Available in PDF, EPUB and Kindle. Book excerpt: This book will consider several clinical and interventional topics for which there is uncertainty, continued debate and/or no consensus based on current guidelines. While there are numerous guidelines in cardiology, new and on-going trials that address new drugs treatments and procedures raise many unanswered questions. Furthermore, most practicing cardiologist taking care of the patients are likely unable to digest all of these studies or guidelines and necessarily correctly apply them to their patients. If one considers in the guidelines the frequent use of Level C (consensus), there are many areas or situations where no trial exists. Also, when applying the results of a trial to an individual patient, there can be uncertainty of how this patient should be managed based on the present body of evidence.

The primary intention is to describe common cardiac problems with significant controversial viewpoints frequently encountered in our daily practice. This book presents 19 chapters, including the Coronary Arteries in Fatal Coronary Events, Prophylactic Antiarrhythmic Therapy in Acute Myocar dial Infarction, Mobile Coronary Care-Is It Really Needed?, The Use of Artificial Pacemakers in Acute Myocardial Infarction, Treatment of Car diogenic Shock, Serum Digitalis Level-Practical Value, Factors Modifying the Efficacy of Digitalis, Hyperlipidemia and Vascular Disease, Antianginal Agents for Coronary Heart Disease, Anticoagulation Therapy for Coronary Heart Disease, When to Operate on Congenital Heart Diseases, Indications for Coronary Artery Surgery and Patient Selection, Car diomyopathy: Diagnostic Criteria and Classification, Therapeutic Approach to Idiopathic Hypertrophic Sub aortic Stenosis, Current Concepts of Hemiblocks, Physical Activity and Coronary Heart Disease, His Bundle Electrocardiography-Its Clinical Value, Computerized Electrocar diography-Its Practical Value, and Echocardiography-Its Practical Value. As the title of the book indicates, the "pro and con" viewpoints are described in each chapter, and the authors final conclusions are expressed at the end of each. Book excerpt: This book addresses the tough clinical issues faced by electrophysiologists and cardiologists who treat patients with Cardiac Implantable Electrical Devices (CIEDs) in real-world practice. With contributions from widely recognized international leaders in the field, this 10-chapter resource covers a variety of controversies with CIEDs, from discerning what device is appropriate to use for heart failure to ethical issues in their use at the end of a patient’s life. To supplement these discussions, chapters review opposing positions on both sides of a controversy and present clinical material to illustrate the different perspectives. Clinical Controversies in Device Therapy for Cardiac Arrhythmias is an essential resource not only for physicians, residents, and fellows in cardiac electrophysiology and cardiology but also for associated professionals including nurses and technicians who work with CIEDs.

They also delineate the surgical management of the heart patient with discussions of postoperative management and its complications and of specific surgical procedures such as coronary artery bypass grafting, valve surgery, pacemaker and defibibrillators, and surgical management of heart failure.

Available in PDF, EPUB and Kindle. Book excerpt: Resolving the Cholesterol Controversy recounts the science and scientific personalities behind the chain of discoveries upon which the lipid hypothesis of atherosclerosis was built. The narrative covers a period of just under 100 years, starting with Anitschkow's experiments with cholesterol-fed rabbits in 1913, and recounts the endeavours and achievements of the leading actors in this protracted scientific drama. The cast is drawn from an extraordinary variety of scientific disciplines: pathology, biophysics, epidemiology, nutrition, cardiology, lipidology, genetics, microbiology, pharmacology, and clinical trial design. Most of the scientists believed that cholesterol played a causal role in atherosclerosis and cardiovascular disease, but there were some who dissented strongly from this conclusion. The breadth of scientific disciplines involved in proving the lipid hypothesis is matched by the geographical spread of the participants. Anitschkow worked in Russia, Endo discovered the first statin in Japan, their commercial development by Merck took place in the USA and evidence of benefit from lowering cholesterol came from trials conducted in Scandinavia and the UK. The subsequent meta-analysis of these statin trials in 2005 proved the validity of the lipid hypothesis beyond any doubt. The history of how this all came about and its impact upon health policy and medical practice is recalled here in Resolving the Cholesterol Controversy.

Book excerpt: This series of four video tapes, which is based on the demonstrations pre-recorded by Robert H Anderson and Anton E Becker, two prominent European morphologists, shows the essence of the abnormal morphology within carefully selected autopsy specimens, comparing the findings with appropriately dissected normal hearts, and supplementing the anatomic material with diagrams and cartoons. Accompanying these tapes is an explanatory book prepared with extensive full colour illustrations based on the specimens and diagrams used in the videos and supplemented by appropriate material from the extensive files of the authors. The introductory chapter of the book discusses the background to sequential segmental analysis and the importance of the morphological myocardial method of recognizing chambers and arterial trunks in congenitally malformed hearts. The book is also available without the videos.Videos and book together address such crucial questions as:∗ How many segments need to be considered within the heart?∗ Is the myocardial morphologic method the best way of recognizing chambers?∗ Is the atrioventricular junction a common structure in the ostium primum defect?∗ Is there such a thing as isomerism of the atrial appendages?∗ What is the univentricular atrioventricular connection — and are there really any univentricular hearts?∗ Is the infundibulum in tetralogy of Fallot too narrow, too shallow, and too short? To answer these, and many more questions, Anderson and Becker alternately act as devil's advocate in the four videos, each lasting approximately 40 minutes. The individual videos are devoted to:Video and Chapter Titles:∗ Atrioventricular septal defects∗ Hearts with isomeric atrial appendages∗ Tetralogy of Fallot and double outlet right ventricle∗ Hearts with univentricular atrioventricular connection

It is ideal for any hospital doctor, generalist, or even senior medical student who may need a cardiology opinion, or for that ma.
