From the Enlightenment onwards, it was widely believed that the condition of humankind could be radically improved. History had embarked on an unstoppable forward trajectory, realizing the promise of freedom and reason. The scientific revolution, the industrial revolution, and the French Revolution, in some views also the socialist revolution, were milestones on this march of progress. But since the late twentieth century the idea of progress has largely disappeared from public debate. Sometimes it has been explicitly declared dead. The wide horizon of future possibilities has closed. The best we can hope for, some say, is to avoid regress. What happened to progress? Why did we stop believing in it, if indeed we did? This book offers answers to these questions. It reviews both the conceptual history of progress and the social and political experiences with progress over the past two centuries, and it comes to a surprising conclusion: The idea of progress was misconceived from its beginnings, and the failure of progress in practice was a result of this flawed conception. The experiences of the past half century, in turn, has allowed us to rethink progress in a more adequate way. Rather than the end of progress, they may herald the beginning of a new, reconstructed idea of progress.

Book excerpt: Avec une baisse des effectifs des médecins généralistes, l'offre de soins de premier recours en France devient problématique. Les inégalités de répartition des praticiens se creusent. Les jeunes médecins généralistes rejettent le modèle d'exercice de leurs ainés. L'âge de la première installation recule. Face aux déserts médicaux, les gouvernements tentent d'apporter des solutions avec les mesures incitatives à l'installation mais celles-ci sont critiquées. Afin d'identifier les facteurs déterminants le lieu d'installation des médecins généralistes et de recueillir leur opinion sur les mesures incitatives, une étude descriptive, observationnelle et transversale a été réalisée entre le 7 janvier et le 9 mars 2014. Un questionnaire électronique commun à deux autres travaux relatifs aux problèmes de démographie médicale a été diffusé aux médecins généralistes lillois ayant passé l'Examen Classant National entre 2004 et 2008, tous modes d'exercice confondus. 310 réponses ont été collectées. Il apparait que la famille était au coeur de la décision du lieu d'installation (compatibilité avec le travail du conjoint, établissements scolaires, services de proximité). Les facteurs professionnels prépondérants étaient la proximité d'un établissement de soin et une opportunité professionnelle (reprise de cabinet, association, poste dans une maison de santé pluridisciplinaire (MSP)). La connaissance de l'offre de soin du secteur était également importante. La plupart des médecins avait entendu parler des mesures incitatives. Les aides logistiques et fiscales étaient les plus citées comme incitant à l'installation. Les principaux freins à leurs recours étaient la priorité laissée à la vie familiale et le sentiment de perte de liberté. Les MSP pourraient augmenter le nombre d'installations de même qu'une meilleure formation théorique à l'installation et davantage de stages ambulatoires. Un tiers des médecins pensaient que les mesures incitatives n'avaient pas d'influence sur les installations. Ainsi, les facteurs privés, environnementaux et professionnels, semblent plus déterminants pour le choix du lieu l'installation que les mesures incitatives qui restent controversées.

Book excerpt: Contexte : La médecine générale est en pleine mutation : multiplication des modes d'exercice, activité libérale délaissée au profit de l'activité salariée, installation de plus en plus tardive et majoritairement en groupe. L'objectif de notre travail était de mettre en évidence les facteurs déterminant le mode d'exercice des jeunes diplômés de médecine générale. Méthode : L'enquête a été réalisée par un questionnaire en ligne chez 677 anciens internes de médecine générale des facultés de médecine de Lille, ayant passé l'ECN entre 2004 et 2008. Le questionnaire était commun à deux autres travaux relatifs aux problèmes de démographie médicale. Le lien du questionnaire a été initialement transmis par courriel, puis par contact téléphonique ou par courrier. Résultats : Le taux de participation était de 45,8%. Notre population comportait 310 médecins dont 56% de femmes. La médecine générale était le premier choix à l'amphithéâtre de garnison pour 78,4% de la population, décision prise durant l'externat le plus souvent. 51% des médecins avaient un exercice libéral, 37,1% un exercice salarié et 11,9% un exercice mixte. 48% des femmes exerçaient en libéral. Peu de médecins (19,3%) étaient installés mais 63% des médecins souhaitant changer de mode d'exercice projetaient de s'installer. La disponibilité pour la vie privée était un facteur déterminant du choix professionnel pour 74% de l'ensemble de notre population. L'importance de ce facteur était similaire pour les deux sexes. Pour 40% des médecins, la volonté d'éviter les charges administratives avait été déterminante de leur choix professionnel. Le stage chez le praticien avait été déterminant pour 72% des médecins ayant choisi l'exercice libéral. Le mode d'installation préférentiel était en groupe, que ce soit en association ou en maison de santé pluridisciplinaire. Conclusion : L'exercice libéral n'est pas délaissé par les jeunes médecins. La qualité de vie et la formation pratique sont au coeur du choix du mode d'exercice.
