A critical incident stress needs assessment was identified as a logical first step in the development of a planned response to this problem. It was determined that a very high incidence of exposure to critical incidents exists among the members of the Tulsa Fire Department who completed the survey, and that those members had experienced a high incidence of reactions to one or more of those incidents. Two recommendations were made for dealing with the risks associated with critical incident stress, on the Tulsa Fire Department as well as in the fire service. Pre-incident training and education concerning critical incident stress, and the development of a critical incident stress debriefing (CISD) team program were recommended to address the problem.

Book excerpt: Critical incident stress debriefing is a psycho-educational group offered to emergency service responders after exposure to potentially traumatizing events. The study utilized critical incident questionnaire methodology (Kivlighan & Goldfine, 1991) and consensual qualitative analysis (Hill, Knox & Williams, 1997) to gain understanding about the effectiveness of CISD group process variables. Process reports from thirty-eight informants were analyzed. Evidence of effective and ethical group work included elements of group planning, performing and processing. Attention to pre-CISD preparation, CISD participant composition, establishing ground rules and bounds for confidentiality, managing disclosure and member pacing were important group leadership aspects. Therapeutic factors suggest the CISD process is largely support group work and focuses on the event, not individual issues or growth. Themes of resolution include connection to peers and family members, recognition of good enough effort, and acceptance of human limitations. Analysis of informants' assumptive beliefs supported adaptive shifts in the acceptance of life's injustice with the belief that life is good and manageable.
