Ayant connu une forte augmentation entre les années 1980 et 2000, de multiples moyens ont été mis en oeuvre par les pouvoirs publics afin de sensibiliser la population et les professionnels de santé à ce problème. La généralisation du dépistage précoce du surpoids de l'enfant par les médecins est un des objectifs de la prévention nationale. Or plusieurs études révèlent que peu de médecins suivent l'IMC et la courbe de corpulence des jeunes patients. Par le biais d'entretiens semidirigés auprès des médecins généralistes haut-normands, nous avons mené une étude qualitative afin de comprendre quels étaient les obstacles à la réalisation de ce dépistage. Ce travail a été accompli afin d'optimiser le dépistage précoce menant à une prise en charge nutritionnelle des enfants plus jeunes, évitant ainsi pour un plus grand nombre d'entre eux le développement d'une obésité. Alors que certains médecins s'organisent pour aborder ce problème avec le plus d'efficacité possible, d'autres sont découragés. Au carrefour du culturel, de l'affectif et du social, les représentations en matière d'alimentation, de corpulence et de santé sont extrêmement variables d'un individu à l'autre, tant dans la population générale qu'au sein des professions de santé. Ces différences de conception que peuvent avoir les soignants et les patients sur le sujet, rend le dépistage précoce de l'obésité de l'enfant difficile à réaliser en pratique. Une meilleure coordination des structures de soins et des différents intervenants autour du médecin généraliste pourrait favoriser le dépistage. De la même manière, cette étude nous a permis de comprendre que le dépistage précoce serait avantagé par le développement de l'informatisation des cabinets médicaux, la généralisation de la formation continue des médecins généralistes avec évaluations des pratiques et le développement de consultation dédiée au problème. Il apparait que la place du médecin généraliste en matière de dépistage précoce de l'obésité de l'enfant est primordiale. Cependant de manière plus générale, il émerge de notre étude que la médecine générale doit se tourner vers une pratique plus préventive, nécessitant pour cela un profond remaniement des mentalités de tout un chacun.

Book excerpt: Contexte : le surpoids et l'obésité de l'enfant constituent un problème majeur de santé publique. En France, en 2015, 17 % des enfants de 6 à 17 ans étaient en surpoids dont 4% obèses. Le médecin généraliste a un rôle clé dans le dépistage et le suivi du surpoids et de l'obésité de l'enfant. En pratique les médecins généralistes rapportent des difficultés notamment un manque de temps, de formation et d'outils. Au cours des dernières décennies, avec l'émergence du numérique, des outils d'aide à la décision clinique ont vu le jour. L'objectif de ce travail était d'élaborer un outil informatique d'aide au dépistage et au suivi du surpoids et de l'obésité de l'enfant en soins premiers puis d'évaluer son acceptabilité auprès des médecins généralistes. Méthode : l'élaboration de l'outil Obéclic a été réalisée à partir d'un cahier des charges et d'une maquette évolutive avec un groupe pilote d'experts. Le contenu de l'outil est issu des recommandations de la Haute Autorité de Santé et des sociétés savantes. Une étude Pilote d'acceptabilité, quantitative et transversale, par auto-questionnaire de type Lime Survey®, a été réalisée auprès d'un échantillon de médecins généralistes des Hauts de France, tirés au sort. Une analyse descriptive des résultats a été réalisée. Résultats : le site Obeclic est gratuit, exhaustif, fiable et accessible sur www.obeclic.fr. Il est constitué de 9 onglets reprenant les différentes étapes du dépistage et du suivi du surpoids et de l'obésité de l'enfant. Le site contient des rappels théoriques et des formulaires (algorithmes interactifs décisionnels) ainsi que des fiches conseils imprimables. Sur les 138 médecins ayant accepté de participer à cette étude, 44 ont répondu de façon complète au questionnaire. 86 % des médecins considèrent l'organisation du site Obéclic adaptée à la pratique de la médecine générale. 77 % considèrent Obéclic utilisable en consultation. 71 % estiment qu'Obéclic est une aide au dépistage et 86 % estiment qu'Obéclic est une aide au suivi du surpoids et de l'obésité de l'enfant. Enfin, 88 % des médecins interrogés pensent utiliser Obéclic à l'avenir. Conclusion : Obéclic est un outil d'aide au dépistage, au suivi du surpoids et de l'obésité de l'enfant acceptable en soins premiers.

En France en 2007, 15,8% des enfants de 3 à 17 ans sont en surpoids dont 2,8% obèses. Plusieurs programmes de prévention se sont développés. En 2003, des réseaux de prise en charge de l'obésité ont été créés. 20 à 40 % des enfants obèses le restent à l'âge adulte. De nombreux facteurs de risques modifiables favorisent le développement de l'obésité, ceci incite à développer la prévention et le diagnostic précoce. Le médecin généraliste, acteur central dans le domaine de la prévention, doit agir avant la prise de poids excessive. Une étude de pratique auprès des médecins généralistes de la Nièvre fut réalisée pour comparer aux recommandations de la HAS de 2011 leurs connaissances et pratiques dans le dépistage du surpoids et de l'obésité de l'enfant. De plus la collaboration des médecins avec le réseau nivernais RESEDIA, prenant en charge les enfants en surpoids ou obèses, fut étudiée. Deux outils simples ont été identifiés : le calcul de l'IMC et l'analyse de la courbe de corpulence. Leur utilisation systématique deux fois par an dans le suivi de la croissance de l'enfant permet de dépister tôt un problème de corpulence. Les médecins ont en général conscience du problème de santé publique mais leur pratique peut largement être améliorée. Les médecins formés ont une pratique plus conforme aux recommandations d'o la nécessité d'une formation médicale continue. Les réseaux sont très intéressant pour la prise en charge adaptée et pluridisciplinaire mais une collaboration avec le médecin traitant est nécessaire pour une prise en charge à long terme.
