Le coût humain et économique de l'épuisement professionnel des accompagnants est déjà considérable. Mais il apparaît de plus en plus clairement que, le sens des actions entreprises, la motivation du personnel et le plaisir éprouvé à accomplir sa tâche, constituent les conditions mêmes de l'efficacité du travail d'accompagnement dans les institutions et services et de la qualité de vie des personnes accompagnées. Dit autrement, la " bientraitance " des intervenants conditionne en grande partie celle des personnes concernées. De nombreux acteurs de terrain, exerçant dans le cadre de la déficience intellectuelle avec des références théoriques très diverses et sur trois continents différents, sont venus à l'UNESCO, au Vlllème Congrès de l'AIRHM, en Avril 2002, ,confronter leurs expériences sur le thème de la prévention de /'usure professionnelle. Que l'on insiste sur les " procédures" qui organisent l'action des accompagnants, ou que l'on s'attache à dégager le " sens " de cette action, comme le promeut la tradition de la psychothérapie institutionnelle, on perçoit bien la complémentarité de ces perspectives. Il se trouve qu'elles convergent de façon impressionnante et que tous les auteurs s'unissent pour délivrer aux Associations et aux Pouvoirs publics un message de grande actualité en cette " année européenne du handicap ". Le voici en deux phrases. L'éthique de la prise en charge des personnes handicapées en général, et de celles qui souffrent de déficience intellectuelle en particulier, requiert la reconnaissance de la valeur du travail de celles et ceux qui assurent leur accompagnement, ainsi que les soins nécessaires. La prévention raisonnée de leur épuisement professionnel et de leur turn over excessif est le gage de la qualité de vie des personnes handicapées.

Book excerpt: Myalgic encephalomyelitis (ME) and chronic fatigue syndrome (CFS) are serious, debilitating conditions that affect millions of people in the United States and around the world. ME/CFS can cause significant impairment and disability. Despite substantial efforts by researchers to better understand ME/CFS, there is no known cause or effective treatment. Diagnosing the disease remains a challenge, and patients often struggle with their illness for years before an identification is made. Some health care providers have been skeptical about the serious physiological - rather than psychological - nature of the illness. Once diagnosed, patients often complain of receiving hostility from their health care provider as well as being subjected to treatment strategies that exacerbate their symptoms. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome proposes new diagnostic clinical criteria for ME/CFS and a new term for the illness - systemic exertion intolerance disease(SEID). According to this report, the term myalgic encephalomyelitis does not accurately describe this illness, and the term chronic fatigue syndrome can result in trivialization and stigmatization for patients afflicted with this illness. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome stresses that SEID is a medical - not a psychiatric or psychological - illness. This report lists the major symptoms of SEID and recommends a diagnostic process.One of the report's most important conclusions is that a thorough history, physical examination, and targeted work-up are necessary and often sufficient for diagnosis. The new criteria will allow a large percentage of undiagnosed patients to receive an accurate diagnosis and appropriate care. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome will be a valuable resource to promote the prompt diagnosis of patients with this complex, multisystem, and often devastating disorder; enhance public understanding; and provide a firm foundation for future improvements in diagnosis and treatment.
