Les principes fondamentaux de l'éthique sont bousculés et le réanimateur est face à des de nombreuses interrogations éthiques qui jalonnent le parcours du patient de son admission à sa sortie. Malgré une réflexion éthique formalisée guidant les décisions d'admission et de non-admission en réanimation pour les malades à haut risque de mortalité, la part laissée à la subjectivité est importante. Il n'y a pas cependant de situation figée pour laquelle un patient ne doit pas bénéficier de la réanimation. Une décision de non-admission en réanimation s'apparente à une décision de limitation thérapeutique. Ce schéma éthique s'accorde avec les principes éthiques : autonomie, bienfaisance, non malfaisance et justice distributive mais l'urgence rend souvent difficile l'analyse complète du dossier du patient.Le manque de lits de réanimation disponibles est susceptible d'influer sur la stratégie de triage soulignant l'impact de critères subjectifs dans le processus décisionnel. Ceci peut conduire à une sur-mortalité. Les études princeps réalisées sur les décisions de limitations et d'arrêt thérapeutiques (LAT) ont été à l'origine du débat sur la non-obstination déraisonnable aboutissant à terme à la promulgation de la Loi Leonetti. Ces données ont permis de corriger des dysfonctionnements éthiques lors des décisions de LAT. A la suite des questionnements récents de notre société nous avons analysé la réflexion des réanimateurs sur l'euthanasie.La réalisation de prélèvements d'organe chez des patients décédés d'arrêt cardiaque impose l'admission en réanimation de patients pour les quels les traitements n'ont plus l'objectif de les sauver, mais de préserver leurs organes pour le bien d'autrui. Ces situations soulignent les interrogations sur : la définition de la mort, l'instrumentalisation du corps, le consentement au don, l'information des proches et plus largement du public.

Available in PDF, EPUB and Kindle. Book excerpt: La loi Leonetti promulguée en 2005 définit les procédures de limitation ou arrêt des thérapeutiques (LAT) invasives dans les cas de fin de vie. En réanimation, la réflexion sur le niveau d'engagement thérapeutique est quotidienne. Méthode : Nous avons réalisé une étude monocentrique rétrospective des décès survenus entre novembre 2011 et décembre 2013 dans le service de réanimation du centre hospitalier de Tourcoing. La mise en place et les circonstances des procédures de LAT instaurées avant un décès étaient étudiées. Résultats : Une procédure de LAT était instaurée pour 177 (79%) des 224 patients décédés. Une décision de limitation seule était prise pour 54 (24,1%) patients. Une décision de limitation suivie d'un arrêt thérapeutique était prise pour 61 (27,2%) patients. Soixante deux (27,7%) décès survenaient après une décision d'arrêt des thérapeutiques. Les limitations concernaient 115 (65%) décisions de LAT et l'arrêt 123 (69,5%). Un antécédent de cirrhose (OR = 11,7 ; IC95% [1,1-106,4] ; p = 0,02), un score de Knaus B, C, D (OR = 5,4 ; IC 95% [2,2-12,7] ; p = 0,0001), la survenue d'une complication médicale en cours de séjour (OR = 3,4 ; IC 95% |1,3-8,1] ; p = 0,0007) et un SOFA neurologique entre 0 et 2 à l'admission (OR = 2,7 ; IC 95% [1,1-5,9] ; p = 0,01) étaient associés à une procédure de LAT. Concernant les décès survenant dans les 72 heures suivant l'admission, une défaillance rénale sévère à l'admission était associée à une LAT (OR = 4,1 ; IC 95% [1,2-13,2] ; p = 0,0002) ainsi qu'un score de Knaus B, C, D (OR = 6,9 ; IC 95% [2,4-19] ; p = 0,01). La réanimation d'un arrêt cardiaque était la mesure la plus fréquemment limitée (91,3%). Le support vasopresseur était ensuite le traitement le plus limité (80%) et arrêté (65,8%). L'échappement thérapeutique motivait la décision dans 121 cas (68,9%). Conclusion : Notre étude confirme la place importante des procédures de LAT parmi la population de patients décédés dans notre service.
