Book excerpt: Patient cost-sharing for primary care and prescription drugs is designed to reduce the prevalence of moral hazard in medical utilization. Yet the success of this strategy depends on two factors: the elasticity of demand for those medical goods, and the risk of downstream hospitalizations by reducing access to beneficial health care. Surprisingly, we know little about either of these factors for the elderly, the most intensive consumers of health care in our country. We remedy both of these deficiencies by studying a policy change that raised patient cost-sharing for retired public employees in California. We find that physician office visits and prescription drug utilization are price sensitive, with implied arc-elasticities that are similar to those of the famous RAND Health Insurance Experiment (HIE). However, unlike the HIE, we find substantial quot;offsetquot; effects in terms of increased hospital utilization in response to the combination of higher copayments for physicians and prescription drugs. These offset effects are concentrated in patients for whom medical care is presumably efficacious: those with a chronic disease. Finally, we find that the savings from increased cost-sharing accrue mostly to the supplemental insurer, while the costs of increased hospitalization accrue mostly to Medicare; thus, there is a fiscal externality associated with cost-sharing increases by supplemental insurers. Our findings suggest that health insurance should be tied to underlying health status, with chronically ill patients facing lower cost-sharing. We also conclude that the externalities to Medicare from supplemental insurance coverage may be more modest than previously suggested due to these offsets.

We compare outcomes before vs. after January 2006, for those over age 66 years vs. for those age 58-64 years, adjusting for the under-reporting of certain cash-only transactions in the WKH data. Our results indicate that from 2005-2007, Part D reduced elderly OOP costs per day's supply of medication by 21.7%, and increased elderly use of prescription drugs by 4.7%, implying a price elasticity of demand of -0.22. These effects occurred primarily during the first year of the program. An age- and time-standardized comparison of our quantity results with previous estimates from Walgreens data shows that our findings are 2.6 times as large. We conclude that Part D lowered elderly patients' OOP costs substantially and increased utilization modestly, and note that in comparing results across studies on this topic, magnitudes may vary substantially due to differences in data and methods.

The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: Economic theories have long postulated the benefits of presenting consumers with more choice. Psychologists have advanced a similar idea, postulating that greater choice is related to consumer satisfaction. Economists and psychologists alike have come to challenge this prevailing notion. Following the pioneering work of Herbert Simon on bounded rationality, researchers have argued that providing greater choice size can adversely affect consumers' ability to locate the best choice, lower satisfaction with choice made, and increase sense of regret. Others have suggested that more choice demands greater cognitive demands. In our study, we extend this novel line of investigation to an uncharted domain: older adults. We posit that older adults in the United States face an increasingly bewildering array of health insurance choices - with deleterious consequences if the wrong choice is made. This is especially the case since the implementation of the Medicare Modernization Act, and in particular Part D of the Medicare program, where older adults must confront (is some cases) over 100 different insurance plans. This is of particular concern as older adults tend to have greater interaction with the medical care system but oftentimes do so with reduced cognitive capacity. To investigate this issue we designed a set of experiments intended to examine the effect of choice size on younger (the control group) and older adults' ability to make better choices, and to evaluate their level of satisfaction with their choices. They are being carried out in a California community. We will present results from two experiments. The first study examines older adults' ability to make choices among varying numbers (3, 10, 20) of prescription drugs insurance plans, where we supply subjects with realisitic information about each drug plans (total annual prescription drug costs and several aspects of benefits: whether drugs can be received by mail, the number of pharmacies in their area, distance to the closest pharmacy, deductible, and cost-sharing questions). Subjects answer both factual questions as well as subjective ones. Our second experiment studies the relationship between choice size and standardization of information. We explore this question within the context of long-term health care insurance. In this study we manipulate the number of insurance choices subjects face (5 vs. 15) and nature of the product (standardized benefits, vs. allowing companies to offer any configuration of benefits they like). We are particularly interested in the interaction between these two treatments, hypothesizing that subjects will be able to comprehend a larger choice set if benefits are standardized (as they currently are in the Medigap market). To control for confounders, in both experiments we also test participants' cognitive, numerical, and information processing abilities. We are currently carrying out the experiments; preliminary evidence presented at the meeting will show that elders are less facile as using health insurance information - although they show more confidence that they understand it. Moreover, the preliminary results do indicate that a higher number of choices reduces the ability to answer objective questions correctly.

Exploiting longitudinal claims data and the drastic reduction of coinsurance from 30% to 10% at age 70 in Japan, we find that the elderly's demand responses are heterogeneous in ways that have not been previously reported. Outpatient services by orthopedic and eye specialties, which will continue to increase in an aging society, are particularly price responsive and account for a large share of the spending increase. Lower cost sharing increases demand for brand-name drugs but not for generics. These high price elasticities may call for different cost-sharing rules for these services. Patient health status also matters: receiving medical services appears more discretionary for the healthy than the sick in the outpatient setting. Finally, we found no evidence that additional medical spending improved short-term health outcomes.

Book excerpt: We use the design of Medicare's prescription drug benefit program to demonstrate three facts about the health consequences of cost-sharing. First, we show that an as-if-random increase of 33.6% in out-of-pocket price (11.0 percentage points (p.p.) change in coinsurance, or $10.40 per drug) causes a 22.6% drop in total drug consumption ($61.20), and a 32.7% increase in monthly mortality (0.048 p.p.). Second, we trace this mortality effect to cutbacks in life-saving medicines like statins and antihypertensives, for which clinical trials show large mortality benefits. We find no indication that these reductions in demand affect only 'low-value' drugs; on the contrary, those at the highest risk of heart attack and stroke, who would benefit the most from statins and antihypertensives, cut back more on these drugs than lower risk patients. Similar patterns exist for other drug-disease pairs, and irrespective of socioeconomic circumstance. Finally, we document that when faced with complex, high-dimensional choice problems, patients respond in simple, perverse ways. Specifically, price increases cause 18.0% more patients (2.8 p.p.) to fill no drugs, regardless of how many drugs they had been on previously, or their health risks. This decision mechanically results in larger absolute reductions in utilization for those on many drugs. We conclude that cost-sharing schemes should be evaluated based on their overall impact on welfare, which can be very different from the price elasticity of demand.

Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.
