Book excerpt: After summarizing the dramatic changes that have taken place in caries prevalence during the 50-year lifespan of ORCA, this publication focuses on current concepts of the control of dental caries. The papers are all written by leading experts in their respective fields and provide an invaluable overview of current thinking in caries research. The main section comprises 19 papers under the headings: 'Defining and diagnosing caries'; 'How to prevent caries?' and 'How to manage caries?', which comprehensively review the basic and clinical sciences of caries control. Finally, looking ahead, the problems of how to translate research into clinical practice are discussed. The issue concludes with a concise summary of the discussions stimulated by the presentations. This publication is unique in bringing together up-to-date, authoritative reviews of this multifactorial disease and is highly recommended to basic and clinical scientists interested in any aspect of caries research, to dental public care planners, and academic dentists involved in teaching aspects of caries aetiology, prevention and management. Book excerpt: Dental caries is defined as a multifactorial, transmissible, infectious oral disease caused primarily by the complex interaction of cariogenic oral flora (biofilm) with fermentable dietary carbohydrates on the tooth surface over time. At the tooth surface and sub-surface level, dental caries results from a dynamic process of attack (demineralization) and restitution (remineralization) of the tooth matter. It is essential to understand that carious lesions, or cavitations in teeth, are signs of an underlying condition, an imbalance between protective factors (i.e., those favouring remineralization) and pathologic factors (i.e., those favouring demineralization), balance tilting towards the latter. In clinical practice, it is very easy to lose sight of this fact and focus entirely on the restorative treatment of caries lesions, failing to treat the underlying cause of the disease.

Book excerpt: This book provides dental professionals with a clear understanding of current clinical and scientific knowledge on the various aspects of pulp treatment for both primary and young permanent teeth. Diagnostic parameters are clearly presented, along with step-by-step descriptions of clinical procedures, including indirect and direct pulp treatments, pulpotomy, and pulpectomy. The rationale for the materials used in each technique and their individual merits and disadvantages are examined in detail. In the case of pulpotomy, all the materials used since the introduction of this treatment modality are discussed (e.g., formocresol, glutaraldehyde, ferric sulfate, and MTA) and the roles of sodium hypochlorite, electrofulguration, and laser therapy are elucidated. Special attention is devoted to pulpectomy and root canal treatment, with consideration of debriding and obturation techniques, rinsing solutions, and root-filling pastes. A further individual chapter is dedicated to restorations of teeth treated with the different types of pulp therapy. The various conservative treatment modalities are also presented, including specific treatments for immature nonvital permanent teeth. The concluding chapter looks to the future and the potential value of stem cells in pulp therapy.

Available in PDF, EPUB and Kindle. Book excerpt: Dental caries, despite significant reductions in some populations, continues to be a major public health problem, and is the most prevalentdisease affecting humans. However, even fundamental aspects of thepathogenesis of caries remain uncertain, calling for continuing scientific enquiry and the application of sophisticated techniques, from epidemiology to molecular biology. Although recent changes in the pattern of dental caries are identifiable, available data does not reveal whether, overall, caries is declining. The conference `Cariology in the Nineties', held at the University of Rochester in 1991, by focusing on the etiology, pathogenesis and prevention of dental caries, emphasised the need to co-ordinate research in monitoring populations worldwide, and to change the focus of research to reflect the changing epidemiology and patterns of dental disease. Contents: Keynote address; Epidemiolgy; Saliva; Teeth; Clinical studies; Microbiology/Plaque; Diet/Nutrition/Prevention. W.H. BOWEN is Professor and Chair, Department of Dental Research,University of Rochester. LAWRENCE A. TABAK is Professor of Dental Research and of Biochemistry, University of Rochester.
