Book excerpt: Recently reported experimental retrograde coronary venous techniques protect the acute ly ischemic myocardium and appear to be a sound approach for temporary support of the severely jeopardized myocardi urn. The current brief report refers to important features of coronary venous interventions, describes apparent improvements, and discusses persist ing difficulties in research and developmental evaluation of results. Taking note of the need of informing the scientific community, the recently established International Work ing Group on Coronary Sinus Interventions (2) wishes to become a forum in which free discussion and criticism should be encouraged in the hope of stimulating research and the development and application of safe methods in the treatment of coronary disease. The eventual objective is to more fully define the limitations of applications and those ranges in which they can be applied without incurring any risks. Our ultimate goal is to maxi mize the efficiency of applications while at the same time preventing the occurrence of undesirable side effects. References I. Available in PDF, EPUB and Kindle. Book excerpt: Interventional cardiology refers to catheter-based treatment of cardiovascular diseases. This dynamic new field is one of the fastest growing fields in medicine. In the United States, more than a million percutaneous coronary interventions (PCI) are performed annually; moreover, the number of procedures performed will likely continue to increase. The field of interventional cardiology has evolved dramatically over the past decade with the availability of embolic protection devices, drug eluting stents, mechanical thrombectomy catheters, etc. Interventional cardiology represents a rapidly moving target for the cardiologist who seeks to integrate recent advances into their clinical practice. The advent of newer technology presents both an opportunity as well as numerous challenges for the cardiologist to treat patients optimally. Interventional Cardiology: Essential Clinicians Guide (Oxford American Cardiology Library) aims to provide clinicians with concise, easily accessible guidance on the pre-procedural, procedural and post procedural aspects of PCI in general situations and in various specific clinical settings. The volume features evidence-based discussion on patient selection, vascular access, general principles of PCI, and various specific settings and optimal post procedure management of these patients. This handbook focuses on tailoring stent type to the individual patient, taking into account specific risk factors and comorbidities, appropriate use of embolic protection and thrombectomy catheters. Highly practical in format, content and style, Interventional Cardiology: Essential Clinician's Guide serves as a succinct, yet comprehensive, easily accessible point-of-care reference for busy practitioners. Useful tools, such as treatment algorithms, charts, tables and illustrations assist in making this volume a practical reference tool.

Book excerpt: It was a great pleasure for us to welcome so many experts from all over the world to our symposium in Aachen. We are also pleased - and you can attribute this to my own vanity - with the success and acceptance of the concept of myocardial reperfusion and revascularization - specifically selective intracoronary lysis followed either by PTCA or bypass surgery - which we have been pursuing since 1979. But after the dramatic immediate effects of the first attempts, which you have to experience yourself, we did not expect it to be any different. We decided against performing a randomized study in which every patient is catheterized, and thrombolytic therapy only given to some of the patients, for which we have been criticized at times. At the previous symposium in Aachen in 1983 on the topic of thrombolysis for acute myocardial infarction, in the final session several speakers were asked directly about this topic, and replied that if they were patients they would not want to be randomized into the placebo group. In the meantime positive results from large, randomized studies have been recorded, which are presented in this volume. I mention in particular the Western Washington Study from Dr. Kennedy and the Interuniversity Study in Holland. Aachen, a relatively small city with 250,000 inhabitants, provided the opportunity to treat a relatively large patient population with acute myocardial infarction.
