Book excerpt: Despite great progress in recent years, efficient delivery of gene therapy or chemotherapy drugs into their target sites with minimal side effects remains one of the biggest challenges for effective cancer therapy. Co-delivery of siRNA targeted for proteins responsible for drug resistance and chemical anticancer drugs represents a promising new approach to overcome drug resistance and to make cancer therapy more effective. However, efficient co-delivery systems that can deliver siRNA and anti-cancer drugs simultaneously into cancer cells have rarely been developed. This thesis is aimed at developing novel non-viral nanocarriers for efficient delivery of antisense ODN and siRNA and codelivery with chemical anticancer drugs for effective cancer therapy. We began by performing a systematic investigation on the efficacy of five generations of polypropyleneimine (PPI) dendrimers to provoke nanoparticle formation from antisense ODNs and then deliver the ODN nanoparticles into cancer cells (Chapter 2). We then developed a novel approach to efficiently package and deliver siRNAs into cancer cells with low generation non-toxic PPI dendrimers by using gold nanoparticles as a "labile catalytic" packaging agent (Chapter 3). Relying on the fundamental understanding gained from Chapters 2 and 3, we then continued the utilization of dendrimers and developed polyamidoamine (PAMAM) dendrimer-modified mesoporous silica nanoparticles (MSNs) as a stimuli-responsive controlled-release delivery system for a chemotherapy drug (Chapter 4). By using a non-gatekeeping approach, we demonstrated nearly zero release of doxorubicin in H2O and complete release once delivered into cancer cells. In Chapter 5, we further utilized MSNs as a codelivery system to simultaneously deliver Doxorubicin and a Bcl-2-targeted siRNA into A2780/AD human ovarian cancer cells for enhanced chemotherapy efficacy. We then investigated the effect of each component in the PAMAM-dendrimer modified MSN-based codelivery system on the cell uptake efficiency of siRNA and its intracellular release and localization (Chapter 6). We further studied the effect of temperature and different inhibitors on the cell uptake efficiency of MSN-Dox-G2 and found that MSN-Dox-G2 might internalize into cells through a non-endocytic process (Chapter 7). Finally, we demonstrated a specific cancer cell-targeted delivery by PEGylating the MSN-Dox-G2/siRNA complex and tagging it with a specific cancer-targeting group (Chapter 8).

However, they require a delivery system in vivo to prevent degradation and off target effects. Silica based nanoparticles, both solid and mesoporous, are a promising option due to their biocompatibility, ease of preparation and morphology control, reproducibility, and facile addition of functional groups including targeting ligands. After a brief introduction to cancer treatment and review of the current nanoparticle treatments undergoing clinical trials, this thesis details the many methods explored over the past ten years to fine-tune particle preparation, pore size, functionalization, and delivery strategies. The majority of both solid and mesoporous silica nanoparticles are synthesized using the sol-gel method and then various functionalization techniques are employed to load and protect the oligonucleotides. Externally loaded systems generally use a combination of polyethylenimine (PEI) and polyethylene glycol (PEG). Mesoporous silica nanoparticles internally load the DNA or RNAi, resulting in the added variable of pore size. Several groups have investigated how pore size alters loading and release kinetics to perfect this variable. Many groups have also tested ligands targeting for over expressed proteins on the intended cancer, triggered release techniques, cell-penetrating peptides in order to create a viable in vivo delivery system. By compiling the techniques employed by researchers over the past ten years, this thesis will elucidate which approaches are most promising for future research. Furthermore, overall strategies within the field are suggested to more easily compare studies and evaluate methods.

Although efforts have been made over the last 50 years or so to develop such delivery technology, no effective and above all clinically approved protocol for cell-specific drug delivery in humans exists as yet. Various particles, macromolecules, liposomes and most recently “nanomaterials” have been said to “show promise” but none of these promises have so far been “reduced” to human clinical practice. The focus of this volume is on cancer indication since the majority of published research relates to this application; within that, we focus on solid tumors (solid malignancies). Our aim is critically to evaluate whether nanomaterials, both non-targeted and targeted to specific cells, could be of therapeutic benefit in clinical practice. The emphasis of this volume will be on pharmacokinetics (PK) and pharmacodynamics (PD) in animal and human studies. Apart from the case of exquisitely specific antibody-based drugs, the development of target-specific drug–carrier delivery systems has not yet been broadly successful at the clinical level. It can be argued that drugs generated using the conventional means of drug development (i.e., relying on facile biodistribution and activity after (preferably) oral administration) are not suitable for a target-specific delivery and would not benefit from such delivery even when a seemingly perfect delivery system is available. Therefore, successful development of site-selective drug delivery systems will need to include not only the development of suitable carriers, but also the development of drug entities that meet the required PK/PD profile.
