Book excerpt: The relatively frequent occurrence of rapid onset and very brief, but often florid, psychotic states, with periodic recurrence, alongside relatively low rates of PTSD and chronic psychosis, were unexpected findings from the 2004 East Timor Mental Health Study, conducted in the context of the country’s recently won independence and in the wake of the atrocities endured in the protracted fight for sovereignty. Further unanticipated was the frequent association of recurrence with the time of the new moon (fulan lotuk) and other times or places of sacred (lulik) or associated cultural significance. The perceived violation of culturally sacrosanct lulik obligations often also appeared to foreshadow the initial onset of such patterns of distress. Significant episodes of trauma and loss appeared a hidden feature of affected individuals histories, which we argue have become symbolically entwined with local cultural understandings of ritual obligation, sacredness, and taboo. This volume develops a dynamic but contextualized multi-level formulation of psychosis and psychotic-symptoms, able to incorporate a range of factors from the biological, through the sociocultural, to the political. The work is truly interdisciplinary drawing on both the quantitative and qualitative findings of our own study but further supported through local ethnography and broader anthropological enquiry into the outcomes of psychosis in non-Western settings; psychoanalysis and psychoanalytic anthropology; evidence and theory exploring links between trauma, dissociation and psychosis; and novel culturally-adaptable psychosocial focused interventions for psychosis. We situate both evidence and theorising in wider epistemological and political context, including in relation to the movement for Global Mental Health. Culturally patterned presentations of brief remitting-relapsing psychosis are ultimately conceived as the trade-off between competing fragmentary and synthetic forces: the former in part secondary to the lasting and deleterious effects of overwhelming loss, trauma and adversity; the latter emboldened by cultural meaning and social response in the context of broad ecological pressures demanding survival and resilience.

This second edition includes a final chapter on "Short-Term Dynamic Psychotherapy," in which the historical background of brief psychotherapy, focusing on the contributions of Freud, Ferenczi, Rand, and Alexander, is examined and synthesized. Throughout, certain basic themes stand out. First is the necessity for building upon a solid foundation of scientific thought, coupled with a readiness to change theories that do not fit with established facts. He also deals with the importance of cultural and socioeconomic factors in individual personality development. The work concludes with a series of chapters on interethnic hostility, nationalism, and urban violence. Marmor's work clarifies the nature of the psychoanalytic process by liberating it from obscurantism and jargon. This book points the way toward unraveling some of the cognitive dissonance in this area. As Leon Eisenberg observed, this is "an admirable vade mecum of dynamic psychiatry both for residents in training and clinicians in practice."

With its origins in nineteenth-century Europe, psychiatry evolved as an ethnocentric body of knowledge, the vehicle of implicit and overt racism. Originally published in 1988 this author, however, saw no reason why the contemporary psychiatrist should not challenge this ethnocentrism. He provides a critical account of the development of psychiatry in relation to its cultural context and then examined contemporary practice of the time in the light of this development. Throughout, the book is informed by an awareness of issues of race and culture and of their difficult interactions, the author emphasising both the frequency of racist attitudes and the very real cultural distinctions in our society, distinctions that can be used to mask what are actually racist sentiments. What emerges is not just a plea for an anti-racist, culture sensitive psychiatry, but a blueprint for how this can be brought about. He argued that the shift towards community work and social psychiatry could reorientate the profession by confronting it with its social setting and responsibilities. This book represented a significant contribution to this literature for all mental health professionals and social scientists with an interest in this field at the time; the author has gone on to write many more.

Thirty-nine papers were presented during the four days of the conference including an invited address by Kenneth Gergen. Unfortunately, even after severely restricting the length of the written papers, the required limitation on printed space permitted only 34 of these to be included in these proceed ings. While most of the papers required editorial adjustment, the authors had final word on style and content. The classification of conference papers tends to be somewhat arbi trary' but we decided to structure the chapters under four headings: phi losophy, metatheory, methodology, and theory. By philosophy we refer primarily to issues in the philosophy of science as these pertain to the dis cipline of psychology. Metatheory refers to theory, drawing on the philo sophy of science and having implications for methodology, about psycho logical theories. Methodology refers to both theory of inquiry and specif ic methods of investigation as these derive from philosophical and meta theoretical considerations. By theory we refer to both analyses of the foundations of extant theory, including historical and sociological ana lyses, and the advancement of substantive theory.

It is the purpose of this book to remove these misconceptions and misunderstandings, and to bring up to date our knowledge in certain fundamental areas of learning theory, behavior therapy, and the biological foundations of per sonality and individual differences. There are three major groups of misconceptions and misunderstandings. The first of these relates to beliefs held by many psychiatrists and cognitive psychologists relating to behavior therapy.
