The 2010 American College Health Association National College Health Assessment found that 28 percent of college students reported that at some point in the last 12 months they had felt so depressed it was hard to function. And 46 percent reported that, in the same period, they had experienced overwhelming anxiety. In addition, 8 percent of students also reported being diagnosed or treated for depression and almost 6 percent seriously considered suicide. This paper presents several research on depression and alcohol abuse among young adults in college. It also describes several alcohol and other drug services and mental health and counseling services at colleges and universities.

Book excerpt: High frequencies of alcohol consumption, smoking tobacco and marijuana use are perceived as normal behaviors among college students and widely encouraged within social settings. The medical consequences of substance use among college students have been thoroughly highlighted throughout the literature. However, more information is needed to determine the impact of alcohol use and smoking on mental health status. It is likely that substance use can have problematic implications for depression and anxiety among college students. The literature suggests that alcohol and drug use elevates problems in mood among individuals who are already at risk for depression. This study investigates the association between substance use and anxiety and depression. An independent-samples t-tests were performed to determine whether or not there were significant differences in average alcohol and marijuana use between students with and without a mental health diagnosis. A chi-square analysis was also conducted to determine if there was a relationship between tobacco use among individuals who were diagnosed with depression. The chi-square results were statistically significant but had a small to moderate effect size. These findings support recommendations for substance use education and prevention. Future research should assess the impact of substance use and mental health on academic performance.

These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. Undergraduate college students face additional risks for alcohol or drug use due to the significant transitions occurring in their personal, social, and environmental lives. The consequences of alcohol or drug use to the college student population and relevant stakeholders are well documented. To prevent and mitigate the negative consequences of college students’ alcohol or drug use, it is essential to understand factors that predict those behaviors. More specifically, it imperative to understand factors that predict alcohol or drug use that results in negative consequences, or problematic use. Psychological distress and impulsivity have been identified as predictors of other high-risk behaviors among college students, but their predictability of problematic alcohol or drug use among the undergraduate college student is not yet fully understood. Gaps in the literature regarding the psychological distress, impulsivity, and substance-using behaviors among this population include a lack of focus on drugs other than alcohol, a lack of differentiation between problematic and non-problematic use, and significant variability in the definitions and operationalization of alcohol and drug use related variables. The primary purpose of the present study with 481 participants was to investigate whether psychological distress (i.e., depression, anxiety, and stress) and impulsivity among undergraduate college students between the ages of 18 and 25 could predict problematic alcohol or drug use. A secondary aim of this study was to determine whether there was a relationship between problematic alcohol use and problematic drug use. Findings indicated that impulsivity was predictive of problematic alcohol use, and depression and impulsivity were predictive of problematic drug use. Neither anxiety nor stress were predictive of problematic alcohol or drug use among this sample. Findings also indicated a statistically significant small to moderate association between problematic alcohol use and problematic drug use.

One recent survey of postsecondary educators found that nearly 80 percent believed emotional wellbeing is a "very" or "extremely" important factor in student success. Studies have found the dropout rates for students with a diagnosed mental health problem range from 43 percent to as high as 86 percent. While dealing with stress is a normal part of life, for some students, stress can adversely affect their physical, emotional, and psychological health, particularly given that adolescence and early adulthood are when most mental illnesses are first manifested. In addition to students who may develop mental health challenges during their time in postsecondary education, many students arrive on campus with a mental health problem or having experienced significant trauma in their lives, which can also negatively affect physical, emotional, and psychological wellbeing. Book excerpt: Social anxiety disorder (SAD) and alcohol use disorders (AUDs; i.e., alcohol dependence, alcohol abuse) are highly prevalent and potentially debilitating. They also commonly co-occur, and when they do, their combined effect may be even more devastating. The onset of SAD most commonly precedes the onset of AUDs, suggesting that SAD may be a marker or risk factor for the onset of these other disorders. Previous research has not sufficiently examined the mechanisms involved in the development of AUDs, and longitudinal research is lacking. The current study examined mechanisms related to the development of AUDs among incoming college freshman students at two large universities in the United States. Incoming freshmen are at higher risk for developing symptoms consistent with SAD, particularly during their first semester, and they may be more likely to cope with their symptoms of anxiety by drinking alcohol. The current study aimed to explicate the relationship between social anxiety and alcohol consumption in college freshmen. Baseline data collection occurred late in the summer after registration for the Fall semester or early in the Fall semester. Follow-up data collection occurred later in the Fall semester. It was expected that social anxiety, the quantity and frequency of drinking alcohol (including frequency of intoxication), and alcohol-related problems would increase among the freshmen over the course of the fall semester. Additionally, several relationships among the variables being examined were hypothesized. Drinking motives, symptoms of depression, and quality of life were expected to mediate the relationship between social anxiety and the drinking outcome variables. In addition, expectancies about alcohol consumption were expected to moderate the mediated relationship. However, there were no increases in social anxiety, alcohol consumption, or alcohol-related problems between baseline and follow-up. There were few hypothesized relationships found, although there was a positive relationship between social anxiety and negative alcohol expectancies and a negative relationship between social anxiety and quality of life. Model testing generated one promising model in which the relationship of positive expectancies regarding alcohol use to alcohol use and problems was mediated by coping with anxiety drinking motives. In particular, the main effect of positive expectancies of alcohol and coping with anxiety drinking motives generated a medium effect whereas the other relationships generated small to medium effects. Clinical implications and limitations of the current study are discussed.

Book excerpt: The Brief Alcohol Screening and Intervention for College Students (BASICS: Dimeff et al., 1999) is a program used in higher education institutions to reduce drinking outcomes. Level of dependence on alcohol is a global and multidisciplinary concern. The connections between mental health (anxiety and depression) have been well established in the literature. Other connections such as family history and personality dimensions are being further explored. This study reviewed the prevalence and consequences of alcohol use as well as the connections to the specific population of college students. Theoretical frameworks from Chickering and Baxter Magolda are discussed. Quantitative analysis including regression was used to examine the connections between level of dependence on alcohol with the mental health, familial history, and personality dimensions of college students. Results indicated that participants were predominately, male, white, 18-19 years-old, freshman or sophomore, domestic students, and resided in campus residence halls. Research question one results showed that gender is a moderator and that participants anxiety, other drug use, and number of consequences significantly predicted their level of dependence. In research question two, gender was again a moderator and self-efficacy was statistically significant and negative. Research question three showed that gender is a moderator and that anxiety, impulsivity, hopelessness, and self-efficacy were statistically significant. For this third model self-efficacy and hopelessness had negative beta scores.

Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.
