This book was released on 2014-03-30 with total page 368 pages. Available in PDF, EPUB and Kindle. Book excerpt: Army chaplains have long played an integral part in America’s armed forces. In addition to conducting chapel activities on military installations and providing moral and spiritual support on the battlefield, they conduct memorial services for fallen soldiers, minister to survivors, offer counsel on everything from troubled marriages to military bureaucracy, and serve as families’ points of contact for wounded or deceased soldiers—all while risking the dangers of combat alongside their troops. In this thoughtful study, Anne C. Loveland examines the role of the army chaplain since World War II, revealing how the corps has evolved in the wake of cultural and religious upheaval in American society and momentous changes in U.S. strategic relations, warfare, and weaponry. From 1945 to the present, Loveland shows, army chaplains faced several crises that reshaped their roles over time. She chronicles the chaplains’ initiation of the Character Guidance program as a remedy for the soaring rate of venereal disease among soldiers in occupied Europe and Japan after World War II, as well as chaplains’ response to the challenge of increasing secularism and religious pluralism during the “culture wars” of the Vietnam Era.“Religious accommodation,” evangelism and proselytizing, public prayer, and “spiritual fitness”provoked heated controversy among chaplains as well as civilians in the ensuing decades. Then, early in the twenty-first century, chaplains themselves experienced two crisis situations: one the result of the Vietnam-era antichaplain critique, the other a consequence of increasing religious pluralism, secularization, and sectarianism within the Chaplain Corps, as well as in the army and the civilian religious community. By focusing on army chaplains’ evolving, sometimes conflict-ridden relations with military leaders and soldiers on the one hand and the civilian religious community on the other, Loveland reveals how religious trends over the past six decades have impacted the corps and, in turn, helped shape American military culture.

Book excerpt: “Biobetters: Protein Engineering to Approach the Curative” discusses the optimization of protein therapeutic products for treatment of human diseases. It is based on the fact that though numerous important therapeutic protein products have been developed for life threatening and chronic diseases that possess acceptable safety and efficacy profiles, these products have generally not been reexamined and modified for an improved clinical performance, with enhancements both to safety and efficacy profiles. Advances in protein engineering, coupled with greatly enhanced understanding of critical product quality attributes for efficacy and safety, make it possible to optimize predecessor products for clinical performance, thereby enhancing patient quality of life and with the potential for great savings in health care costs. Yet despite such knowledge, there is little movement towards such modifications. This book examines engineering protein therapeutic products such that they exhibit an optimal, not just an adequate, clinical performance profile. Two product classes, therapeutic enzymes for lysosomal storage diseases (enzyme replacement therapies, ERT) and monoclonal antibodies (mAbs), are used as examples of what modifications to such proteins could be made to enhance clinical performance, “closer to a cure” as it were. For ERT, the key to optimizing clinical performance is to ensure the ERT is endowed with moieties that target the protein to the relevant target tissue. Thus, for Gaucher Disease, our best example of how to optimize an ERT to address a disease that manifests in specific target tissues (macrophages and monocytes), the enzyme has been extensively modified to target macrophages. For diseases such as Pompe Disease, largely a disorder of muscle, optimal performance of ERT will depend on endowing the enzyme with the ability to be taken up via the Mannose 6 Phosphate Receptor, and so one of the chapters in the book will discuss such approaches. Moreover, a major failure of biotechnology based products is to gain access to the CNS, a key target tissue in numerous diseases. Thus, a chapter has been devoted to strategies to access the CNS. Additionally, immune responses to therapeutic proteins can be highly problematic, eliminating the efficacy of life saving or highly effective protein therapeutics. This is especially poignant in the case of Pompe Disease wherein great improvement in muscle strength and functionality is lost following development of an immune response to the ERT with consequent patient deterioration and death. Thus, a chapter regarding protein engineering, as well as other non-clinical approaches to diminishing immunogenicity is a valuable part of the book. Monoclonal antibodies (mAbs) can be engineered to bind targets relevant to a wide variety of diseases; binding affinity, however, is only part of the equation and one of the chapters will present a molecular assessment approach that balances affinity with pharmacokinetics and manufacturability. As with other proteins immunogenicity can be problematic, being responsible for loss of efficacy of anti-TNF mAbs, often after prolonged successful treatment. The authors will also share their perspective on the consequences of physico-chemical modifications occurring to mAbs once they reach the circulation or their target, a research area open to further development from a protein engineering as well as analytical perspective. This book will also discuss novel platforms for protein therapeutics, technologies that exceed mAbs with respect to potency, and hence, potentially efficacy. These platforms consist largely of repeat domain proteins with very high affinity for their target ligands, but while potentially more efficacious, immunogenicity may be a major problem limiting use. The economics surrounding the issue of biobetters is another high-profile issue - this final chapter will explore the incentives and disincentives for developing biobetters and consider incentives that might make their pursuit more rewarding.

