Pour mener à bien ces nouvelles missions, le pharmacien d'officine va devoir développer des relations de plus en plus personnalisées avec sa clientèle. C'est un véritable changement d'état d'esprit. La première partie « Un projet ambitieux pour la pharmacie » met l'accent, d'une part, sur l'historique du projet de loi, et d'autre part, sur les priorités de la loi. Elaborée à l'issue d'un long processus de concertation, la loi HPST a fait évoluer l'organisation territoriale du système de santé et le cadre d'exercice des professionnels de santé. Le pharmacien d'officine est directement concerné par cette nouvelle législation qui s'inscrit dans le prolongement des réformes antérieures. Attendue par la profession, la loi HPST fait sensiblement évoluer le positionnement du pharmacien par rapport au malade, afin que ce dernier ait une prise en charge plus adaptée. La nouvelle relation est celle d'un soignant avec un soigné. La deuxième partie « le renouveau de l'art pharmaceutique » aborde les nouvelles missions du pharmacien. La loi consacre l'officine comme poste avancé de 1er recours afin de mieux prendre en charge le patient dès son entrée dans l'officine. D'expert du médicament, le pharmacien d'officine doit devenir, à travers ces nouvelles actions en matière d'éducation thérapeutique, rendez-vous santé, tests de dépistage, accompagnement des malades chroniques ou encore pharmacien correspondant, un acteur de santé public avec de solides connaissances en médecine de premier recours. Le travail en réseau est devenu un mode de fonctionnement permettant d'optimiser le parcours de soins. Pour la première fois, la loi HPST introduit dans le cadre législatif la coopération entre professionnels de santé. Ils pourront désormais coordonner leurs interventions autour du patient. La troisième partie est consacrée à « l'impact sur la pratique quotidienne du pharmacien ». Le métier de pharmacien se trouve à la confluence d'enjeux importants : la maîtrise des dépenses de santé, la baisse du volume et du prix des médicaments, le développement de nouvelles missions, l'incertitude sur l'avenir du réseauofficinal. Tous ces éléments obligent à repenser différemment l'économie de l'officine. La nouvelle convention pharmaceutique signée le 4 avril 2012 met en oeuvre la diversification des modes de rémunération du pharmacien. Elle fait entrer l'officine dans l'ère des honoraires et des missions rémunérées. L'acte pharmaceutique est désormais au centre de l'évolution de l'officine. Il oblige lepharmacien à une régulière mise à niveau, afin d'être mieux qualifié dans l'expression de son conseil et de ses actes. La loi HPST introduit le concept de développement professionnel continu. Ces mesures traduisent concrètement la volonté des pouvoirs publics de garantir des soins de qualité à l'ensemble de la population.

Afin d'évaluer la perception des professionnels de santé vis-à-vis de l'évolution de ces missions, une enquête a été réalisée en 2019 auprès de 106 pharmaciens d'officine et de 102 médecins généralistes. Cette thèse fait en première partie un état des lieux des activités déployées récemment en officine. La deuxième partie est composée des résultats de l'enquête, et de la discussion de ces données face à l'actualité du métier de pharmacien d'officine.

De plus, depuis le dernier avenant à la convention les pharmaciens sont davantage rémunérés en fonction du type d'ordonnance plutôt qu'au volume de boites dispensées. Le travail que nous avons réalisé présente dans un premier temps les différentes propositions de modification de l'exercice pharmaceutique en France. Nous avons ensuite souhaité recueillir la perception des nouvelles missions du pharmacien par les patients mais également les pharmaciens eux-mêmes à travers une enquête menée dans l'ancienne région Poitou-Charentes. Les résultats de cette enquête mettent en évidence que les patients et les pharmaciens sont en majorité favorables à voir s'accroitre l'implication du pharmacien aux côtés du médecin dans la prise en charge de patient. Le développement des soins pharmaceutiques, déjà déployés dans d'autres pays, semble donc attendu.

Nor does the premise that organisms defend themselves at the cellular or molecular levels. For nearly two thousand years “immunity,” a legal concept invented in ancient Rome, serves almost exclusively political and juridical ends. “Self-defense” also originates in a juridico-political context; it emerges in the mid-seventeenth century, during the English Civil War, when Thomas Hobbes defines it as the first “natural right.” In the 1880s and 1890s, biomedicine fuses these two political precepts into one, creating a new vital function, “immunity-as-defense.” In A Body Worth Defending, Ed Cohen reveals the unacknowledged political, economic, and philosophical assumptions about the human body that biomedicine incorporates when it recruits immunity to safeguard the vulnerable living organism. Inspired by Michel Foucault’s writings about biopolitics and biopower, Cohen traces the migration of immunity from politics and law into the domains of medicine and science. Offering a genealogy of the concept, he illuminates a complex of thinking about modern bodies that percolates through European political, legal, philosophical, economic, governmental, scientific, and medical discourses from the mid-seventeenth century through the twentieth. He shows that by the late nineteenth century, “the body” literally incarnates modern notions of personhood. In this lively cultural rumination, Cohen argues that by embracing the idea of immunity-as-defense so exclusively, biomedicine naturalizes the individual as the privileged focus for identifying and treating illness, thereby devaluing or obscuring approaches to healing situated within communities or collectives.

This discerning insight, which dates back more than 2000years to Aristotle, seems to have been ignored by most psycholo gists. For more than 40years theories of human action have assumed that cogni tion and action are merely two sides of the same coin. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
