2) optimized the controller to mimic able-bodied ankle muscle activation data during VFT sessions, and (3) tested the resultant FES controller on able-bodied individuals as they underwent VFT to determine the effects of the FES controller on able-bodied postural control. As expected, the resultant FES-VFT system did not deteriorate able-bodied postural control. The results of this research contribute toward the development of a more effective standing balance rehabilitation system for individuals with iSCI.
