Book excerpt: Degeneration of the intervertebral disc (IVD) is a significant cause of pain and immobility for many people. Tissue engineering with mesenchymal stem cells (MSC) offers promise for repair or regeneration of disc tissue if they can be encouraged to differentiate to a nucleus pulposus phenotype. The cells were exposed to conditions approximating the in vivo extracellular environment of the IVD; low glucose (2.5 mg/mL), 5% hypoxia, and medium conditioned by the cells of the nucleus pulposus (notochordal and chondrocyte-like). A novel hydrostatic pressure bioreactor was designed and validated to simulate physiologic-range pressures (1MPa at 0.5 Hz and static) to in vitro cultures of MSC. These conditions were compared to the "standard" cell culture conditions of 4.5 mg/mL glucose, 20% oxygen, no hydrostatic pressure, and a "neutral" conditioned medium. The bioreactor was shown to not adversely affect cell culture and MSC were sensitive to all factors tested.

Book excerpt: Perfused bioreactors have been developed with different designs and with various cell lines such as fibroblasts, osteoblasts and C2C12 myoblasts. All these current systems are custom made and mostly capable for only one cell type. Small seeding spaces and chambers inhibit longer cell studies and the observation of cell interaction. This study presents an advanced perfusion bioreactor which allows cell observation for longer than 5 days on an area of 2.8cm2 with a volume of 17ml. Initial experiments investigated homogeneity of the heat distribution, which is precisely controlled and stable over the length of the experiment. The addition of perfusion to the system results in the manual feeding process with modified F12 nutrient media being unnecessary. The perfusion and perfusion rate are user controllable up to 8ml/h. Different inlets allow cell seeding, cell feeding and chemical stimulation. Since the metabolism by-products are diluted and removed by the flow of the perfused system, no inhibited growth occurs and the pH-value will maintain constant at 7.4 which removes the need for balancing the CO2 environment. Fully enclosed and sealed assemblies with a controlled hot plate can be used outside of the incubator and incorporated into the stage of a microscope to track and monitor cell growth. The bioreactor chamber consists of three parts of transparent annealed cast Acrylic plus sealing material. Acrylic is chosen since it is machinable by laser cutting, which is a fast and easy method of manufacture. Due to the annealing process sterilizing by ethanol is possible. Heat distribution analysis was made with an IR-camera. And the pH was tested by indicator paper. The flow rate was set at 3.3ml/h. Computer simulations for flow and heat distribution and standard tests with cell cultures showed that a round bioreactor chamber design has advantages due to more uniform conditions. To track the cells during their distribution and over their whole life cycle, a completely transparent system is being developed. It includes an Indium Tin Oxide (ITO) coated glass plate, which can be used as a hot plate with precise controlled heating properties to heat the whole chamber.

Book excerpt: Mesenchymal stem cells (MSC) are becoming increasingly involved in the regenerative medicine field, particularly to treat diseases that are not effectively curable with the current therapies. Two scientific barriers are nevertheless responsible for MSC use and commercialization limitations. On one side, large amounts of cells are needed to reach the high cell dose requirements. On the other side, cells being the final product themselves, directly injected into the patient, their quality have to be controlled (stem cell phenotype, differentiation capability). MSC cultivation on microcarriers in a stirred bioreactor seems to meet these challenges. However, a precise knowledge about the impact of the technologies and the hydrodynamics generated, on the physiological cell response, is necessary to improve the scale-up of MSC cultures in bioreactors. In this context, present work is dedicated to the study of the impact of the agitation mode (orbital or mechanical) on the cell attachment, expansion and detachment on various microcarrier types, in the case of MSC derived from the Wharton's jelly (WJ-MSC) of umbilical cords. To quantify more precisely cell distribution and expansion on microcarriers, an automatic and in situ counting method was developed, which need no detachment step. This allowed the identification of commercial microcarriers suitable for WJ-MSC cultures, which were then compared to home-made microcarriers, synthesized by a partner laboratory, in terms of cell attachment and expansion, and detachment efficiency. In parallel to these works, the impact of the impeller design on the microcarrier suspension in stirred tank bioreactors was investigated. Based on a dimensional analysis and CFD simulations, it resulted in the establishment of two models relating the minimal agitation rate to ensure all particle suspension (Njs) with the impeller geometrical characteristics (design, size, off-bottom clearance) and the material properties of both the solid and the liquid phases. CFD models validation allowed then to develop a strategy to optimize the geometrical configuration of an impeller, dedicated to MSC cultures on microcarriers in a minibioreactor. Parameters characterizing the hydromechanical stress encountered by the solid phase were wisely chosen and integrated into CFD simulations. Based on a design of experiments, and the hydrodynamics data recovered from simulations, response surfaces were built and a multiobjective optimization was achieved in order to determine the geometry minimizing the particle stress, and also by adhered cells. WJ-MSC cultures in minibioreactors equipped with impellers displaying various geometries were finally validated, with a preliminary comparison of the impact of these geometries on the cell expansion.

Book excerpt: This reference work presents the origins of cells for tissue engineering and regeneration, including primary cells, tissue-specific stem cells, pluripotent stem cells and trans-differentiated or reprogrammed cells. There is particular emphasis on current understanding of tissue regeneration based on embryology and evolution studies, including mechanisms of amphibian regeneration. The book covers the use of autologous versus allogeneic cell sources, as well as various procedures used for cell isolation and cell pre-conditioning , such as cell sorting, biochemical and biophysical pre-conditioning, transfection and aggregation. It also presents cell modulation using growth factors, molecular factors, epigenetic approaches, changes in biophysical environment, cellular co-culture and other elements of the cellular microenvironment. The pathways of cell delivery are discussed with respect to specific clinical situations, including delivery of ex vivo manipulated cells via local and systemic routes, as well as activation and migration of endogenous reservoirs of reparative cells. The volume concludes with an in-depth discussion of the tracking of cells in vivo and their various regenerative activities inside the body, including differentiation, new tissue formation and actions on other cells by direct cell-to-cell communication and by secretion of biomolecules.
