Book excerpt: India in particular and South-Asia in general have witnessed a rapid increase in the prevalence and incidence of cardiovascular disease over the past 25 years. Lifestyles changes, unhealthy diet, lack of regular physical exercise, and obesity have all led to rising prevalence of metabolic syndrome. It is of no wonder that metabolic syndrome is being increasingly recognized as a clinical entity which is believed to be associated with increased risk of cardiovascular disease beyond individual risk factors, though this is at times debated. This monograph addresses the total nuance of metabolic syndrome in its entirety and answers questions frequently asked on this subject. The authors are internationally respected investigators in their own right having made major contributions in the particular field and are revered teachers as well. The book itself has clear sections which makes it very user friendly and divided into two volumes. The first volume has the evaluation of metabolic syndrome and the vastness of the problem and how it leads to smouldering dysfunctional endothelium making such a patient vulnerable to vascular disease. It purely deals with clinical issues we face daily in metabolic syndrome and patients. It is fairly broad-based to answer most of the queries which arise in a busy clinician’s head while dealing with metabolic syndrome on a day-to-day basis. Of special interest are chapters on metabolic syndrome in children, non-alcoholic fatty liver disease and hypogonadism.

Book excerpt: This book provides a unique and timely multidisciplinary synthesis of our current knowledge of the anatomy, pharmacology, physiology and behavioral data of the serotonin (5-HT)-dopamine (DA) interactions. Central serotonergic and dopaminergic systems play a critical role in the regulation of normal and abnormal behaviors. Moreover, recent evidence suggests that the dysfunction of the DA and 5-HT neurotransmitter systems contribute to various mental disorders including depression, schizophrenia, drug addiction and Parkinson's disease. This extremely important topic is of wide interest within the scientific community, with relevance not only to specialists but also to general practitioners and students. The book provides a valuable contribution to the debate on new pharmacological approaches for several psychopathological states, with contributions from expert neuroscientists and pharmacologists who comprehensively survey the most significant currently active areas of dopamine/serotonin interactions. * Provides an understanding of the interaction between Serotonin and Dopamine. * Appeals equally to specialists, general practitioners, students and researchers. * Contributes to the debate on new pharmacological approaches to several psychopathological states * Gives a comprehensive anatomical description plus the physiology and pharmacology of dopaminergic and serotonergic systems * Singles out neuropsychiatric and suggests new therapeutic approaches.

The challenge is to identify the underlying cause of dizziness so as to streamline the treatment and improve quality of life. In day to day ENT practice incidence of dizziness is reported to be 10–15%. It is often associated with a range of otological, neurological, and psychiatric disorder. There are fewer subjects in medicine which keep us more confused than vertigo. Dizziness and vertigo may be disturbing and incapacitating causing problems to people doing regular or specialized tasks such as driving, operating heavy machinery or flying. As the sensations are hard to describe, patients often use the words "dizziness" and "vertigo" interchangeably and inconsistently. Patients with the same underlying disorder might describe the symptoms very differently and might also differently describe the same "dizzy" event depending upon the how the question is directed. The aim of this project is to explore the experience and learning of some of the eminent medical professionals, develop relevant updates, and make them easily available for utilization by the Indian clinical practitioners.

Available in PDF, EPUB and Kindle. Book excerpt: A number of scar revision techniques can be utilized to treat specific types of scars, in combination with each other or with adjunctive therapies to achieve optimal results. Pathological conditions like hypertrophic scars and keloids are still challenging. Though a number of therapies exist to treat keloids, such as intralesional steroids and anti-metabolites, cryosurgery, to name a few, none are definitive in treatment. Understanding the molecular basis of keloids may lead to development of new therapies. Striae, also known as ‘stretch marks’, are extremely common, disfiguring and a real challenge to treat. Management of striae is a neglected field of research. Except for topical retinoids, the efficacy satisfactorily. Current treatment options include chemical peels, lasers and collagen induction therapy, with a tendency towards active intervention. However, studies are necessary to determine treatment strategies which produce results that are effective with minimal adverse effects. Post-acne scarring is one of the most common causes of facial scars, causing considerable cosmetic disability. Acne is a common skin disorder, unfortunately affecting a large number of teenagers. Usually patients have a combination of different types of scars; hence no single modality is useful. A combination of procedures is generally required for a satisfactory cosmetic outcome. The use of fractional lasers is gaining importance, as it is a minimally invasive procedure as compared to aggressive resurfacing procedures of the past. Collagen induction therapy is another promising technique. Today, patients are also getting up-to-date information about the various modalities available for scar treatment through media, such as magazines, advertisements and internet. However, exaggerated advertisements and information from nonprofessional sources, particularly with regard to lasers can influence patient expectations and result in unrealistic expectations. Patients need to understand that deep scars cannot be eliminated fully and can only be improved. They usually have high expectations from procedural techniques. Proper counseling is extremely important for a happy and satisfactory outcome. Current innovations and techniques in scar prevention and management and increasing use of minimally invasive surgeries are paving the way toward smoother, less noticeable scars. Every surgeon practicing aesthetics should be well versed with modalities of scar management and this is the aim of this clinical update.

Available in PDF, EPUB and Kindle. Book excerpt: Epigenetics of Stress and Stress Disorders, a new volume in the Translational Epigenetics series, examines the epigenetic mechanisms involved in modifying DNA following prolonged stress or trauma. This is accomplished through the evaluation of both the physiological and molecular effects of stress on the body that can eventually lead to stress disorders. The book begins by providing a psychiatric, biological, and phenomenological foundation for understanding stress disorders, before delving into the genomics of stress disorders. From here, chapter authors discuss a range of recent epigenetic research in the area, highlighting epigenome-wide association studies (EWAS), exciting developments in noncoding RNA studies, possible effects of prolonged stress on telomere shortening, and the long-term physical effects of PTSD on the health of patients. The book also examines the effect of adversity during sensitive periods or development and across the life span. The book concludes by looking at possible transgenerational stress-induced epigenetic alterations on future offspring and important areas of research for public health, along with the potential for epigenetic therapeutics or “epidrugs. Examines the epigenetics of stress, trauma, and related stress disorders Connects new research to clinical practice and highlights implications for patient care, drug discovery, and public health Discusses the epigenetic effect of adversity across the life span, and transgenerational stress-induced epigenetic alterations Features chapter contributions from international experts in the field
