Book excerpt: Les indications thérapeutiques des fractures diaphysaires de l'avant-bras chez l'enfant sont fonction de l'âge du patient et de l'angulation de la fracture. Le traitement orthopédique reste la prise en charge de référence mais en cas de réduction insuffisante ou d'instabilité, une stabilisation chirurgicale par enclouage centromédullaire élastique stable (ECMES) s'impose avec parfois la nécessité d'aborder le foyer de fracture en cas d'irréductibilité. Une des difficultés est de ne pas retarder cette décision d'aborder en cas d'échec d'alignement des fragments. L'objectif était de mettre en évidence des facteurs anatomiques pouvant aider à cette prise de décision. De façon rétrospective, les cas de 148 patients de moins de 16 ans pris en charge au bloc opératoire de chirurgie pédiatrique au CHU de Bordeaux sur une période de trois ans ont été analysés, composant une série comparable à celles rapportées dans la littérature. Parmi eux, 61 cas ont requis un traitement par ECMES dont 22 avec abord chirurgical du foyer de fracture. Or, un lien statistique a pu êttre démontré entre le caractère transverse du trait dans une fracture chevauchée du radius et le recours à un ECMES avec abord du foyer de fracture, légitimant, dans ces circonstances précises, la décision précoce d'abord chirurgical et les avantages qui en découlent, notamment en terme de durée opératoire et d'exposition aux rayonnements.

Book excerpt: Introduction : les fractures diaphysaires des os de l’avant-bras sont fréquentes chez l’adolescent et l’adulte. Leur prise en charge ne fait pas consensus entre les équipes chirurgicales pédiatriques et adultes. Notre hypothèse était que les fractures diaphysaires de deux os de l’avant-bras étaient pourvoyeuses de complications. Matériel et méthodes : il s’agit d’une étude rétrospective monocentrique. Nous avons inclus les fractures diaphysaires d’un ou deux os de l’avant-bras prises en charge au bloc opératoire. Nous avons comparé deux groupes : groupe A âgé entre 12 à 16 ans et groupe B âgé entre de 16 à 20 ans. Nous avons analysé le mécanisme lésionnel, le type de fracture et de traitement, les complications, les résultats radiologiques et cliniques selon le score qDASH. Résultats : 71 patients étaient inclus, 47 dans le groupe A et 24 dans le groupe B. Cinq patients du groupe A présentait un traumatisme à haute énergie et 14 patients dans le groupe B. Dans le groupe A, 45 patients ont été opéré par embrochage centromédullaire élastique stable (ECMES). Dans le groupe B, 19 patients ont été ostéosynthèsés par plaque, 1 par plaque et Ecmes, 3 par ECMES et 1 par plâtre brachio anté brachio palmaire. Concernant le risque de complications, on retrouvait une augmentation statistiquement significative s’il s’agissait d’une ostéosynthèse plaque (p = 0.046), d’un traumatisme à haute énergie (p=0,002) et d’une fracture ouverte (p=0,04). Le score moyen qDASH était meilleur dans le groupe A (p = 0.148), mais non significatif. Conclusion : ce travail a permis de confirmer notre hypothèse de recherche et que les résultats à long terme sont bons, ce qui est en accord avec la littérature. Nous avons pu montrer que l’ostéosynthèse par plaque chez l’adulte jeune ne doit pas être systématique, car pourvoyeuse de plus de complications. Un traitement par ECMES peut être proposé chez l’adulte jeune même lorsque le cartilage de croissance est fusionné.
