Frank Porporino and I arrived at the starting point for our work together by very different routes. Originally trained as an experimental psychologist, I had become in creasingly restive within the confines of the laboratory, and spent a sab batical year in the equivalent of a clinical internship. I then spent some time as a part-time consultant in a local penitentiary. Most of my time in the institution was spent with inmates with a variety of problems, probably about 50 individuals over the course of a year. Although this was far fewer than a full-time psychologist in the system might encounter, it served as a quick cram course on problem prisoners and prisoner problems. Very quickly my stereotypes about convicts were shown to be virtually useless. I learned that the criminal classes included all levels of society, and that the behavior of prisoners was the same as that of other human beings in a difficult environment.

The inmate's appraisal of the stressor was determined and the coping strategies employed were matched with the particular stressor to which the inmate was responding. Minority inmates reported the use of significantly more planning strategies and positive reinterpretation than European-American inmates, while European-American inmates reported the use of significantly more behavioral disengagement than minority inmates. Both mental disengagement and denial decreased with age. Separation from loved ones was the most commonly reported stressor throughout the inmates' incarceration; however, problems related to negative aspects of the prison environment, other inmates, and ambiguity of the situation increased with time. The use of spirituality was the most commonly reported coping strategy throughout the inmates' incarceration. Almost all other forms of coping increased with time, except for a decrease in maladaptive coping strategies.

Pallone characterizes opinion on these questions as falling into two broad camps: the "tender-hearted," those who see an overlap between mental illness and criminal behavior, and are treatment-oriented; and the "tough-minded," those who have little confidence in psychiatric categories, do not really accept arguments about diminished responsibility, and who feel the emphasis should be on punishment. Which is closer to the truth? When this book was first published, the incidence of mental disorder among prisoners was nearly four times greater than among comparable groups in the general population in part because prisoners are disproportionately drawn from demographic groups with a high incidence of mental disorder—nonwhite and from lower socioeconomic strata. But other data is equally dismaying: mental retardation is 50 percent higher; alcohol and drug abuse is between five and eight times greater; and neurogenic disorders may be 1,700 times greater. In all categories of mental illness, the incidence among prisoners is far higher than among the general population. Pallone asserts that evidence suggests that the design and implementation of mental health care needs serious reevaluation, particularly in view of Supreme Court decisions mandating mental health care despite obstacles with implementation. Palone saw mental health care as the primary issue for those who manage prisons. Sadly, this remains as true as when this book was first published.