Book excerpt: The book provides an up-to-date account of the neuropsychological, cognitive-neurological, and neuropsychiatric aspects of movement disorders. The past ten years have seen an explosion of research covering non-motor aspects of Parkinson's disease and, more recently, movement disorders such as essential tremor, dystonia, corticobasal syndrome, progressive supranuclear palsy, and multiple system atrophy. It is often these neurobehavioral features that become troubling to the patient: they are sometimes difficult to recognize and treat, are associated with diminished patient and caregiver quality of life, and may hasten disease progression, loss of independence, and institutionalization. This book discusses the most recent diagnostic and treatment guidelines for such cognitive and psychiatric conditions in Parkinson's disease and other movement disorders, while providing practical tips and strategies for general assessment. The rapid accumulation of research in this field makes it increasingly difficult for one or two people to author a comprehensive text in an expert manner. The world-class team assembled for this volume succeeds in covering widely diverse areas such as the pathology, neuroimaging, assessment, and treatment of an ever-growing set of neurobehavioral features of movement disorders -- cognitive impairment and dementia, depression, apathy, anxiety, psychosis, and impulse control disorders. The text also surveys fundamental knowledge about basal ganglia function and dysfunction, assessment and evaluation techniques applicable to a range of movement disorders, and quality of life issues more broadly.

Available in PDF, EPUB and Kindle. Book excerpt: Neurosurgery is a rapidly developing and technically demanding branch of surgery that requires a detailed knowledge of the basic neuro-sciences and a thorough clinical approach. The Oxford Textbook of Neurological Surgery is an up-to-date, objective and readable text that covers the full scope of neurosurgical practice. It is part of the Oxford Textbooks in Surgery series, edited by Professor Sir Peter Morris. The book is split into 20 overarching sections (Principles of Neurosurgery, Neuro-oncology of Intrinsic Tumours; Extra-axial Tumours and Skull Lesions; Cerebro-Pontine Angle Tumours; Sellar and Supra-Sellar Tumours; Posterior Fossa Tumours; Pineal tumours; Uncommon Tumours and Tumour Syndromes; Neurotrauma and Intensive Care; Vascular Neurosurgery; Principles of Spinal Surgery; Spinal Pathology; Spinal Trauma; Peripheral Nerve Surgery; Functional Neurosurgery; Epilepsy; Paediatric Neurosurgery; Neurosurgery for Cerebrospinal Fluid Disorders and Neurosurgical Infection). Each section takes a dual approach with, 'Generic Surgical Management' chapters that focus on specific clinical problems facing the neurosurgeon (e.g. sellar/supra-sellar tumour, Intradural Spina Tumours etc.) and 'Pathology-Specific' chapters (e.g. Glioma, Meningeal Tumours, Scoliosis and Spinal Deformity, Aneurysm etc.). Where appropriate, this division provides the reader with easily accessible information for both clinical problems which present in a regional fashion and specific pathologies. The generic chapters cover aspects such as operative approaches, neuroanatomy and nuances. Specifically each chapter in the book incorporates several strands. Firstly the fundamental neuroscience (anatomy, pathology, genetics etc.) that underlies the clinical practice. Secondly, a review of the requisite clinical investigations (e.g. angiography, electrodiagnostics, radiology). Thirdly, a thorough evidence based review of clinical practice. Following this a consideration of the key debates and controversies in the field with 'pro-' and 'con-' sections (e.g. minimally invasive spine surgery, microsurgical treatment of aneurysms) is provided. A summary of the key papers and clinical scales relevant to neurosurgery form the concluding part. The book is a 'one-stop' text for trainees and consultants in neurosurgery, residents, those preparing for sub-specialty exams and other professionals allied to surgery who need to gain an understanding of the field. It acts as both a point of reference to provide a focussed refresher for the experienced neurosurgeon as well as a trusted training resource.

Book excerpt: Brain function is under metabolic control, which in turn determines the equilibrium of homeostatic systems that affect neuronal and glial networks on the molecular, cellular, and systems levels. The collection of articles ranges from molecules and mechanisms involved in regulating homeostasis and neuronal excitability to therapeutic mechanisms tailored to restore homeostatic function. It also features neurological diseases and novel treatment approaches that are based on metabolic and homeostatic interventions. Together, the collection of articles outlines novel strategies to restore brain function in neurology and highlights limitations of conventional pharmacological approaches. We suggest that restoration of molecular and biochemical networks could lead to a new era of therapeutic opportunities.
