Book excerpt: Introduction : le cancer du sein est la néoplasie la plus fréquente chez la femme, en France et dans le monde. Le dépistage est la clé de voute des actions permettant une réduction de la mortalité de cette pathologie. Le médecin généraliste, souvent en première ligne, a un rôle capital dans le dépistage et la prévention. Devant une diminution du taux de participation au dépistage organisé du cancer du sein depuis deux années consécutives nous nous sommes intéressés aux pratiques des médecins généralistes des Hauts de France concernant le dépistage organisé et individuel du cancer du sein selon le niveau de risque des patientes. Matériel et méthodes : étude quantitative, descriptive, observationnelle réalisée à partir d'un questionnaire envoyé, avec l'aide de l'URPS à un échantillon de 508 médecins généralistes des Hauts-de-France. Résultats : cent quinze questionnaires exploitables ont été recueillis. Deux-tiers des sondés réalisaient la palpation mammaire. Tous les sondés vérifiaient qu'une mammographie avait été réalisée dans les deux dernières années et la plupart d'entre eux exposaient les bénéfices/risques à leurs patientes. Près de la moitié de l'échantillon pense qu'un dépistage avant 50 ans serait justifiable. Cependant le dépistage individuel engendrait des difficultés pour 64% d'entre eux et ils étaient 79% à ne pas connaitre les recommandations HAS de 2014. Discussion : le dépistage organisé semble maitrisé. Même si les controverses le concernant poussent à une meilleure information, elles ne remettent pas en cause l'efficacité de la mammographie pour notre échantillon. En revanche le dépistage individuel présentait des lacunes qui pourraient être améliorées. Conclusion : une évaluation pertinente des risques individuels permettrait aux généralistes d'adapter les informations et les prescriptions à chaque patiente. Pour cela il est indispensable que les connaissances des recommandations soient à jour et qu'ils aient les bons outils pour réaliser un dépistage ciblé et adapté à chaque patiente. La mise en place d'une consultation dédiée à 25 ans est une piste à suivre.

Book excerpt: Introduction : En France. 1e cancer au sein reste le cancer féminin le plus fréquent. Un examen clinique des seins (ECS) annuel pour les femmes dès l'âge de 25 ans est ecommandé. Pour être bénéfique et performant, il nécessite une technique rigoureuse. Identifier le risque de cancer de chaque patiente permet d'adapter le type de dépistage et le suivi. Les objectifs de notre travail étaient d'analyser la pratique et les modalités de l'ECS par les médecins généralistes, évaluer leurs connaissances sur les différentes modalités de dépistage et mettre en évidence d'éventuels freins à cette pratique ainsi qu'un besoin de formation. Méthode : Étude quantitative descriptive, réalisée à partir d'un questionnaire envoyé à des médecins généralistes exerçant en Bourgogne-Franche -Comté entre le 15/02/2019 et le 18/03/2019. Résultats : 221 médecins ont répondu au questionnaire, 60% de femmes et 40% d'hommes. 90,5% déclaraient pratiquer l'ECS. Les moments opportuns les plus retrouvés étaient la réalisation d'un frottis cervico-vaginal (69%), un renouvellement de contraception (67,5%) et à la demande de la patiente (64,5%). Les facteurs principaux motivant la pratique de l'ECS étaient le rôle du médecin généraliste en tant qu'acteur de prévention (90,5%), l'amélioration du diagnostic précoce du cancer du sein (64,5%),et le fait qu'il s'agisse d'une modalité de dépistage simple et peu coûteuse (58,5%). 68,5% ont répondu examiner les seins une fois par an. Pour 25% de ces médecins l'âge de leurs patientes à leur premier examen était de 25 ans, et 68,5% disaient ne pas se fixer d'âge limite pour arrêter. La technique de palpation par quadrants était la plus utilisée(62%). 52,4% des médecins ne pratiquant pas l'ECS, le justifiaient par le fait qu'il s'agit plutôt du rôle du gynécologue ; 28,6% ne trouvaient pas d'intérêt à l'ECS versus mammographie et avaient egalement peur d'une plainte de la patiente. Près de 60% des médecins déclaraient ne pas connaître les modalités de dépistage selon les niveaux de risque mais 67,5% exprimaient un désir de formation. 86% enseignaient l'autopalpation à leurs patientes. Il existait une différence significative de pratique selon le genre et le fait d'avoir un Diplôme InterUniversitaire en gynécologie-obstétrique. Conclusion : L'ECS est largement pratiqué, par davantage de généralistes femmes, mais selon des modalités hétérogènes et une technique standardisée peu appliquée. Un suivi personnalisé des femmes et impliquant davantage le médecin généraliste, nécessite une remise à niveau de ses connaissances ainsi qu'une formation plus efficiente et adaptée à ses besoins.

L’objectif de cette thèse est d’estimer, en population générale, le taux d’absence de participation aux mammographies et la prévalence de femmes non à jour de cet examen et aussi de connaître les caractéristiques associées au fait d’être en retard ou de ne jamais avoir réalisé de dépistage du cancer du sein. Il est décrit les particularités des femmes pratiquant le DO (dépistage organisé) ou le DI (dépistage individuel). La prévalence de l’absence de participation au dépistage est de 6,5% et celle des femmes non à jour est de 20,2%, ce qui est compatible avec les données de la littérature en population générale. Ces femmes ont davantage un faible niveau socio-économique, une faible qualité de protection sociale, n’ont pas de suivi gynécologique, n’ont jamais rencontré de cas de cancer du sein dans la famille ou l’entourage et se sentent en mauvais état de santé. Elles sont plus souvent isolées socialement. Les généralistes ont un rôle central puisqu’ils sont les mieux placés pour délivrer une information éclairée aux patientes et les solliciter à adhérer au dépistage et cela régulièrement. La majorité des patientes réalisent un DO (54,7%) et celles qui pratiquent plutôt le DI sont cadres, ont un plus haut niveau d’étude et de revenu. Même si les disparités socio-économiques semblent s’amenuiser depuis la généralisation du DO en 2004, des inégalités demeurent et les femmes précaires restent les plus vulnérables face aux risques de découvrir chez elles un cancer du sein à un stade avancé par défaut de prévention.

While MRTs, if effective, could satisfy a desire of women seeking to have a genetically related child without the risk of passing on mtDNA disease, the technique raises significant ethical and social issues. It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. This study examines the ethical and social issues related to MRTs, outlines principles that would provide a framework and foundation for oversight of MRTs, and develops recommendations to inform the Food and Drug Administration's consideration of investigational new drug applications.
