Book excerpt: Currently, surgical management provides the definitive treatment of choice for most pituitary adenomas, craniopharyngiomas and meningiomas of the sellar region. The elegant minimally invasive transnasal endoscopic approach to the sella turcica and the anterior skull base has added a new dimension of versatility to pituitary surgery and can be adapted to many lesions in the region. In this multi-author book with numerous color illustrations the main aspects of the endonasal endoscopic approach to the skull base are presented, starting with a clear description of the endoscopic anatomy, the panoramic view afforded by the endoscope and the development of effective instruments and adjuncts. After the diagnostic studies, the strictly surgical features are considered in detail. The standard technique is described and particular aspects are treated, including the new extended approaches to the cavernous sinus, spheno-ethmoid planum and clival regions. The book stresses the importance of teamwork and has been produced by one of the pioneering groups in the field of endoscopic approaches to the pituitary. It is a useful guide primarily for neurosurgeons and sinonasal endoscopic surgeons but also for other specialists involved in the diagnosis and treatment of skull base lesions.

Book excerpt: This monograph gives an up-to-date account of the original results of the author's research in the complex field of the central control over relationships which have become established in the course of evolution between the hypothalamus and the pituitary gland. The author has limited the scope of this research on logical grounds to the study of central chemically reactive structures in the regulation of the adrenal glands through the intermediary of the hypothalamo-hypophyseal system. Through the author's skill and expertise in the analysis of the extensive and sometimes complicated literature he has successfully undertaken a differential approach to the analysis of the concrete role of catecholamines, acetylcholine, and serotonin in relation to the endocrine system. The vast range of experimental investigations and results of clinical observations published in the last few decades have revealed the humoral neurosecretory factors of the hypothalamus as impor tant regulators of the princ ipal functions of the pituitary gland and, through it, of the cyclic activity of the peripheral group of endocrine glands. Until recently the region of the cascade of regulatory in fluences preceding the hypothalamus was terra incognita. It is for this reason that Evgenii Vladimirovich Naumenko's monograph ac quires its special importance for the neurophysiologist, for its author has striven, by the use of experimental methods, to ascertain in detail the nature of influences which for a long time were in terpreted as the general dogmas of an abstract "nervism.

Book excerpt: This book provides the ultrastructural study of the pituitary–gonadal axis through the electron microscope. It is a compilation of extensive work carried out by Dr. D.A. Bhiwgade and his researchers over the last four decades. It explains the ultrastructural characteristics of pituitary cells in different morphological states and provides a comprehensive approach to identifying the structural and functional distinctions between various pituitary cell types. The book includes ultrastructural evaluations of the testis and epididymis after different experimental interventions to study the physiological correlations of the organs with spermatogenesis and the consequent structural deformities in sperm. An extensive section of different types of placentae is added for understanding the varied interactions of fetal and maternal tissues in the placental barrier. Placental types seen in bats, shrews, rodents, and monkeys are explained at the ultrastructural level to distinguish the tissue interrelationships within different placental layers. Detailed sketches have been provided to illustrate the correlations between the maternal and fetal tissues in different placental types for better understanding. This book is a ready reckoner and offers guidance to researchers and clinicians in understanding the cellular architecture and intracellular changes associated with various physiological states and clinical conditions of the pituitary, testis, epididymis, and placenta. This book offers assistance to all those using electron microscopy to evaluate cellular and tissue systems.

Based on a recent scientific meeting held in Angra dos Reis, Brazil, this volume of Frontiers of Hormone Research presents the latest research results: specialists from Latin and North America, Europe and Australia discuss in their contributions different aspects of pathogenesis, diagnostics and therapy of pituitary disease and pituitary tumors. Topics covered include pituitary cell development and organization in adult pituitary, tumorigenesis, prolactinomas, growth hormone and acromegaly, hypothalamic-pituitary-adrenal axis, and Cushing disease. This update on basic and clinical pituitary research is essential reading for endocrinologists, neuroendocrinologists and neurosurgeons as well as researchers and biochemists interested in the different aspects of pituitary physiopathology.

Book excerpt: The pituitary, albeit a small gland, is known as the "master gland" of the endocrine system and contributes to a wide spectrum of disorders, diseases, and syndromes. Since the publication of the second edition of The Pituitary, in 2002, there have been major advances in the molecular biology research of pituitary hormone production and action and there is now a better understanding of the pathogenesis of pituitary tumors and clinical syndromes resulting in perturbation of pituitary function. There have also been major advances in the clinical management of pituitary disorders. Medical researchers and practitioners now better understand the morbidity and mortality associated with pituitary hormone hyposecretion and hypersecretion. Newly developed drugs, and improved methods of delivering established drugs, are allowing better medical management of acromegaly and prolactinoma. These developments have improved the worldwide consensus around the definition of a "cure" for pituitary disease, especially hormone hypersecretion, and hence will improve the success or lack of success of various forms of therapy. It is therefore time for a new edition of The Pituitary. The third edition will continue to be divided into sections that summarize normal hypothalamic-pituitary development and function, hypothalamic-pituitary failure, and pituitary tumors; additional sections will describe pituitary disease in systemic disorders and diagnostic procedures, including imaging, assessment of the eyes, and biochemical testing. The first chapter will be completely new – placing a much greater emphasis on physiology and pathogenesis. Two new chapters will be added on the Radiation and Non-surgical Management of the Pituitary and Other Pituitary Lesions. Other chapters will be completely updated and many new author teams will be invited. The second edition published in 2002 and there have been incredible changes in both the research and clinical aspects of the pituitary over the past 8 years – from new advances in growth hormones to pituitary tumor therapy. - Presents a comprehensive, translational source of information about the pituitary in one reference work - Pituitary experts (from all areas of research and practice) take readers from the bench research (cellular and molecular mechanism), through genomic and proteomic analysis, all the way to clinical analysis (histopathology and imaging) and new therapeutic approaches - Clear presentation by endocrine researchers of the cellular and molecular mechanisms underlying pituitary hormones and growth factors as well as new techniques used in detecting lesions (within the organ) and other systemic disorders - Clear presentation by endocrinologists and neuroendocrine surgeons of how imaging, assessment of the eyes, and biochemical testing can lead to new therapeutic approaches
