The protective role of CD4+ and CD8+ T cells has been inferred from studies on patients who recovered from SARS and MERS and accumulating data are now showing their relevance in SARS-CoV-2 infection. Moreover, memory T cells induced by previous pathogens can shape the susceptibility to, and the clinical severity of other infections, but the complete picture has yet to be elucidated. If the virus is not rapidly eliminated, COVID-19 may progress towards a secondary inflammatory phase that is directly responsible for a worsening in clinical symptoms and immune system impairment. Besides marked lymphopenia, COVID-19 patients’ T cell compartment displays several alterations involving different subpopulations of T cells in terms of phenotype, metabolic profile and functionality.

Subsequently, with the application of recombinant DNA techniques, there have been remarkable advances in our understanding of the molecular biology of coronaviruses, and a mass of structural data concerning coronavirus genomes, mRNAs, and pro teins now exists. More recently, attention has been focused on the role of essential and accessory gene products in the coronavirus replication cyde and a molecular analysis of the structure-function relation ships of coronavirus proteins. Nevertheless, there are still large gaps in our knowledge, for instance, in areas such as the genesis of coronavirus subgenomic mRNAs or the function of the coronavirus RNA-dependent RNA polymerase. The diseases caused by coronaviruses have been known for much longer than the agents themselves. Possibly the first coronavirus-related disease to be recorded was feline infectious peritonitis, as early as 1912. The diseases associ ated with infectious bronchitis virus, transmissible gastroenteritis virus, and murine hepatitis virus were all well known before 1950.

Were the immune and other protective systems of the body able to maintain themselves over the course of time, and if there were no degradation related to age, the everyday loss of energy and vitality that occurs in the lives of older people as a consequence of viruses, arthritis, and other debilitating circumstances would be greatly lessened. The objective of gerontologists is not just to extend the life span but rather to improve the vigor, health, and quality oflife. To date, we have not developed a single index to measure immunity that is of use clinically in the evaluation of older people and of their immunologic compe tence. It may not be surprising that just such a clinical index may be available in the not-too-distant future. We can also look forward to the assembling of a greater body of information explaining how and why the immune system fails with age while, paradoxically, the incidence of autoimmune diseases increases with age. It is this latter phenomenon that may playa part in a wide range of chronic diseases from rheumatoid arthritis to senile dementia.

In order to assure aperfect balance within the enormous com plexity of the immune system, it is not surprising that multiple self-regulatory mechanisms are organized at different levels, such as antibody feedback, idiotypic-anti-idiotypic responses, suppres sor and helper T cells, lymphokine signals and genetic require ments. A nu mb er of observations in recent years have, however, demonstrated that consistent contributions to the immunological homeostasis are given also by signals generated outside of the immune system, namely,in the central and autonomous nervous system as weIl as in the endocrine apparatus. Furthermore, the interactions between the immune system and the other body homestatic mechanisms seem to be bidirectional: if immunological cells may be targets of neuroendocrinological factors, immunological products seem in turn to contribute to the neuro endocrine homeostasis.
