Book excerpt: Ross here presents an original and controversial look at the moral principles that guide parents in making health care decisions for their children, and the role of children in the decision-making process. She opposes the current movement to increase child autonomy, in favor of respect for family autonomy and proposes significant changes in what informed consent allows and requires for pediatric health care decisions. The first systematic medical ethics book that focuses specifically on children's health care, Ross's work has important things to say to health care providers who work with children as well as to ethicists and public policy analysts.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides a comprehensive, scholarly, and practical account of delirium that will be of value for all doctors and nurses involved in the care of the elderly. It not only offers a state of the art update on delirium, covering its history, epidemiology, pathophysiology, assessment, diagnosis, causes, prevention, and management, but also presents evidence-based and practical information relevant to daily clinical routine. Owing to the complex multifactorial causes of delirium, different aspects of delirium in the elderly are discussed from a variety of perspectives. The book closes by presenting a series of case vignettes, delirium assessment tools and screening scales, and a list of those drugs highly associated with delirium. Delirium is a cognitive disorder characterized by deficits in attention, arousal, consciousness, memory, orientation, perception, speech, and language. It is a common and serious problem among older persons at every healthcare interface. Although it occurs in 10–60% of the older hospitalized population, delirium remains a relatively misunderstood and misdiagnosed condition. This book will be of interest to professionals working in geriatrics, geriatric psychiatry, general psychiatry, or neurology, internists, intensive care unit specialists, and all who care for the elderly in hospitals or the community.

Book excerpt: Implementing safety practices in healthcare saves lives and improves the quality of care: it is therefore vital to apply good clinical practices, such as the WHO surgical checklist, to adopt the most appropriate measures for the prevention of assistance-related risks, and to identify the potential ones using tools such as reporting & learning systems. The culture of safety in the care environment and of human factors influencing it should be developed from the beginning of medical studies and in the first years of professional practice, in order to have the maximum impact on clinicians' and nurses' behavior. Medical errors tend to vary with the level of proficiency and experience, and this must be taken into account in adverse events prevention. Human factors assume a decisive importance in resilient organizations, and an understanding of risk control and containment is fundamental for all medical and surgical specialties. This open access book offers recommendations and examples of how to improve patient safety by changing practices, introducing organizational and technological innovations, and creating effective, patient-centered, timely, efficient, and equitable care systems, in order to spread the quality and patient safety culture among the new generation of healthcare professionals, and is intended for residents and young professionals in different clinical specialties.

This book was released on 2015-01-08 with total page 164 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book disentangles the issues in connection with the advancement of Health Technology Assessment (HTA) and its interface with health policy. It highlights the factors that should shape its progress in the near future. Interdisciplinary and critical views from a number of professionals are put together in a prescient order to cast some light and make recommendations as to the next steps HTA should take to be fit for purpose. A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.
