Twenty-four patients, who received dental implants at the University of Toronto, were 60+ yrs and had osteoporosis at the time of implant placement, and their controls matched for age, sex and implant related features were examined clinically and radiographically. Clinical and demographic variables recorded at implant placement and follow-up examination, were analyzed. Implant survival rates of 95.1% and 100%, and success rates of 91.4% and 100% were noted in the osteoporosis and control samples respectively. All failures in the osteoporosis sample occurred in the maxilla of a single subject, raising suspicion that these were related to individual problems specific to this subject. Due to the paucity of adverse outcomes and with all the implant failures having occurred in one subject, no relationship of adverse outcomes with clinical and demographic variables could be analyzed.

Osteoporosis also results in loss of bone mineral density throughout the body, including the maxilla and mandible. Successful osseointegration of dental implants is attributed to their ability to integrate well with bone. The influence of bone quality on dental implant osseointegration has been discussed in several studies, and higher rates of dental implant failure have been reported in patients with low bone quality and an inadequate bone volume. Osteoporosis represents a risk factor for osseointegration, and this relationship may be derived from the association of the disease with a deficiency in bone formation. This condition would compromise the healing capacity and the apposition of bone at the implant interface. Currently, there is no clear consensus regarding dental implant treatment in osteoporotic individuals. Studies have revealed contradictory reports regarding the success and failure of dental implants in patients with osteoporosis. Antiresorptive agents have been widely used to treat osteoporosis. Dental implant placement in patients on bisphosphonate therapy may trigger osteonecrosis of the bone. Hence, it is important to analyze factors that have to be taken into consideration prior to implant therapy in patients with osteoporosis and those undergoing treatment. This chapter outlines dental implant osseointegration under osteoporotic conditions. The possible effect of bisphosphonate therapy on dental implant survival will also be discussed based on the current literature.

Healthy patients showed a very high success rate over a long-term follow-up, nevertheless implant failures could happen due to lack (early failure) or loss of osseointegration (late failure). The impact of systemic diseases on the outcome of implant therapy is still unclear, however it seems that the control of systemic disease may be more important than the disease itself.The aim of this prospective study is to evaluate and to compare the outcomes of dental implants in healthy patients and in patients affected by systemic diseases.Two groups of patients were selected in the study, those who had systemic disorders (MCP) and who were under treatment with several drugs and those who were healthy (HP) and who didnu2019t take any drug. Post-surgical complications were evaluated in the 2 groups. The implants were restored three months after placement in the mandible and 4 months after placement in the maxilla. Dental implant failure was considered as an implant that was removed or that had to be removed, a survival implant was an implant that was functional even though it didnu2019t meet the success criteria and a successful implant was an implant that met the success criteria. The outcome measures were: periapical radiographs that were taken at implant placement, at the restoration phase and each year after implant placement; probing depth and bleeding on probing were recorded at 4 sites for each implant at the restoration phase and each year after implant placement.Two groups of patients were enrolled in the study those who were medically compromised (20 patients) and who have received 73 Tissue Level implants (Prama, Sweden & Martina, Due Carrare, Italy) and those who were healthy and who have received 67 Tissue Level implants (Prama, Sweden & Martina, Due Carrare, Italy). The early post-operative complications were higher in the medically compromised group (5 Implants in 3 patients) than in the healthy group (3 implants in 2 patients) even though the difference was not significant. One single early failure and one late failure were registered in the MCP group, the two patients were considered ASA II patients. The percentage of implants showing less than 1.5mm of bone loss after 1 year was 100% in both groups. The mean PD and BoP was not different in the 2 groups after 1 year.The medically compromised patients didnu2019t show any increased risk for implant failure and implant survival when compared to healthy patients. Systemic diseases did not represent a contraindication to implant treatment, even though these patients require a careful follow up for the implant maintenance as well as for the medical condition.

Book excerpt: Background : Dental rehabilitation with conventional prostheses after microvascular jaw reconstruction could result often unsuccessful because of the altered oral anatomy and the lack of adequate dentition for fixation. Although the efficacy and long-term safety of osseointegrated implants have been documented in atrophic patients, their outcomes in the oral cancer population remain unclear.Aim: to evaluate the long-term clinical outcomes and the survival and success rates of implants placed in patients who underwent mandibular or maxillary reconstruction with a fibula free flap; to investigate the relationship between implant prognosis and therapeutic variables.Materials and Methods: Between 1998 and 2015, 21 patients underwent maxillary or mandibular resections for benign or malignant oral tumors and simultaneous microvascular reconstructions with fibula free flaps. 20.8 (8u201338) months after the reconstructive surgery 108 dental implants were placed. Eighteen patients received a fixed prosthesis while three received an implant-supported overdenture. Clinical data as the probing pocket depth, the presence of suppuration and bleeding on probing and the occurrence of hyperplastic tissue formation were collected from medical records and implant bone loss was measured on radiographs to determine survival and success rates. The possible influence of radiation therapy, the reconstructive technique and the use of soft tissue grafts as prognostic factors were determined. To statistically analyze the results Kruskal-Wallis test and Fisher test were used. For the survival analysis, Kaplan-Meier curves and Log-rank test were employed.Results: The mean follow-up after implant insertion was 90.2 (20u2013204) months. The implant survival and success rate was respectively 97.2% and 95.4% at the 12-month follow-up, 86.5% and 73.5% at 60 months, and 79.3% and 64.7% at 120 months. Implant failure was more common in patients that had implants placed after they underwent radiation therapy (p = 0.001) versus those who did not. Peri-implant probing depth measures ranged from 2 mm to 9 mm (mean 3.8u00b12 mm). Peri-implant bone loss ranged from 0.5 mm to 8.1 mm (mean 2.2u00b11 mm) at the 10-year follow-up. Peri-implant bone loss values were higher in case of fibular distraction (p=0,04). Peri-implant hyperplasic tissue was identified in 20.3% of the surviving implants at the 10-year follow-up and its presence impacted the rate of peri-implant bone loss (p=0,001). Connective tissue and skin grafts positively influenced both survival and success of implants and reduced the risk of peri-implant hyperplastic tissue formation.Conclusion:The overall implant survival rate was acceptable. However, a relevant number of implants was identified as unsuccessful at a long-term follow-up. The use of soft tissue grafts can be considered a reliable procedure to significantly improve the long-term outcomes.
