Book excerpt: The development of head and neck cancer is a multistep process in which genetic as well as epigenetic alterations interfere with the regular functions of proto-oncogenes and tumor suppressor genes. In addition, carcinogenesis is promoted by the impairment of mechanisms responsible for the maintenance of genetic stability. The accumulation of crucial events such as evasion of apoptosis, lack of senescence, deregulated proliferation, invasion and metastasis, as well as neoangiogenesis eventually determines the malignant phenotype. The 16 articles of this volume span from advances in the understanding of underlying molecular pathways (conditional mouse model, folate activity, DNA repair, genomic heterogeneity, p53 family members, human papillomavirus), to preclinical and clinical studies investigating potential novel therapeutic targets (epidermal growth factor receptor, nuclear factor-kappaB, vascular endothelial growth factor, chemokine receptors, P53-based immunotherapy, virus-modified tumor vaccination), and biometric considerations on the evaluation of prognostic factors. This state-of-the-art publication is recommended reading not only for ENT specialists and oncologists, but also for molecular biologists and other subspecialists interested in head and neck cancer research.

Available in PDF, EPUB and Kindle. Book excerpt: Head and neck cancer (HNC) is a heterogeneous group of cancers that, if combined, represent one of the most common cancer types. Patients with HNC suffer significant morbidity and mortality due to the importance of the structures involved. Over two-thirds of these patients are diagnosed at a late stage, leading to a poor prognosis. Therefore, advancements in early detection and treatment of HNC are crucial. This second volume provides an up-to-date overview of the theoretical background in the field of head and neck cancer (HNC) as well as of the emerging research that is impacting our understanding of this disease. The book begins with a comprehensive review of the epidemiology, etiology, symptoms, diagnosis, and staging of HNC. Next, it covers the essentials of potentially malignant disorders of the oral cavity, an important variety of HNC. Subsequently, it covers the newly emerging research in the field of HNC. The overall goal is to shift towards precision medicine (discussed in detail in Volume I), which will bring individualized clinical benefit to patients with HNC. This second volume of Early Detection and Treatment of Head & Neck Cancers concludes with the topic of chronic pain associated with HNC, including both the mechanisms of pain and the management strategies, and the emerging oral mucoadhesive drug delivery approach for HNC. All HNC surgeons, scientists, residents, and individuals whose lives have been touched by this disease, will recognize the impact pain has upon a patient’s health and his or her recovery trajectory.

Book excerpt: This second edition provides a comprehensive view of consolidated and innovative concepts, in terms of both diagnosis and treatment. Written by leading international physicians and investigators, this book emphasizes the necessity of combining local and systemic treatments to achieve the objective of yielding higher cure rates and lower toxicities. Heavily updated from the previous edition, it highlights new surgery and radiotherapy techniques, disease awareness, patient quality of life, and comprehensive management. Head-and-neck cancers are a complex clinical entity and their response to treatment is also known to vary markedly in function of host-related factors. Notwithstanding the impressive progresses observed in the field of imaging, head and neck cancers are often diagnosed at a late stage and the presence of locally advanced disease in a significant number of patients implies the use of aggressive treatments in order to both ensure local disease control and reduce distant metastasis risks. In comparison with the first edition, Head and Neck Cancer, Second Edition provides a detailed update of innovative concepts in chemo- and bio-radiation, viral infection impact on tumor growth and response to treatment, and impact of tumor- and host-related factors on treatment outcome.

Book excerpt: Squamous cell cancers of the head and neck (SCCHN), also known as head and neck cancers (HNC) encompass malignancies of the oral cavity, larynx, nasopharynx and pharynx, and are diagnosed in over 500,000 patients worldwide each year, accounting for 5% of all malignancies. In the past several years, there have been significant developments in understanding of HNC. It is now recognized that although alcohol and tobacco use has represented the likely predominant cause of SCCHN, the incidence of a second class of SCCHN related to oncogenic human papillomavirus (HPV) infection is increasing, with a four-fold increase in the past 2 decades, and now thought to represent up to 30% of cases. The first effective target for SCCHN, the EGFR-targeting antibody cetuximab, was approved as recently as in 2006; since then, a growing body of research has identified additional signaling pathways as important in disease pathogenesis, and in resistance to treatment. Proteins such as c-Met, Src, and HER2 are emerging as new therapeutic targets, with a considerable ferment in the clinical trial community. As a capstone of research progress, 2011 marked the first reports of high throughput sequencing of SCCHN tumors, with these efforts identifying unexpected players such as Notch as frequent subject of mutation, spawning new hypotheses for future research. This book will be of interest to researchers who are interested in better understanding the biology of head and neck cancers, with the goals of better designing therapies, identifying risk factors, or investigating the molecular basis of the disease.

Available in PDF, EPUB and Kindle. Book excerpt: Tumor biology varies at different subsites in the oral cavity, and therefore carcinoma of the gingivo-buccal sulcus (also known as Indian cancer) behaves differently from carcinoma of the tongue. Oral cancer is very often diagnosed at an advanced stage with neck metastasis, and most cases have a guarded prognosis. Loco-regional recurrences are frequent and morbidity following treatment is considerable. However, results have improved in recent years, particularly in early and certain advanced stages, due to better imaging and case selection, improved surgical techniques, including those for reconstructive surgery, a multidisciplinary team approach and a better understanding of tumor biology and prognostic factors. This book comprehensively and systematically reviews all these aspects of diagnostic and therapeutic advances in oral cancer, particularly discussing early detection, epidemiology, chemoprevention and current research. Written by specialists who are active leaders in basic sciences or clinical fields, the chapters address individual and collective issues in managing patients with oral cancer, and provide insights into various treatment approaches. The authors share their knowledge and experience and provide extensive literature sources to encourage clinicians to be flexible, innovative and help them make the best, educated decisions for individual patients.
